Observer Variability in BI-RADS Ultrasound Features and Its Influence on Computer-Aided Diagnosis of Breast masses

Objective: Computer classification of sonographic BI-RADS features can aid differentiation of the malignant and benign masses. However, the variability in the diagnosis due to the differences in the observed features between the observations is not known. The goal of this study is to measure the variation in sonographic features between multiple observations and determine the effect of features variation on computer-aided diagnosis of the breast masses. materials and methods: Ultrasound images of biopsy proven solid breast masses were analyzed in three independent observations for BI-RADS sonographic features. The BI-RADS features from each observation were used with Bayes classifier to determine probability of malignancy. The observer agreement in the sonographic features was measured by kappa coefficient and the difference in the diagnostic performances between observations was determined by the area under the ROC curve, Az, and interclass correlation coefficient. Results: While some features were repeatedly observed, κ = 0.95, other showed a significant variation, κ = 0.16. For all features, combined intra-observer agreement was substantial, κ = 0.77. The agreement, however, decreased steadily to 0.66 and 0.56 as time between the observations increased from 1 to 2 and 3 months, respectively. Despite the variation in features between observations the probabilities of malignancy estimates from Bayes classifier were robust and consistently yielded same level of diagnostic performance, Az was 0.772-0.817 for sonographic features alone and 0.828-0.849 for sonographic features and age combined. The difference in the performance, ΔAz, between the observations for the two groups was small (0.003-0.044) and was not statistically significant (p mbined with age was 0.833 (CI: 0.800-0.862). Increasing BmI Z-Scores 3 Years after Diagnosis among a multiethnic Cohort of Childhood Cancer Survivors Treated in South Los Angeles

Background: Due to successful treatment modalities, the majority of pediatric cancer patients will survive. Increased body mass index (BmI) is a complication among pediatric cancer survivors. methods: This retrospective single-center study examined BmI changes among a cohort of predominantly Hispanic patients who were treated in South Los Angeles. Data were collected at diagnosis, 1, 2 and 3 years after. Analyses included z-scores derived from calculated BmIs compared over 3 years per gender, diagnosis, and treatment modality. The unhealthy BmI z-score was defined as >1.04. Results: Thirty-four percent of the predominantly Hispanic sample had unhealthy BmI z-scores of >1.04 correlating to at or greater than the 85th percentile for age and gender. The study cohort’s BmI z-scores significantly increased from 0.15 to 1.29 at year 3 (P median BmI z-score significantly increased to the unhealthy category at 3 years. Conclusions: Due to the predominance of Hispanic patients in this group, culturally sensitive interventions beginning at diagnosis should be considered.

Recall of past activity levels in young adults is predictive of adolescent activity levels

Purpose: To determine whether a global physical activity question (GPAq) administered in young adulthood can be used to accurately rank former physical activity levels (PA) relative to peers of the same age and sex during adolescence. methods: Data were obtained from the Physical Activity in Young Adults Study, a 10- year follow-up study of 12 - 16 year old adolescents. Five hundred twenty-eight young adults, mean age 24.5 (±1.0) years, completed a GPAq regarding current and past (adolescent) PA. GPAq answers were used to determine whether young adults could accurately rank their past (adolescent) PA levels relative to peers of the same age and sex. GPAq responses were compared with actual self-reported PA levels assessed by the modifiable Activity Questionnaire for Adolescents (mAQ-A;collected during adolescence). For adolescent PA, an average of 4 years of activity from the mAQ-A 1990-1993, was used. Results: Regardless of gender, Jonckheere-Terpstra tests for trend (p mAQ-A. Higher perceived PA tracked with greater past PA activity and lower perceived PA tracked with lower past PA activity. Conclusions: Young adults who classified themselves as more active as an adolescent were found to be relatively more active based upon self-reported activity from recall questionnaires collected during that time. These findings suggest that young adults can reasonably estimate relative PA levels during their adolescent years with GPAq. Global physical activity questions may be valid for the assessment and classification of previous PA levels.

为准确衡量烟叶不同生长期及晾制过程中不同时期细菌数量的变化，采用8个白肋烟品种对生产过程的细菌总数进行了连续分离计数。采集烟株封顶时的顶叶、收获成熟叶片及收获调制后3周和5周的烟叶样品，经表面灭菌后，用捣碎机捣碎。然后用0．1％ Nacl系列稀释，涂布于TSA培养基上，在28～30℃的培养箱内分别培养24、48、72h后进行菌落观察和计数。结果表明，不同品种的细菌菌落在颜色、形态上的差异很大，细菌数量大约在干重102～106菌落形成单位（cfu）／g。在检测的2年中，1999年的细菌总数明显低于2000年，尤其在晾制过程中这一差别更为明显。

Factors associated with success of telaprevir-and boceprevir-based triple therapy for hepatitis C virus infection

To evaluate new therapies for hepatitis C virus (HCV), data about real-world outcomes are needed. mETHODSOutcomes of 223 patients with genotype 1 HCV who started telaprevir- or boceprevir-based triple therapy (may 2011-march 2012) at the mount Sinai medical Center were analyzed. Human immunodeficiency virus-positive patients and patients who received a liver transplant were excluded. Factors associated with sustained virological response (SVR24) and relapse were analyzed by univariable and multivariable logistic regression as well as classification and regression trees. Fast virological response (FVR) was defined as undetectable HCV RNA at week-4 (telaprevir) or week-8 (boceprevir). RESULTSThe median age was 57 years, 18% were black, 44% had advanced fibrosis/cirrhosis (FIB-4 ≥ 3.25). Only 42% (94/223) of patients achieved SVR24 on an intention-to-treat basis. In a model that included platelets, SVR24 was associated with white race [odds ratio (OR) = 5.92, 95% confidence interval (CI): 2.34-14.96], HCV sub-genotype 1b (OR = 2.81, 95%CI: 1.45-5.44), platelet count (OR = 1.10, per x 104 cells/μL, 95%CI: 1.05-1.16), and IL28B CC genotype (OR = 3.54, 95%CI: 1.19-10.53). Platelet counts > 135 x 103/μL were the strongest predictor of SVR by classification and regression tree. Relapse occurred in 25% (27/104) of patients with an end-of-treatment response and was associated with non-FVR (OR = 4.77, 95%CI: 1.68-13.56), HCV sub-genotype 1a (OR = 5.20; 95%CI: 1.40-18.97), and FIB-4 ≥ 3.25 (OR = 2.77; 95%CI: 1.07-7.22). CONCLUSIONThe SVR rate was 42% with telaprevir- or boceprevir-based triple therapy in real-world practice. Low platelets and advanced fibrosis were associated with treatment failure and relapse.

Automated Cell Detection and morphometry on Growth Plate Images of mouse Bone

microscopy imaging of mouse growth plates is extensively used in biology to understand the effect of specific molecules on various stages of normal bone development and on bone disease. Until now, such image analysis has been conducted by manual detection. In fact, when existing automated detection techniques were applied, morphological variations across the growth plate and heterogeneity of image background color, including the faint presence of cells (chondrocytes) located deeper in tissue away from the image’s plane of focus, and lack of cell-specific features, interfered with identification of cells. We propose the first method of automated detection and morphometry applicable to images of cells in the growth plate of long bone. Through ad hoc sequential application of the Retinex method, anisotropic diffusion and thresholding, our new cell detection algorithm (CDA) addresses these challenges on bright-field microscopy images of mouse growth plates. Five parameters, chosen by the user in respect of image characteristics, regulate our CDA. Our results demonstrate effectiveness of the proposed numerical method relative to manual methods. Our CDA confirms previously established results regarding chondrocytes’ number, area, orientation, height and shape of normal growth plates. Our CDA also confirms differences previously found between the genetic mutated mouse Smad1/5CKO and its control mouse on fluorescence images. The CDA aims to aid biomedical research by increasing efficiency and consistency of data collection regarding arrangement and characteristics of chondrocytes. Our results suggest that automated extraction of data from microscopy imaging of growth plates can assist in unlocking information on normal and pathological development, key to the underlying biological mechanisms of bone growth.
