Institutional analysis of intra-and post-operative tracheostomy management for risk reduction

Objectives:Determine variability in intra-and post-operative management of tracheostomies(trachs)at our institution as existing literature suggests that trachs are a frequent trigger for airway-related *** the development of an institution-wide protocols for trach ***:A 39-question online survey was sent to 55 providers who perform open and percutaneous trachs at three of the hospitals within our large,urban,academic medical *** providers were identified by surveillance of the operating room schedules for 1 ***:The survey was completed by 40 of the 53 eligible providers(75.5%).Response rate by question *** included members of all departments that perform trachs at our institution(Otorhinolaryngology,Trauma Surgery,Thoracic Surgery,General Surgery,Cardiovascular Surgery and Interventional Pulmonology).While most responses demonstrated uniformity in practice,notable variations included the following:80%of percutaneous trach providers stated that morbid obesity was not a contraindication to performing a trach outside of the operating room(n=20)while 58%of open trach providers stated that morbid obesity was a contraindication;only 35%of open trach providers perform a Bjork flap(n=350).The survey also identified significant variability in practice with regards to timing of trach suture ***:Lack of uniformity was identified in several practices related to intra-and post-operative tracheostomy *** did,however,trend toward consensus in many *** results are being used to establish a more consistent approach to tracheostomy management across our institution to ensure standardization of practice amidst the rapidly evolving practices of trach *** for practice:With ongoing evolution in the methods of trach placement and its management,the concepts put forth here will be a resource for health care providers at other institutions to consider intra-institutional analysis and

Physical activity during pregnancy and the role of theory in promoting positive behavior change:A systematic review

Background: Physical activity(pa) during pregnancy provides physical and psychological benefits for mother and child. U.S. guidelines recommend ≥30 min of moderate exercise for healthy pregnant women most days of the week; however, most women do not meet these recommendations. Theory assists in identifying salient determinants of health behavior to guide health promotion interventions; however, the application of theory to examine pa among pregnant women has not been examined cohesively among multiple levels of influence(e.g.,intrapersonal, interpersonal, neighborhood/environmental, and organizational/political). Subsequently, this systematic review aims to identify and evaluate the use of health behavior theory in studies that examine pa during ***: Articles published before July 2014 were obtained from Pub Med and Web of Science. Inclusion criteria applied were:(1) empiricallybased;(2) peer-reviewed;(3) measured factors related to pa;(4) comprised a pregnant sample; and(5) applied theory. Fourteen studies were included. Each study's application of theory and theoretical constructs were ***: Various theories were utilized to explain and predict pa during pregnancy; yet, the majority of these studies only focused on intrapersonal level determinants. Five theoretical frameworks were applied across the studies—all but one at the intrapersonal level. Few determinants identified were from the interpersonal, neighborhood/environmental, or organizational/political ***: This systematic review synthesized the literature on theoretical constructs related to pa during pregnancy. Interpersonal, community,and societal levels remain understudied. Future research should employ theory-driven multi-level determinants of pa to reflect the interacting factors influencing pa during this critical period in the life course.

This paper describes a model for rehabilitation professionals to consider when they de-sign new systems for delivering A ssistive Technology devices and services. Background: Many AssistiveTechnology(AT) devices to assist persons with disabilities in their daily life are already available in theWestern world. The large number of choices may confuse a rehabilitation professional. Where do you be-gin? How do you prescribe and recommend AT devices? New AT devices should meet the needs of peo-ple with disabilities. Because there are mayn different customs and each custom has environments for liv-ing and working, the devices in one culture may not be successful in another. How can engineers and prod-uct designers begin to consider these special needs? Method The guiding principle for prescribing or de-veloping AT devices is that professionals begin with the person who has a disability. First, the person whohas the disability must be respectfully asked about the actions they wish to be able to do although he orshe is unable to do because of impairments. Second, this information must be compared with professionalknowledge about the person’s missing or impaired performance skills, (e. g., missing limb, weakness, in-coordination, blindness or paralysis)and knowledge about the person’s strong performance skills in orderto devise a useful solution. Finally, with this information the rehabilitation professional or the product en-gineer can begin to solve the interesting puzzle of how to enable the person who is handicapped. A "dam-aged/performance skill only requires that the desired activity be accomplished in a different way. The so-lution is like a bridge that crosses a river.
