Background and Objectives: Views on living arrangement from elderly and informal caregivers are crucial to “ageing in place”. They might be related to the experience in the use of elderly care services, which remains inconclusive in previous literature. This study aimed to explore the association of previous experience in formal and informal long-term care services with views of both elderly and their informal caregivers on living arrangement. Research Design and Methods: This study adopted a cross-sectional design. Assessment records of Minimum Data Set-Home Care for community-dwelling elderly who were eligible for subsidized long-term care services in Hong Kong from 2004 to 2014 were made available. Multivariate logistic regression was applied to examine associations between both views on elder’s living arrangement from the elderly, caregivers and their previous informal caregiver support, and experience in formal care services. Results: 82,306 dyads of elderly and informal caregivers were included in the analysis. The elderly with previous use of home and community-based services (OR = 0.84, 95% CI 0.80 - 0.88) and informal caregivers (OR = 0.78, 95% CI 0.76 - 0.81) believed that the elderly should live away from home. Temporal trends of fewer elderly and caregivers supporting the idea of living away from home were also observed. Discussion and Implications: The results highlighted the importance of informal caregiver’s support and previous utilization of formal home and community-based services. It was concluded that resources and information of community-based care have a significant association with views on living at home also proper support services and training of care for the elderly should be made available to informal caregivers to reduce their burden.

Antralization at the edge of proximal gastric ulcers:Does Helicobacter pylori infection play a role?

AIM: To determine the prevalence of antralization at the edge of proximal gastric ulcers, and the effect ofH. pylori eradication on the mucosal appearances.METHODS: Biopsies were taken from the antrum, body and the ulcer edge of patients with benign proximal gastric ulcers before and one year after treatment. Gastric mucosa was classified as antral, transitional or body type.H. pylori positive patients receivedeither triple therapy, or omeprazole.RESULTS: Patients with index ulcers in the incisura, body or fundus (n=116) were analyzed. Antral-type mucosa was more prevalent at the ulcer edge inH. pylori-positive patients ***-negative patients (93% vs 60%, OR=8.95,95%CI: 2.47-32.4, P=0.001). At one year, there was a significant reduction in the prevalence of antralization (from 93 % to 61%, P=0.004) at the ulcer edge in patients with H. pyloribeing eradicated. However, there was no difference in the prevalence of antralization at the ulcer edge in those with persistent infection.CONCLUSION: H. pylori infection is associated with antralization at the edge of proximal gastric ulcers, which may be reversible in some patients after eradication of the infection.

BACKGROUND Liver cirrhosis is a major risk factor for hepatocellular carcinoma(HCC)development in chronic hepatitis B(CHB). Serum Mac-2 binding protein glycosylation isomer(M2 BPGi) is a novel serological marker for fibrosis. The role of M2 BPGi in prediction of HCC is *** To examine the role of serum M2 BPGi in predicting HCC development in hepatitis B e antigen(HBeAg)-negative *** Treatment-naive CHB patients with documented spontaneous HBeAg seroconversion were recruited. Serum M2 BPGi was measured at baseline(within3 years from HBeAg seroconversion), at 5 years and 10 years after HBeAg seroconversion and expressed as cut-off index(COI). Multivariate cox regression was performed to identify predictors for HCC development. ROC analysis was used to determine the cut-off value of M2 *** Among 207 patients(57% male, median age at HBeAg seroconversion 40 years old) with median follow-up of 13.1(11.8-15.5) years, the cumulative incidence of HCC at 15 years was 7%. Optical time-stretch imaging enables the continuous capture of non-repetitive events in real time at a line-scan rate of tens of MHz—a distinct advantage for the ultrafast dynamics monitoring and high-throughput screening that are widely needed in biological ***,its potential is limited by the technical challenge of achieving significant pulse stretching(that is,high temporal dispersion)and low optical loss,which are the critical factors influencing imaging quality,in the visible spectrum demanded in many of these *** present a new pulse-stretching technique,termed free-space angular-chirpenhanced delay(FACED),with three distinguishing features absent in the prevailing dispersive-fiber-based implementations:(1)it generates substantial,reconfigurable temporal dispersion in free space(41 ns nm^(−1))with low intrinsic loss(o6 dB)at visible wavelengths;(2)its wavelength-invariant pulse-stretching operation introduces a new paradigm in time-stretch imaging,which can now be implemented both with and without spectral encoding;and(3)pulse stretching in FACED inherently provides an ultrafast all-optical laser-beam scanning mechanism at a line-scan rate of tens of *** FACED,we demonstrate not only ultrafast laser-scanning time-stretch imaging with superior bright-field image quality compared with previous work but also,for the first time,MHz fluorescence and colorized time-stretch *** results show that this technique could enable a wider scope of applications in high-speed and high-throughput biological microscopy that were once out of reach.

BACKGROUND：In Shenzhen, the Emergency Medical Service （EMS） system has been in service since 1997. This study aims to examine the operation of Shenzhen 120 EMS center and to identify the reasons of calling ***：In this retrospective quantitative descriptive study, the data from the Shenzhen 120 EMS registry in 2011 were ***：Shenzhen 120 EMS center is a communication command center. When the number of 120 are dialed, it is forwarded to the closest appropriate hospital for ambulance dispatch. In 2011, the Shenzhen 120 EMS center received 153 160 ambulance calls, with an average of 420 calls per day. Calling emergency services was mainly due to traffic accidents. Trauma and other acute diseases constituted a majority of ambulance transports. The adult patients aged 15-60 years are the principal users of EMS. There are no recognized ＇paramedic＇ doctors and nurses. The pre-hospital emergency service is under the operation of emergency departments of hospitals. Shenzhen at present does not have specialized pre-hospital training for doctors and nurses in posttrauma management. Moreover, specialized pre-hospital training, financial support, and public health education on proper use of EMS should be ***：The Shenzhen 120 EMS center has its own epidemiology characteristics. Traumatic injury and traffic accident are the main reasons for calling ambulance service. In-depth study emphasizing the distribution and characteristics of trauma patients is crucial to the future development of EMS.

Objective: To develop an auto-segmentation method for delineating the biological tumor volume of nasopharyngeal carcinoma from positron emission tomography images. Methods: A phantom consisting of a water tank with fixed background fluorodeoxyglucose [18F-FDG] activity and spheres with diameters ranging from 2.1 to 5 cm with varying activities of FDG were used to simulate tumors of different sizes and FDG uptake. The phantom was scanned with a PET/CT scan at different sphere to background intensity ratios. An optimum fixed percentage threshold (FT) approach and a signal-to-background ratio (SBR) approach were developed to estimate the true size of the spheres from the PET images. Both approaches were further evaluated in patient images for validation. Twenty-two patients with NPC from stage T1 to T4 were included. The PET based biological tumor volumes (BTV) were delineated with both FT (BTVFT) and SBR (BTVSBR) approaches and compared with the gross tumor volume localized from MRI (GTVMR). The mean volumes of BTVFT and BTVSBR were compared and the degree of overlap between GTVMR and both BTVs was evaluated. Paired t-tests were used for statistical analysis. Results: The optimal FT value was 36.5% of maximal intensity, and SBR approach was represented by an inverse linear regression model. The estimated volume of spheres segmented by both approaches shows no significant difference from the true volume of spheres (p > 0.05), but the average absolute errors were smaller from SBR approach than FT approach (p = 0.008). GTVMR was larger than both BTVFT (p = 0.003) and BTVSBR (p SBR with GTVMR is significantly larger than with BTVFT (0.52 and 0.42 respectively, p MR for radiotherapy planning purpose.
