Distinct risk factors for early and late blood transfusion following pancreaticoduodenectomy

Background: The International Study Group of Pancreatic Surgery(ISGPS) has defined two periods of postpancreatectomy hemorrhage, early(24 h). A previously published Blood Usage Risk Score(BURS) aimed to predict early and late blood transfusion. The primary aim of this study was to define risk factors for early and late blood transfusion after pancreaticoduodenectomy. Secondary aims were to assess the predictive accuracy of the ***: In this retrospective observational study, multivariable analyses were used to identify independent risk factors for both early and late blood transfusion. The predictive ability of the BURS was then assessed using a receiver operating characteristic(ROC) curve ***: Among 628 patients, 99(15.8%) and 144(22.9%) received early and late blood transfusion, respectively. Risk factors for blood transfusion differed between early and late periods. Preoperative anemia and venous resection were associated with early blood transfusion whilst Whipple’s resection(as opposed to pylorus preserving pancreaticoduodenectomy), lack of biliary stent and a narrow pancreatic duct were predictors of late blood transfusion. The BURS was significantly predictive of early blood transfusion,albeit with a modest degree of accuracy(AUROC: 0.700, P < 0.001), but not of late blood transfusion(AUROC: 0.525, P = 0.360). Late blood transfusion was independently associated with increasing severity of postoperative pancreatic fistula(POPF)(OR: 1.85, 3.18 and 9.97 for biochemical, types B and C POPF,respectively, relative to no POPF).Conclusions: Two largely different sets of variables are related to early and late blood transfusion following pancreaticoduodenectomy. The BURS was significantly associated with early, albeit with modest predictive accuracy, but not late blood transfusion. An understanding of POPF risk allows assessment of the need for late blood transfusion.

On the evaluation of ALD TiO_(2),ZrO_(2) and HfO_(2) coatings on corrosion and cytotoxicity performances

Magnesium alloys have been widely studied as materials for temporary implants,but their use has been limited by their corrosion ***,coatings have been proven to provide an effective *** only little explored in the field,Atomic Layer Deposition(ALD)stands out as a coating technology due to the outstanding film conformality and density ***,we provide first insights into the corrosion behavior and the induced biological response of 100 nm thick ALD TiO_(2),HfO_(2)and ZrO_(2)coatings on AZ31 alloy by means of potentiodynamic polarization curves,electrochemical impedance spectroscopy(EIS),hydrogen evolution and MTS colorimetric assay with L929 *** three coatings improve the corrosion behavior and cytotoxicity of the ***,HfO_(2)coatings were characterized by the highest corrosion resistance and cell viability,slightly higher than those of ZrO_(2)***_(2)was characterized by the lowest corrosion improvements and,though generally considered a biocompatible coating,was found to not meet the demands for cellular applications(it was characterized by grade 3 cytotoxicity after 5 days of culture).These results reveal a strong link between biocompatibility and corrosion resistance and entail the need of taking the latter into consideration in the choice of a biocompatible coating to protect degradable Mg-based alloys.

Background:To compare the visual outcome and patients’satisfaction after ultrathin Descemet stripping automated endothelial keratoplasty(UT-DSAEK)and Descemet membrane endothelial keratoplasty(DMEK)performed on fellow eyes of the same ***:In this retrospective study,the records of 18 pseudophakic patients affected by Fuchs endothelial dystrophy who underwent DMEK in one eye and UT-DSAEK in the fellow eye were *** corrected visual acuity(BCVA),corneal pachymetry,keratometry,corneal aberrations,photopic and mesopic contrast sensitivity,and endothelial cell counts measured 12 months after surgery in either eye were analyzed and *** results of a satisfaction questionnaire were also ***:Twelve months after surgery,BCVA was not significantly different in UT-DSAEK and DMEK eyes(0.10±0.04 and 0.07±0.07 logMAR,respectively);at both 4-and 6 mm optical zones total and posterior corneal higher order aberrations(HOAs),posterior astigmatism and total coma were significantly lower after DMEK;BCVA in both groups was significantly correlated mainly with anterior corneal aberrations;contrast sensitivity was higher after DMEK especially in mesopic conditions and at medium spatial frequencies;the endothelial cell density was similar,although slightly higher in the UT-DSAEK group(p=0.10).The satisfaction questionnaire showed that although patients were highly satisfied from both procedures,more than half of them preferred DMEK and reported a more comfortable and quicker postoperative ***:DMEK and UT-DSAEK showed no evidence of difference in terms of postoperative BCVA,although DMEK had a better performance in terms of contrast sensitivity,posterior corneal aberrations and overall patient satisfaction.

Although the morphological features of hepatitis C virus(HCV)recurrence after orthotopic liver transplantation(OLT)have been well established in the last decades,the differential diagnosis still represents a challenge for the pathologist,especially early recurrent hepatitis C vs mild acute cellular *** present review focuses on the role of the pathologist and the pathology laboratory in the management of recipients with recurrent hepatitis C,the usefulness of early and late post-OLT liver biopsies,and the potential role of ancillary techniques(immunohistochemistry and reverse transcription-polymerase chain reaction,RT-PCR).The English literature on the topic is reviewed,focusing on the histopathology,the immunohistochemistry and the use of RT-PCR on HCV-positive post-OLT *** different histopathological illustrations of early and chronic recurrent hepatitis C are presented,with special focus on the main differential diagnoses and those features with prognostic relevance(cholestasis above all).The usefulness of ancillary techniques are discussed,especially HCV RNA quantitation by ***,the usefulness of long-term protocol biopsies is addressed:their usefulness for the study of allograft disease progression is clear,but their meaning in the long term is still *** significance of plasma cell infiltrate in HCV-positive allografts,the prognostic weight of graft steatosis,and the impact of donor age in recurrent hepatitis C also represent additional open issues.

Problematic Internet use in drug addicts under treatment in public rehab centers

BACKGROUND Problematic Internet use(PIU)or Internet addiction has been recognized to be a behavioral addiction characterized by excessive or poorly controlled preoccupations,urges,or behaviors regarding computer use and Internet access that leads to impairment or distress resembling substance *** To investigate the prevalence and characteristics of Internet use and abuse in a group of drug addicts from Southern Italy,by means of a specific questionnaire[“Questionario sull’Utilizzo delle Nuove Tecnologie”(QUNT)].MethODS All subjects(183)were heavy smokers,almost 50%of them used heroin and/or opioid compounds,30%alcohol,10%cannabis,8%cocaine,and 5%were polydrug *** 10%of the individuals were also suffering from gambling *** The time spent online was more than 4 hours a day in the total sample,with a slight prevalence in male *** and cannabis users spent more than 6 hours online,significantly more than opioid and alcohol *** of the QUNT factors was not different in both *** users showed higher scores at the“loss of control”,“pornography addiction”,and“addiction to social networks”factors,for the stimulant effect of this ***,15 out of the total 17 cocaine users were pathological *** and statistically significant relationships were observed between some QUNT factors and body mass *** These findings indicate that PIU is less severe in subjects taking sedative substances,such as heroin/opioids and alcohol,than in subjects taking ***,it may be used as a“stimulant”trigger in cocaine and cannabis *** effect of abuse drugs was noted on possible sex-related differences in QUNT *** observed a sort of“protective”effect of a love relationship and/or living together with a partner,as those engaged subjects showed lower scores on different items than single subjects or those living *** relationship between time spent online(and re
