Acute Metabolic Complications of diabetes in the Oueme-Plateau Provincial Teaching Hospital: Epidemiological, Clinical, Paraclinical and Evolutionary Aspects

Introduction: diabetes mellitus, a metabolic disease, is now an important public health problem across the world. Our aim was to study the epidemiological, clinical, paraclinical, and evolutionary aspects of acute metabolic complications (AMC) of diabetes in the endocrinology department of the Oueme-Plateau Provincial Teaching Hospital. Methods: This was a descriptive cross-sectional study with data collection based on the medical records of patients presenting with acute metabolic complications of diabetes in the endocrinology department of the Oueme-Plateau Provincial Teaching Hospital during the 3 years study period (from January 2020 to december 2022). Results: Over 788 patients hospitalized during the study period, 157 had an acute metabolic complication of diabetes, which is a hospital prevalence of 19.9%. Among these 157 cases, 140 were suitable for analysis and therefore constituted our study sample. The mean age of the patients was 49.9 ± 14.7 years, with 19 as minimum and 90 years as maximum. Most of them were women (52.4%). Hypertensive diabetics accounted for 54.9% of cases. Ketoacidosis was the most common complication (62.1%), followed by hyperosmolar hyperglycemic syndrome (23.6%) and hypoglycemia (14.3%). Infection (69.3%) and therapeutic noncompliance (22.9%) were the most common triggers. The average length period of their hospital stay was 7.1 ± 5.1 days, and the outcome was satisfactory in 87.9% of the cases. Unfortunately, there were 3.6% deaths during hospitalization. Conclusion: Acute metabolic complications of diabetes were dominated by ketoacidosis. Infection was the dominant factor in decompensation. Therapeutic education of diabetic patients as well as clinical and biological monitoring must be more stringent and rigorous.

Phytochemical Screening, Study of Acute Toxicity and Hypoglycemic and Antihyperglycemic Activities of Silver Nanoparticles from Guibourtia tessmannii (Harms) J. Leonard (Caesalpiniaceae)

According to the World Health Organization, diabetes affects approximately 347 million people worldwide. Its management is not within the reach of all social classes, therefore medicinal plants are still the first resort for many populations in Africa. The biological material used in this study was the trunk bark of Guibourtia tessmannii. 50 g of trunk bark powder were decocted in 500 ml of distilled water for 5 minutes were carried out. The method used for the synthesis of silver nanoparticles (AgNps) was an organometallic bio-reduction of silver nitrate salts mediated by various secondary metabolites contained in the plant extract. The study of the toxicity acute was conducted according to guideline 423 of the OECd protocol. The pharmacological activities were each carried out with 28 female rats divided into 7 groups of four rats. It was a question for the hypoglycemic activity of administering various doses of silver nanoparticles and other substances to the rats thirty minutes after the carbohydrate intake and for the anti-hyperglycemic activity of administering the same substances to the rats thirty minutes before the carbohydrate intake. The extraction yield was 8.76%. Only the alkaloid test was negative. After acute toxicity study, the Ld50 was greater than 2000 mg/kg. Blood sugar tests revealed that glibenclamide 5 mg/kg, which is the reference molecule, lowered blood sugar more than the other treatments applied in the other batches. It was followed by treatment with silver nanoparticles at a dose of 400 μg/kg in both tests. It was therefore concluded that silver nanoparticles from G. tessmannii are good for the formulation of improved traditional medicines and bring up their afficacity .

Elevated fibroblast growth factor 23(FGF23)in X-linked hypophosphatemia(XLH)results in rickets and phosphate wasting,manifesting by severe bone and dental ***,a FGF23-neutralizing antibody,an alternative to conventional treatment(phosphorus and active vitamin d analogs),showed significant improvement in the long bone ***,we examined whether FGF23 antibody(FGF23-mAb)also improved the dentoalveolar features associated with ***-week-old male Hyp mice were injected weekly with 4 or 16 mg·kg−1 of FGF23-mAb for 2 months and compared to wild-type(WT)and vehicle(PBS)treated Hyp mice(n=3–7 mice).Micro-CT analyses showed that both doses of FGF23-mAb restored dentin/cementum volume and corrected the enlarged pulp volume in Hyp mice,the higher concentration resulting in a rescue similar to WT ***23-mAb treatment also improved alveolar bone volume fraction and mineral density compared to vehicle-treated *** revealed improved mineralization of the dentoalveolar tissues,with a decreased amount of osteoid,predentin and *** periodontal ligament attachment was also observed,evidenced by restoration of the acellular *** preclinical data were consistent with the retrospective analysis of two patients with XLH showing that burosumab treatment improved oral *** together,our data show that the dentoalveolar tissues are greatly improved by FGF23-mAb treatment,heralding its benefit in clinics for dental abnormalities.

Turkey wattle is a by-product generated during slaughter,which has a large amount of connective tissue and is a potential source of glycosaminoglycans(GAGs).This study aimed to extract GAGs from the turkey wattle(Meleagris gallopavos)and investigate its anticoagulant activity,and its cytotoxic and antiproliferative activity in tumor *** sulfate(CS-4 and CS-6)and dermatan sulfate(dS)present in the F-VII fraction and hyaluronic acid(HA)in the F-V fraction were *** HA showed a high molecular weight(1.0×10^(6) da)and an apparent cytotoxic/antiproliferative effect against human hepatoma cells.F-VII fraction has an apparent cytotoxic/antiproliferative effect against renal adenoma cells and anticoagulant potential in *** results achieved in this study demonstrated that wattle,a by-product of turkey industrialization,has the potential to be valued as a raw material for the extraction of GAGs with bioactive properties.

Effect of ezetimibe on the prevalence of cholelithiasis

AIM:To investigate the prevalence of cholelithiasis among patients treated with ***:A retrospective,case-control study based on computerized medical records from patients of the Clalit Health Services,Sharon-Shomron region,from 2000 to *** 20-85 years of age,who had been treated with ezetimibe and statins or statins only for at least 6 mo,and who had an abdominal ultrasound were included in the *** data included age,gender,ezetimibe treatment duration,presence of hypothyroidism or diabetes,and existence of cholelithiasis as determined by *** were subjects after gallbladder resection,with hemolysis,myeloproliferative or inflammatory bowel diseases,and those treated with ursodeoxycholic acid and *** treated with statins and ezetimibe(study group) were compared to patients treated with statins only(control group).RESULTS:The study group included 25 patients and the control group *** patients in the study were treated with *** study group included 13 males(52%) and 12 females(48%),the control group 76 males(45%) and 92(55%) females(P = 0.544).The groups did not differ in age(mean age:68 ± 8 years,range 53-85 years vs mean age:71 ± 8 years,range 51-85 years;P = 0.153) or in the rate of diabetic and hypothyroid patients [11(44%) vs 57(33%),P = 0.347 in the study group and 5(20%) vs 23(14%),P = 0.449 in the control group,respectively].Patients in the study group were treated with ezetimibe for an average of 798 ± 379 *** was found in 4(16%) patients in the study group and in 33(20%) patients in the control group(P = 0.666).CONCLUSION:Ezetimibe does not appear to influence the prevalence of gallstones.

Antarctic Sea Ice Concentration in the Brazilian Earth System Model Simulations

Sea ice is an important and complex component of the Earth’s system, acting as both an indicator and an amplifier of climate change. Here, we investigated the ability of the Brazilian Earth System Model (BESM-OA2.5) and four state-of-the-art climate models participating in the fifth phase of the Coupled Model Intercomparison Project, Version 5 (CMIP5) to represent the Antarctic Sea Ice Concentration (SIC) seasonal cycle. We validated the sea ice model’s performance using satellite data from 1980 to 2005 and calculated the skill and RMSE of each model. BESM-OA2.5 results for melt-freeze transitions in the Southern Ocean are consistent with CMIP5 models and satellite data. In February, when the sea ice reaches its annual minimum, the BESM-OA2.5 has the best fit among the models. However, in September, when the Antarctic sea ice reaches its annual maximum, the SIC simulated by BESM-OA2.5 indicated the largest area covered by ice compared to satellite, particularly on the Polar Front. Similar results were found in the CMIP5 models evaluated here. We suggest that the large bias simulated in the Polar Front is related to the inability of the sea ice model to represent the complex ocean-atmosphere-sea ice interactions. The subject is considered a hot topic in climate change studies and lacks conclusive answers.

Background: Of the cancers diagnosed in Rwanda, stomach cancer is one of the most encountered. In fact, Rwanda belongs to the region where it is most incident in Africa. Most of the patients present with advanced disease. Studies showed that some gastric cancers overexpress Human Epidermal Growth Factor Receptor 2 (HER2/neu) protein and can be treated with Herceptin/Tras-tuzumab. This targeted therapy improves survival in patients with advanced disease. We conducted a study on Immunohistochemical profile of HER2/neu in gastric adenocarcinomas at two main Rwandan tertiary centers. Methodology: We tested for HER2/NEU in gastric adenocarcinomas diagnosed at University Teaching Hospital of Kigali (CHUK) and University Teaching Hospital of Butare (CHUB). demographic and pathologic parameters were collected. Immunohistochemistry (IHC) for HER2/neu using c-erb/HER-2/neu (clone SP3) Rabbit Monoclonal antibody was done. Using the guidelines established by Hoffman et al., the agreed score between 2 Rwandan pathologists and 1 USA pathologist was considered each time. data were entered and statistically analyzed using SPSS 22. descriptive statistical analysis method was used. P-value calculated with Chi-square analysis for positive vs negative and equivocal negative to correlate HER2/neu overexpression with other variables across both hospitals. Results: A total of 286 cases were tested. HER2/neu overexpression (score 3+ or positive) was found in 29 cases (10.1%). 8 cases (2.8%) were equivocal negative (score 2+) while 249 cases (87.1%) were negative (score 0 and 1+). Conclusion: HER2/neu is overexpressed in a subset of gastric cancers in Rwanda, a phenomenon that has been reported in other areas of the world. Testing for HER2/neu could identify patients who would get a targeted treatment with Herceptin.
