Conservative Surgery for Subungual Melanoma In Situ Using Matriderm^(■)

Background: Standard treatment for subungual melanoma is wide local excision including digit amputation in order to obtain safety margins. Level of amputation has been discussed in order to achieve appropriate oncological results along with preservation of limb function wherein local excision of subungual melanoma in situ without amputation is an option. Methods: We report two cases of subungual melanoma in situ treated by using a conservative approach. Both cases were treated with local excision with the resected tumor defect covered with an artificial dermis, Matriderm?. One case was of a 65-year-old male with a left thumb subungual melanoma in situ and another of a 41-year-old female with the same type of melanoma in the left hallux. Full-thickness skin graft was used for reconstruction after 7 weeks from the first surgery in the latter, and in the former case re-epithelization occurred within 90 days after surgery. To date, no clinical signs of recurrence have been found, and finger/toe functions were completely preserved with a minimized scar and little graft bed sensitivity. Follow-up time is two years and ten months for the left thumb case, and one year and six months for the left hallux subungual melanoma in situ. Conclusion: We present two cases of subungual melanoma in situ treated with digit sparing surgery including excision of the periosteum followed by the use of an artificial dermal template to guarantee coverage of the tumor bed defect, providing good functional and cosmetic outcome.

Scalp cooling is a widely used method to prevent chemotherapy-induced transient alopecia. Although the success rates in maintaining hair can be high, authors have expressed their concerns regarding the risk of scalp metastasis after scalp cooling. We report a case of breast cancer scalp metastasis in a patient submitted to scalp cooling for alopecia prevention and discuss the physiopathology, risks and benefits of this treatment for patients under this treatment.

This study aimed to investigate the trends of finishers in the Sao Silvestre race in Brazil,taking into account sex,age,and performance levels.A total of 31775 runners(women,n=13847;men,n=17928),aged(45.2±16.8)years,finishers in the Sao Silvestre race between 2007 and 2021,were considered in the present ***(event year,date of birth,sex,and race times)were downloaded from the official race *** man-Whitney U test,Spearman correlation,and robust regression model were *** increased over time for both *** age groups,“31-40 years”(women)and“>60 years”(men)were those with the highest number of *** found a decrease in performance across the years(β=2.45;p<0.005),as well as significant differences in race times for both sexes(U=42.844;p<0.001),with men presenting better performances than *** time,it was observed an increase in the performance gap between the sexes,but in general,the performance decreased(β=1.76;p<0.001).Stakeholders should consider improving the strategies to improve women and young people's participation in running events.

Optimization and Evaluation of Biosurfactant Produced by Pantoea sp. Using Pineapple Peel Residue, Vegetable Fat and Corn Steep Liquor

In this study, the authors have investigated the potential of a bacterial strain of Pantoea sp., isolated from wastewater of the textile industry, for the production of biosurfactant. The biosurfactant production was optimized by the combination of CCD (central composite design) and RSM (response surface methodology). To assess the effects and interactions of medium the vegetable fat (1.5, 2.0 and 2.5 v/v), the variables corn steep liquor (2.0, 5.0 and 8.0 v/v) and pineapple peel residue (10.0, 25.0 and 40.0 v/v) on the surface tension were evaluated. The empirical model developed through RSM in terms of the effective operational factors mentioned above was found to be adequate to describe the biosurfactant production. Compositional analysis of the produced biosurfactant has been carried out by FT-IR (Fourier transform infrared spectoscopy) and subjected to the test of removing hydrocarbons. Through the analysis, vegetable fat and pineapple peel residue were found to be the most significant factors, whereas corn steep liquor had less effect within the ranges investigated. A maximum reduction in surface tension of 30.00 mN/m was obtained under the optimal conditions of 2.0% (v/v) vegetable fat concentration, 5.0% (v/v) corn steep liquor and 25.0% (v/v) pineapple peel residue concentration of medium. FT-IR spectrometer analysis of the biosurfactant characterized it as a glycolipid derivative. The biosurfactant exhibited the ability to solubilize the hydrocarbons tested, working between 64% and 92%. According to consists of bars with a length proportional to the absolute value of the estimated effects divided by the standard error.
