The Effect of Longitudinal Stretching of Muscles and Nerve versus Deep Transverse Friction Massage in the Management of Patients with Carpal Tunnel Syndrome

Background and Purpose: Carpal tunnel syndrome (CTS) is a common source of hand numbness and pain. The goal of this study was to compare longitudinal stretching of muscles and nerve to deep transverse friction massage for decreasing pain and improving hand function in patients with chronic CTS. Subjects and Methods: Thirty patients, 25 to 40 years old, were included in this study and randomized into two groups: the first group (36.8 ± 0.262) followed a physical therapy program group receiving stretching of wrist and hand flexors in combination with ultrasound, strengthening exercises of wrist and hand flexors, and wrist splint, three sessions per week for 12 weeks;and the second group (35.9 ± 0.225) followed the physical therapy program of deep transverse friction massage on the carpal tunnel site at the wrist joint, with using ultrasonic therapy, strengthening exercises of wrist and hand flexors and wrist splint, three sessions per week for 12 weeks. Outcome measures were range of motion (ROM), pain using visual analog scale (VAS), and hand grip strength measured with a sphygmomanometer. Results: There was a significant difference in both groups in all outcome measures compared to baseline. Over stretching group showed significant difference at the end of study in terms of range of motion of wrist flexion, extension and strength of handgrip when compared to deep friction massage group P < 0.05. However, there is no significant difference between groups in pain. Discussion and Conclusion: This study showed that the effect of stretching exercises is more beneficial than deep transverse friction massage in the treatment of patients with CTS in decreasing the compression of the median nerve in the tunnel.

Nano-sized Ni particles on TiO2-SiO2 were synthesized by the two methods of photo-assisted deposition(PAD) and impregnation.H2,which is a promising energy carrier,with a low CO concentration was produced by the photocatalytic reforming of glucose(a model biomass) on the Ni/TiO2-SiO2 *** supported Ni enhanced the rate of H2 production while it suppressed CO *** catalysts were characterized by X-ray diffraction,X-ray absorption fine structure,transmission electron microscope,and nitrogen adsorption *** H2 production and CO selectivity were strongly dependent on the preparation method,and PAD-Ni/TiO2-SiO2 was the better catalyst for H2 production with the lowest CO concentration.

Tumor characteristics of hepatocellular carcinoma after direct-acting antiviral treatment for hepatitis C: Comparative analysis with antiviral therapy-naive patients

BACKGROUND Insufficient and contradictory data are available about the relation between directacting antivirals(DAAs)and hepatocellular carcinoma(HCC)development in patients with hepatitis C virus(HCV).AIM To analyze differences in basic clinical,radiological,and laboratory characteristics in addition to tumor behavior upon HCC diagnosis between patients with and without a previous history of DAAs *** This multicenter case-control study included 497 patients with chronic HCVrelated HCC,allocated into one of two groups according to their history of antiviral treatment for their *** Group I included 151 HCC patients with a history of DAAs,while 346 patients who had never been treated with DAAs were assigned to group II.A significant difference was observed between both groups regarding basic assessment scores(Child,MELD,and BCLC),which tended to have more advanced liver disease and HCC stage upon diagnosis in group ***,serum albumin was significantly affected,and serumα-fetoprotein was significantly higher in group II(P<0.001).In addition,group I showed significant HCC multicentricity than group II,while the incidence of portal vein thrombosis was significantly higher in group I(P<0.001).CONCLUSION The basic clinical scores and laboratory characteristics of HCC patients are advanced in patients who are naïve to DAAs treatment;however,HCC behavior is more aggressive in DAA-treated patients.

Application of Statistical Design Strategies to Optimize the Preparation of Cuo Nanoparticles by Hydrothermal Technique

Synthesis of CuO nanoparticles by hydrothermal technique in presence of cetyltrimethylammonium bromide (CTAB) as surfactant was carried out by statistically designed experiments based on Box Behnken method. Reaction parameters as time, temperature and surfactant concentration have been studied to show their effect on CuO particle size and morphology. The results of experimental design indicate that the surfactant concentration, reaction time and temperature were significant in. CuO particles were characterized using XRD and SEM. These work findings showed that CuO nanoparticles were formed at 100oC. On other hand, their crystallinity was improved with rising temperature from 100 to 200oC to achieve particle size of CuO in the range of 49-92 nm.

In Vitro Activity of Lawsonia inermis （Henna） on Some Pathogenic Fungi

The present study was conducted to investigate antifungal activity ofLawsonia inermis （Henna plant）. Leaf samples of the plants were collected from Eastern Nile of Khartoum state, Sudan. Ethanol and petroleum ether extracts in various concentrations were obtained by maceration （cold method）. The extracts were bioassay in vitro to know their bioactivity to inhibit the growth of tested fungi. The cup plate agar diffusion method was adopted to assess the antifungal activity of the extracts against tested yeasts while agar incorporated method was used for other molds. Both extracts revealed anti fungal activity against all yeast strains except Pichiafabianii which was found resistant to both ethanol and ether extracts. The results displayed antifungal activity against tested fungi. Minimum mould concentration （MMC） of the extracts was determined. The obtained results revealed antifungal activity of henna leaves extracts which support the traditional use of the Henna in therapy of fungal infections. The possibility of therapeutic use of Sudanese henna as antifungal agents is recommended.

Side Effects of Pfizer-BioNTech COVID-19 Vaccine among Libyan Young Adults: Observational Study

Purpose: Since the mRNA technology used to manufacture Pfizer-BioNTech COVID-19 Vaccine is new, it is still difficult to predict its consequences. However, many young people, especially in developing countries, think that they are less susceptible to severe infection with the Coronavirus 2 (SARS-CoV-2), Therefore, they do not prefer to take the COVID-19 vaccines. Accordingly, studies in this aspect are necessary to increase people’s awareness of the acceptance of vaccination, especially for the youth. Patients and Methods: This study was conducted on healthy young adults of both sexes. Participants (n = 81) were received two doses of the Pfizer-BioNTech COVID-19 Vaccine. A paper questionnaire was prepared, the survey asks about the side effects related to receiving the Vaccine. All participants completed the questionnaire twice, once after the first dose and again after the second dose. Data were collected and statistically analyzed. Results: The most common side effects were Muscle pain, Tiredness, Headache, and Fever. Less common side effects were Chills, Fainted, Breathlessness, and Coughing. Rare side effects include nausea and vomiting, Diarrhea, Anaphylactic Reaction, and Swollen Lymph Nodes. Side effects that are strongly associated with females are Headache, and Joint pain, for males, Anaphylactic reaction. The study revealed a significant difference between the number of females (96.5%) and males (85%) who suffered from the side effects of the Pfizer-BioNTech COVID-19 Vaccine (P = 0.033). Conclusion: Most of the side effects reported in our study were consistent with Pfizer’s fact sheet for recipients and caregivers. Through our follow-up to the participants in the study, we noticed that the reported side effects were not too bad, and they were able to carry out their daily activities, so we recommend taking the vaccine, especially for young people.

Antibiotic Susceptibility Patterns of Isolated Bacteria from Otitis Media in Children at mohamed Aden Sheikh Children Teaching Hospital in Hargeisa, Somaliland

Background: Otitis media (OM) is a group of inflammatory diseases of the middle ear. OM is a prevailing problem among children in Hargeisa. The antibiotic susceptibility of etiologic bacteria is not investigated in Somaliland which hinders the effective treatment of OM cases in children. Objective: This study aimed at determining the etiologic bacteria and its antibiotic susceptibilities in children presenting with OM to a pediatric referral hospital in Hargeisa for the period March 2013-May 2017. Methods: A cross-sectional retrospective study was conducted on a random sample of 270 children with OM. The laboratory used standard microbiological techniques for bacterial isolation and Kirby Bauer disk diffusion method for antibiotic susceptibility testing. Data were entered and analyzed using Epi Info 7 and any associations among the study variables tested with Chi2 test with confidence level of 95% and p value of Results: The rate of bacterial isolation was 96.3%. The predominant bacterial isolate was S. aureus (31.48%) followed by P. aeruginosa (24.81%) and P. mirabilis (15.93%) respectively while the least prevalent isolates were coagulase negative Staphylococcus (1.48%), S. pyogenes (0.74%) and Enterobacter spp. (0.37%) in descending order. Age group 0 - 3, χ2 (143,270 = 223.245, p = 0.000) showed highest bacterial isolation. There was no significant relationship between bacterial isolate and gender, χ2 (11,270 = 9.2283, p = 0.6008). S. aureus showed highest sensitivity towards ciprofloxacin (85.7%), amikacin (76.5%), and gentamicin (73.8%). All isolates showed mixed resistance pattern. Conclusion: S. aureus, P. aeruginosa and P. mirabilis were the leading causative pathogens of otitis media. No association was established between isolate distribution and gender. Background: Parkinson’s disease（PD） is the second most common neurodegenerative disease. Increasing evidence has demonstrated that neuroinflammation is a prominent pathological hallmark of PD. Based on this, modulation of inflammation in the brain can be offered as one of the therapeutic approach to control neuronal damage in PD. Objective: The purpose of this study was to study the effect of the brain preconditioning with single low dose of LPS on inflammatory profile in 6-OHDA model of PD. Methods: Before administration of 6-OHDA, animals in preconditioning group was pretreated with low dose of LPS, while vehicle group rats were preconditioned with normal saline. To clarify the neuroprotective effect of single brain LPS preconditioning, motor coordination and balance were evaluated using rotarod. In addition, the effect of the LPS preconditioning on neuronal damage were observed in the substantia nigra by using nissl staining. Finally, gene and protein expressions associated with different cascades induced by LPS preconditioning including TLR4 and inflammatory signaling pathways were analyzed using RT-PCR and Western Blot. Results: Based on the behavioral assessments, preconditioned animals performed significantly better on the behavioral performance compared to the vehicle group. Furthermore, neuroprotective effect of brain LPS preconditioning were consistent with the histological observation. In addition, our result showed that the brain LPS preconditioning reduced inflammatory response in the brain of PD rat by enhancing NFκB inhibitors and anti-inflammatory cytokines. Conclusion: Generally, reduction in inflammatory response through the brain LPS preconditioning may contribute to the induction of tolerance to neuronal damage in PD. This neuroprotection parallel the reprogramming strategy that leads to the synthesis of new markers to change molecular response against brain lesions. Breast and Ovarian Cancer in Young Women of the Arabian Gulf Region: Relationship to Age

It is widely known that cancer is a disease of “old-age”. However available data show that this is not the case for many types of cancers. Incidences of breast and ovarian cancers have varying rates of change with age. Breast cancer data of Arabian-gulf women, show that the incidence rates increase with age and reach a maximum at 39 year. It then declines linearly with age to about 55 years. The rate of increase and its changes with age are similar to those of many other countries. In the premenopausal phase the relationship between incidence and age could be adequately modeled using a linear model for the logarithmic transformations of age and incidence. Similar observations are made for the ovarian cancer incidences. Results: It is shown that the rate of increase in breast and ovarian cancer incidence with respect to age is increasing in the premenopausal ages. Moreover, the burden of the disease with respect to mortality and “Disability Adjusted Life Years” or DALY, varied considerably among the six gulf countries. Conclusions: We conclude, based on the age incidence relationship that the number of cancer cases may double in the next period that follows our study period (1998-2009). Moreover, if the six countries have identical relationship between age and the two types of cancer, there should be an integrated and unified effort to have a common strategy for prevention and control.
