BACKGROUND High-sensitivity cardiac troponin(hs-ctn)levels are frequently elevated in elderly patients presenting to the emergency department for non-cardiac ***,most studies on the role of elevated hs-ctn in elderly populations have investigated the prognostic value of hs-ctn in patients with a specific diagnosis or have assessed the relationship between hs-ctn and *** to investigate the in-hospital prognosis of consecutive elderly patients admitted to the Internal Medicine Department with acute non-cardiac events and increased hs-ctnI *** In this retrospective study,we selected patients who were aged≥65 years and admitted to the Internal Medicine Department of our hospital between January 2019 and December 2019 for non-cardiac *** patients were those who had hs-ctnI concentrations≥100 ng/*** investigated the independent predictors of in-hospital mortality by multivariable logistic regression *** One hundred and forty-six patients(59%female)were selected with an age range from 65 to 100(mean±SD:85.4±7.61)*** median hs-ctnI value was 284.2 ng/*** 72(49%)patients the diagnosis of hospitalization was an infectious *** overall in-hospital mortality was 32%(47 patients).Individuals who died did not have higher hs-ctnI levels compared with those who were discharged alive(median:314.8 vs 282.5 ng/L;P=0.565).there was no difference in mortality in patients with infectious vs non-infectious disease(29%vs 35%).Multivariable analysis showed that age(OR 1.062 per 1 year increase,95%CI:1.000-1.127;P=0.048)and creatinine levels(OR 2.065 per 1 mg/dL increase,95%CI:1.383-3.085;Pthe only independent predictors of *** was 49%in patients with eGFRt condition in elderly patients admitted to the hospital for non-cardiac *** presence of severe renal impairment is a marker of extremely high in-hospital mortality.

Probing top-quark couplings indirectly at Higgs factories

We perform a global effective-field-theory analysis to assess the combined precision of Higgs couplings, triple gauge-boson couplings, and top-quark couplings, at future circular e＋e- colliders, with a focus on runs below the tt-production threshold. Deviations in the top-quark sector entering as one-loop corrections are consistently taken into account in the Higgs and diboson processes. We find that future lepton colliders running at center-of-mass energies below the tt production threshold can still provide useful information on top-quark couplings, by measuring virtual top-quark effects. With rate and differential measurements, the indirect individual sensitivity achievable is better than at the high-luminosity LHC. However, strong correlations between the extracted top-quark and Higgs couplings are also present and lead to much weaker global constraints on top-quark couplings. this implies that a direct probe of top-quark couplings above the t~ production threshold is also helpful for the determination of Higgs and triple-gauge-boson couplings. In addition, we find that below the e＋e- -＋tth production threshold, the top-quark Yukawa coupling can be determined by its loop corrections to all Higgs production and decay channels. Degeneracy with the ggh coupling can be resolved, and even a global limit is competitive with the prospects of a linear collider above the threshold. this provides an additional means of determining the top-quark Yukawa coupling indirectly at lepton colliders.

Dysbetalipoproteinemia is a rare familial dyslipidemia characterized by approximately equally elevated serum cholesterol and triglyceride levels due to accumulated remnant lipoproteins in apolipoprotein E2/E2 *** is associated with an increased risk for premature cardiovascular disease. thus, making a diagnosis of dysbetalipoproteinemia aids in assessing cardiovascular risk correctly and allows for genetic counseling. However, the diagnostic work-up can be challenging. Diagnosis of dysbetalipoproteinemia should be considered in patients mixed dyslipidemia when the apolipoprotein B concentration is relatively low in relation to the total cholesterol concentration or when there is significant disparity between the calculated low density lipoprotein(LDL) and directly measured LDL cholesterol concentrations. Other indices are also informative in the diagnostic process. We present herein two phenotypically different cases(a 44-year-old man with severe hypertriglyceridemia and a 49-year-old woman with mixed dyslipidemia) of genotypically proven familial dysbetalipoproteinemia and a diagnostic algorithm of the disease.

Epicardial adipose tissue is defined as a deposit of adipocytes with pathophysiological properties similar to those of visceral fat, located in the space between the myocardial muscle and the pericardial sac. When compared with subcutaneous adipose tissue, visceral adipocytes show higher metabolic activity, lipolysis rates, increased insulin resistance along with more steroid hormone receptors. the epicardial adipose tissue interacts with numerous cardiovascular pathways via vasocrine and paracrine signalling comprised of pro-and anti-inflammatory cytokines excretion. Both the physiological differences be-tween the two tissue types, as well as the fact that fat distribution and phenotype, rather than quantity, affect cardiovascular function and metabolic processes, establish epicardial fat as a biomarker for cardiovascular and metabolic syndrome. Numerous studies have underlined an association of altered epicardial fat morphology, type 2 diabetes mellitus(t2 DM) and adverse cardiovascular events. In this review, we explore the prospect of using the epicardial adipose tissue as a therapeutic target in t2 DM and describe the underlying mechanisms by which the antidiabetic drugs affect the pathophysiological processes induced from adipose tissue accumulation and possibly allow for more favourable cardiovascular outcomes though epicardial fat manipulation.

this paper analyses the impact of geopolitical risks and economic policy uncertainty on stock *** uses an unbalanced panel dataset of monthly observations for 22 countries for the period 1985–*** controls for a set of macroeconomic and market structure variables while also taking into consideration the poten-tial effects of the 2007–2009 financial *** paper shows that the impact of geopolitical risks is negative and statistically *** economic interpretation of our main results is that a one-unit standard deviation increase in geopolitical risks decreases stock returns by 10.53–42.14%of its sample *** comparison,this paper uses alternatively the global economic policy uncertainty index and the economic policy uncertainty country index,and also finds a statistically significant negative relationship although it is weaker in the latter case.

Hepatic ischemia-reperfusion syndrome has been the subject of intensive study and experimentation in recent decades since it is responsible for the outcome of several clinical entities,such as major hepatic resections and liver *** addition to the organ’s post reperfusion injury,this syndrome appears to play a central role in the dysfunction of distant tissues and ***,continuous research should be directed toward finding effective therapeutic options to improve the outcome and reduce the postoperative morbidity and mortality *** is a synthetic analog of prostaglandin I2,and its experimental administration has shown encouraging *** has already been approved by the Food and Drug Administration in the United States for pulmonary arterial hypertension and has been used in liver transplantation,where preliminary encouraging results showed its safety and feasibility by using continuous intravenous administration at a dose of 5 ng/kg/*** improves renal and hepatic function,diminishes hepatic oxidative stress and lipid peroxidation,reduces hepatictoll-like receptor 9 and inflammation,inhibits hepatic apoptosis and restores hepatic adenosine triphosphate(AtP)levels and AtP synthases,which is necessary for functional maintenance of *** exhibits vasodilatory properties and antiplatelet activity and regulates proinflam-matory cytokines;therefore,it can potentially minimize ischemia-reperfusion ***,it may have beneficial effects on cardiovascular parameters,and much current research interest is concentrated on this compound.

Dear Editor, Hypersensitivity to mosquito bites （HMB） is primarily seen in children and adolescents in Asia and Central America. HMB may be related to the reactivation of Eps- tein-Barr virus （EBV） in infectzd natural killer （NK） cells and is directly associated with chronic active EBV infection （CAEBV） and NK/t-cell leukemia/lymphoma. the median age at diagnosis of HMB is 6.7 years, and local cutaneous reactions include erythema, bullae, ne- crosis, and ulceration. In addition, systemic symptoms, including high-grade fever, malaise, lymphadenopathy, hepatosplenomegaly, hepatic dysfunction, hematuria, and proteinuria, are often present. While the etiology of HMB remains unclear, mosquito salivary gland extract may trigger EBV reactivation in latently infected NK cells. Upon reactivation, EBV oncogenes, such as latent membrane protein 1 （LMP1）, may induce immortaliza- tion of NK cells, eventually progressing to lymphoma.
