Liver transplantation is a major abdominal operation and the intimate anatomic relation of the liver with the right hemidiaphragm predisposes the patient to various manifestations in the chest ***,chronic liver disease affects pulmonary function before and after liver transplantation resulting in a considerable percentage of patients presenting with morbidity related to chest *** review aims to identify the potential chest complications of surgical interest during or after liver *** of surgical interest are defined as those conditions that necessitate an invasive procedure(such as thoracocentesis or a chest tube placement)in the chest or a surgical intervention performed by a thoracic *** complications will be classified as perioperative and postoperative;the latter will be categorized as early and *** thoracocentesis or a chest tube placement is usually sufficient when invasive measures are deemed necessary,in some patients,thoracic surgical interventions are warranted.A high index of suspicion is needed to recognize and treat these conditions promptly.A close collaboration between abdominal surgeons,intensive care unit physicians and thoracic surgeons is of paramount importance.

AIM： To analytically assess the effect of pupil size upon the refractive power distributions of different designs of multifocal contact ***： Two multifocal contact lenses of center-near design and one multifocal contact lens of center-distance design were used in this study. Their power profiles were measured using the NIMO TR1504 device （LAMBDA-X, Belgium）. Based on their power profiles, the power distribution was assessed as a function of pupil size. For the high addition lenses, the resulting refractive power as a function of viewing distance （far, intermediate, and near） and pupil size was also ***： The power distribution of the lenses was affected by pupil size differently. One of the lenses showed a significant spread in refractive power distribution, from about ？3 D to 0 D. Generally, the power distribution of the lenses expanded as the pupil diameter became greater. The surface of the lens dedicated for each distance varied substantially with the design of the ***： In an experimental basis, our results show how the lenses power distribution is affected by the pupil size and underlined the necessity of careful evaluation of the patient’s visual needs and the optical properties of a multifocal contact lens for achieving the optimal visual outcome.

Apolipoprotein E（APOE） is a major protein component of peripheral and brain lipoprotein transport *** in peripheral circulation does not cross the blood brain barrier or blood cerebrospinal fluid barrier. As a result,peripheral APOE expression does not affect brain APOE levels and vice versa. Numerous epidemiological studies suggest a key role of peripherally expressed APOE in the development and progression of coronary heart disease while brain APOE has been associated with dementia and Alzheimer＇s disease. More recent studies, mainly in experimental mice, suggested a link between Apoe and morbid obesity. According to the latest findings, expression of human apolipoprotein E3（APOE3） isoform in the brain of mice is associated with a potent inhibition of visceral white adipose tissue（WAT） mitochondrial oxidative phosphorylation leading to significantly reduced substrate oxidation,increased fat accumulation and obesity. In contrast, hepatically expressed APOE3 is associated with a notable shift of substrate oxidation towards non-shivering thermogenesis in visceral WAT mitochondria, leading to resistance to obesity. These novel findings constitute a major paradigm shift from the widely accepted perception that APOE promotes obesity via receptor-mediated postprandial lipid delivery to WAT. Here, we provide a critical review of the latest facts on the role of APOE in morbid obesity.

This paper introduces an innovative Software Quality Assurance framework tailored for B2C e-commerce systems, seamlessly integrating software quality with business objectives. Drawing from elements of the ISO 25000 series and ISO 20000 standards, this framework specifically addresses challenges inherent to e-commerce. By establishing business-relevant KPIs, the framework ensures that ongoing improvement initiatives resonate with the company’s strategic goals. Additionally, the paper presents a Dynamic Bayesian Network model as a hands-on tool for implementing the framework within e-commerce organisations.

Climate Change vs Cultural Heritage: An Adaptation Strategy for the Archaeological Site of Ancient Messene

This article examines the effects of climate change on archaeological sites, using as a case study the archaeological site of Ancient Messene in Greece, and proposes an integrated adaptation strategy. In order to determine the site’s most significant risks, a climate data analysis was conducted, taking into account three different climate emission scenarios (Representative Concentration Pathways) and two future periods (2031-2060, 2071-2100). The Intergovernmental Panel on Climate Change (IPCC) vulnerability assessment methodology was adopted to assess the vulnerability of the archaeological site to the effects of climate change. This is the first time such a methodology has been followed in Greece. The analysis revealed that the site’s critical hazards are fire, desertification, and flooding. The geographical location of the site in an intensely dry microclimate and the lack of safe and functional electromechanical and road infrastructure increase its vulnerability. The materials of the monuments are indirectly and directly impacted by climate change, whereas the vegetation is negatively impacted by the frequency of extreme events, especially wildfires. Based on the analysis results, a five-axis adaptation strategy was developed.

The Old Town of Corfu is an excellent example of a historic town and a World Heritage Site, distinguished by its authentic and unique character, as reflected in its Venetian-era fortifications and extensive historic building stock. Simultaneously, the Old Town of Corfu is also a vibrant modern city vulnerable to various pressures, including climate change. This paper aims to evaluate the effects of climate change on this modern city monument, assess its vulnerability using the Intergovernmental Panel on Climate Change’s methodology, and develop a comprehensive set of adaptation proposals. The methodology of this paper is based on the analysis of climate data for the Old Town of Corfu, from which the assessment of the extreme weather events and climate changes that pose the greatest threat to the Old Town and the assessment of its vulnerability to these threats are derived. The dense geometrical characteristics of the city’s structure, the intense pathology observed in the materials and structures of the historic building stock, problems in the existing electromechanical infrastructure, and the poor management of issues such as increased tourism and heavy traffic congestion are the primary factors that make the Old Town of Corfu vulnerable to the effects of climate change.

Circulating microRNAs as biomarkers for diabetic neuropathy: A novel approach

Oxidative stress stemming from tissue exposure to constant hyperglycemia is one of the major pathogenetic pathways of diabetic macro-and microvascular complications. Diabetic polyneuropathy, commonly manifesting as distal,symmetrical sensorimotor polyneuropathy, is characterized by progressive severity of symptoms, with rates analogous to the quality of glycemic control achieved by the patients and physicians. Palliative care with analgesics and aggressive glycemic control often improve quality of life in the absence of causative treatment. Currently, there is a growing body of evidence indicating the role of microRNAs in the pathogenesis of diabetic complications, with emphasis on diabetic nephropathy and neuropathy. Therefore, in this review, we aim to explore the role of microRNAs and their polymorphisms in the pathophysiology of diabetic polyneuropathy, as well as, the possibility of novel diagnostic and therapeutic applications by epigenetic profiling and manipulation.

BACKGROUND Gastroesophageal reflux disease(GERD) occurs when the reflux of stomach contents causes troublesome symptoms and/or complications. When medical therapy is insufficient, surgical therapy is indicated and, until now, Laparoscopic fundoplication(LF) constitutes the gold-standard method. However, magnetic sphincter augmentation(MSA) using the LINX^® Reflux Management System has recently emerged and disputes the standard therapeutic *** To investigate the device’s safety and efficacy in resolving GERD *** This is a systematic review conducted in accordance to the PRISMA *** searched MEDLINE, ***, EMBASE, Cochrane Central Register of Controlled Trials CENTRAL databases from inception until September *** Overall, 35 studies with a total number of 2511 MSA patients were included and analyzed. Post-operative proton-pump inhibitor(PPI) cessation rates reached 100%, with less bloating symptoms and a better ability to belch or vomit in comparison to LF. Special patient groups(e.g., bariatric or large hiatal-hernias)had promising results too. The most common postoperative complication was dysphagia ranging between 6% and 83%. Dilation due to dysphagia occurred in 8% of patients with typical inclusion criteria. Esophageal erosion may occur in up to 0.03% of patients. Furthermore, a recent trial indicated MSA as an efficient alternative to double-dose PPIs in moderate-to-severe *** The findings of our review suggest that MSA has the potential to bridge the treatment gap between maxed-out medical treatment and LF. However, further studies with longer follow-up are needed for a better elucidation of these results.
