Dear Editor,We read with great interest the article by Presicce et al.[1]entitled"Wunderlich syndrome,an unexpected urological complication in a patient with coronavirus disease 2019(COVID-19):A case re-port."The authors aimed to shed light on the pathophysiology,dif-ferential diagnosis,and management of a rare urological bleeding complication in the course of severe acute respiratory syndrome co-ronavirus 2(SARS-CoV-2)*** the findings of the present case study were also discussed in a well-written article by Singh et al.,[2]we believe that several issues need to be highlighted and further clarified in this underrecognized field.

Colorectal cancer(CRC) constitutes the third most frequently reported malignancy in the male population and the second most common in women in the last two decades. Colon carcinogenesis is a complex, multifactorial event, resulting from genetic and epigenetic aberrations, the impact of environmental factors, as well as the disturbance of the gut microbial ecosystem. The relationship between the intestinal microbiome and carcinogenesis was relatively undervalued in the last decade. However, its remarkable effect on metabolic and immune functions on the host has been in the spotlight as of recent years. There is a strong relationship between gut microbiome dysbiosis, bowel pathogenicity and responsiveness to anti-cancer treatment;including immunotherapy. Modifications of bacteriome consistency are closely associated with the immunologic response to immunotherapeutic agents. This condition that implies the necessity of gut microbiome manipulation. Thus, creatingan optimal response for CRC patients to immunotherapeutic agents. In this paper, we will review the current literature observing how gut microbiota influence the response of immunotherapy on CRC patients.

Heart failure(HF)and atrial fibrillation(AF)demonstrate a constantly increasing prevalence during the 21st century worldwide,as a result of the aging population and the successful interventions of the clinical practice in the deterioration of adverse cardiovascular *** and AF share common risk factors and pathophysiological mechanisms,creating the base of a constant *** impairs systolic and diastolic function,resulting in the increasing incidence of HF,whereas the structural and neurohormonal changes in HF with preserved or reduced ejection fraction increase the possibility of the AF *** temporal relationship of the development of either condition affects the diagnostic algorithms,the prognosis and the ideal therapeutic strategy that leads to euvolaemia,management of noncardiovascular comorbidities,control of heart rate or restoration of sinus rate,ventricular synchronization,prevention of sudden death,stroke,embolism,or major bleeding and maintenance of a sustainable quality of *** indicated treatment for the concomitant HF and AF includes rate or/and rhythm control as well as thromboembolism prophylaxis,while the progress in the understanding of their pathophysiological interdependence and the introduction of the genetic profiling,create new paths in the diagnosis,the prognosis and the prevention of these diseases.

In the context of the Cigeo project,the French National Radioactive Waste Management Agency(Andra)is studying the behaviour of a deep geological facility for radioactive waste deposit in the CallovoOxfordian(COx)*** assessment of durability of this project requires the prediction of irreversible strain over a large time *** mechanical interaction of the host rock and the concrete support of tunnels must be investigated to ensure the long-term sustainability of the *** instantaneous and time-dependent behaviour of the claystone-concrete interface is experimentally investigated with direct shear tests and long-duration shear tests of a few *** mechanical and structural state of the claystone which is affected after interaction with concrete reflects to the response of the claystone-concrete interface,and thus different types of COx claystone-concrete interfaces are *** delayed deformation of the interface is found to be linked to the level of the normal loading and the loading history while a different response of the interface was observed from the short-and long-duration tests,indicating a possible progressive modification of interface under long-duration loadings.

Estimation of quality of life in Cypriot patients with inflammatory bowel disease

AIM To investigate the health-related quality of life(HRQoL)of patients suffering with idiopathic inflammatory bowel disease(IBD).METHODS The Greek validated version of the Short Inflammatory Bowel Disease Questionnaire was used for evaluating the quality of life of IBD *** questionnaire was distributed to 100 consecutive patients suffering from IBD and presenting for a clinic appointment at the endoscopy unit of Larnaca General Hospital during the period from October to November *** criteria for participating in this study were constituted by the documented diagnosis of either ulcerative colitis(UC)or Crohn’s disease(CD)after endoscopy and histologic examination at least 6 months before the study,adult patients(18 years old or older),the capability of verbal communication and the patient’s written consent for attending this *** majority of the questionnaires were completed by a nurse practitioner who specializesin IBD patient *** Regarding the physical dimension in patients with UC,males scored significantly higher than females(4.2 vs3.4,P=0.023).Higher scores were also observed in UC patients younger than 35 or older than 50 years(4.0 and 4.2 vs 3.2,respectively,P=0.021).The psychological dimension revealed similar results in patients with UC,with males,and older ages scoring higher(5.0 vs 3.0,P=0.01 and 4.7 vs 2.7,P<0.5,respectively),whereas regarding CD higher scores were observed in married compared to unmarried(3.83vs 2.33,P=0.042).No statistical differences in any parameters in the social dimension were *** the treatment of,patients with CD,overall higher scores were observed when treated with biological factors compared to standard therapy in all dimensions but with statistical significant difference in the social dimension(5.00 vs 3.25,P=0.045).CONCLUSION The study reveals a negative impact of IBD on HRQo *** risks are age and gender in patients with UC and family status in patients with CD.

A typological,environmental and socio-cultural study of semi-open spaces in the Eastern Mediterranean vernacular architecture:The case of Cyprus

Semi-open spaces-largely incorporated in vernacular dwellings in Cyprus during the 19th and 20th centuries-formed diachronically significant socio-cultural, functional and environmental features of the vernacular architecture of the area. The climate of the Eastern Mediterranean region, i.e., hot summers and mild winters, encouraged the use of open weather protected spaces, thus leading to the widespread incorporation of such spaces in the vernacular architecture of the region. This paper focuses on the interconnections between architectural forms and human comfort, convenience or pleasure in relation to the semiopen spaces found in the vernacular architecture of Cyprus-an island in the Eastern Mediterranean region. For the purpose of this research, characteristic traditional settlements found in the coastal, lowland and mountainous areas-which represent three different climatic regions and topographies of the island-were selected for an in-depth investigation. The findings confirm a high frequency of semi-open spaces, as well as the existence of a remarkable richness of typologies. Although semi-open spaces constitute a fundamental part of the structure of these vernacular dwellings in all climatic regions examined, they dominate in the lowland regions due to the particularly hot climate, as well as the specific activities of the inhabitants of these areas. The prevailing architectural forms and constructions of these spaces in each climatic region under investigation was found to be closely adapted to the local resources, terrain and climate, while also being related to the social, household and agricultural needs of the inhabitants, thus underlining their sustainable and locus-specific conception. Research findings can contribute towards critically re-thinking semi-open spaces and their inherent value in rehabilitation projects, as well as in contemporary residential architecture.

Background:The right ventricle to pulmonary artery conduit(RVPAC)may impair right ventricular(RV)function in patients with functional single right *** of the RVPAC using a ring-reinforced end with dunked insertion into the RV through a limited ventriculotomy may reduce the impact on RV *** compared RV segmental strain between patients with a traditional RVPAC and ring-reinforced RVPAC using feature tracking cardiovascular magnetic resonance(CMR)***:Patients with CMR examinations after Stage I operation with RVPAC between 2000 and 2018 were *** mass,volumes,late gadolinium enhancement(LGE),and peak radial and circumferential strain of the 4 segments near the RVPAC insertion site were ***:The study included 71 CMR examinations in 61 patients(30 traditional RVPAC,31 ring-reinforced RVPAC).Prior to Stage II,the ring-reinforced RVPAC group had better peak radial strain and circumferential strain in 1 of 4 segments proximal to the RVPAC insertion site compared to the traditional RVPAC *** to Stage III operation,the ring-reinforced group had better peak radial and circumferential strain in 2 of 4 *** at the RVPAC insertion site was observed in 97%of patients before Stage II and 95%of patients before Stage *** type and regional strain were not associated with transplant-free survival during a median follow-up of 6.3 ***:Compared to a traditional RVPAC,the ring-reinforced RVPAC is associated with improved regional mechanics at the conduit insertion *** investigation of long-term outcomes after ring-reinforced RVPAC is warranted.

Background: Studies have indicated that there is a positive and indirect relationship between physical activity(PA) and quality of life(QoL). The current study examined this relationship through a social cognitive model with consideration to the intermediary effects of exercise self-efficacy,and physical(PCS, physical component summary) and psychological(MCS, mental component summary) health. Additionally, this model was widened to include concepts from the ecological theory, and any causal associations among neighborhood environment, PA, and ***: Six hundred and eighty-four physically active adults(39.16 ± 13.52 years, mean ± SD), living in Athens, Greece, completed a series of questionnaires measuring PA, QoL, exercise self-efficacy, PCS, MCS, neighborhood environment, and family and friend support for PA. The examined models were analyzed using structural equation ***: The social cognitive and ecological models proved to be of appropriate fit. Within the social cognitive model, PA positively affected QoL through the mediating effects of exercise self-efficacy, PCS, and MCS. With regards to the ecological model, neighborhood environment positively influenced QoL through the intermediary effects of family support for PA, exercise self-efficacy, PA, PCS, and ***: Results indicated that the most important mediators in the examined models were exercise self-efficacy and health. Further, findings demonstrated the role of neighborhood environment in enhancing PA and QoL. Future studies should be carried out applying longitudinal data for a better understanding of these associations over time.

Epicardial adipose tissue is defined as a deposit of adipocytes with pathophysiological properties similar to those of visceral fat, located in the space between the myocardial muscle and the pericardial sac. When compared with subcutaneous adipose tissue, visceral adipocytes show higher metabolic activity, lipolysis rates, increased insulin resistance along with more steroid hormone receptors. The epicardial adipose tissue interacts with numerous cardiovascular pathways via vasocrine and paracrine signalling comprised of pro-and anti-inflammatory cytokines excretion. Both the physiological differences be-tween the two tissue types, as well as the fact that fat distribution and phenotype, rather than quantity, affect cardiovascular function and metabolic processes, establish epicardial fat as a biomarker for cardiovascular and metabolic syndrome. Numerous studies have underlined an association of altered epicardial fat morphology, type 2 diabetes mellitus(T2 DM) and adverse cardiovascular events. In this review, we explore the prospect of using the epicardial adipose tissue as a therapeutic target in T2 DM and describe the underlying mechanisms by which the antidiabetic drugs affect the pathophysiological processes induced from adipose tissue accumulation and possibly allow for more favourable cardiovascular outcomes though epicardial fat manipulation.

This research is concerned with the novel application and investigation of‘Soft Actor Critic’based deep reinforcement learning to control the cooling setpoint(and hence cooling loads)of a large commercial building to harness energy *** research is motivated by the challenge associated with the development and application of conventional model-based control approaches at scale to the wider building *** Actor Critic is a model-free deep reinforcement learning technique that is able to handle continuous action spaces and which has seen limited application to real-life or high-fidelity simulation implementations in the context of automated and intelligent control of building energy *** control techniques are seen as one possible solution to supporting the operation of a smart,sustainable and future electrical *** research tests the suitability of the technique through training and deployment of the agent on an EnergyPlus based environment of the office *** agent was found to learn an optimal control policy that was able to minimise energy costs by 9.7%compared to the default rule-based control scheme and was able to improve or maintain thermal comfort limits over a test period of one *** algorithm was shown to be robust to the different hyperparameters and this optimal control policy was learnt through the use of a minimal state space consisting of readily available *** robustness of the algorithm was tested through investigation of the speed of learning and ability to deploy to different seasons and *** was found that the agent requires minimal training sample points and outperforms the baseline after three months of operation and also without disruption to thermal comfort during this *** agent is transferable to other climates and seasons although further retraining or hyperparameter tuning is recommended.
