Chronic pain, posttraumatic stress disorder, and opioid intake: A systematic review

BACKGRoUND The literature suggests that there is a high degree of co-occurrence between chronic pain and posttraumatic stress disorder(PTSD). An association has been found between PTSD and substance abuse. PTSD is a severe disorder that should be taken into account when opioids are prescribed. It has been found that the prevalence of opioid use disorder(oUD) in chronic pain patients is higher among those with PTSD than those without this disorder.AIM To perform a systematic review on the association between PTSD, chronic noncancer pain(CNCP), and opioid intake(i.e., prescription, misuse, and abuse).METHoDS We conducted a systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The Patient, Intervention,Comparator, and outcomes(PICoS) criteria were formulated a priori in the protocol of the systematic review. A search was conducted of the PRoSPERo database. In March 2019, searches were also conducted of 5 other databases:Pub Med, MEDLINE, Psyc INFo, Web of Science, and PILoTS. The Scottish Intercollegiate Guidelines Network checklist for cohort studies was used to assess the selected studies for their methodological quality and risk of bias. Each study was evaluated according to its internal validity, participant sampling,confounding variables, and the statistical analysis.RESULTS A total of 151 potentially eligible studies were identified of which 17 were retained for analysis. only 10 met the selection criteria. All the studies were published between 2008 and 2018 and were conducted in the United States. The eligible studies included a total of 1622785 unique participants. of these, 196516 had comorbid CNCP and PTSD and were consuming opiates. The participants had a cross-study mean age of 35.2 years. The majority of participants were men(81.6%). The most common chronic pain condition was musculoskeletal pain:back pain(47.14% across studies;range: 16%-60.6%), arthritis and joint pain(31.1%;range: 18%-67.5%), and neck pain(28.7%;range: 3.6%-63%). At present,there is an increasing demand for natural bioactive compounds able to provide health benefits when included and consumed in a functional food or in a nutraceutical.In this regard,microalgae are promising natural sources with great potential,not only considering that these organisms are largely underexplored,but also because microalgae can be produced at large scale and their chemical composition might be tuned to over-synthesize a particular target compound.The use of advanced sustainable extraction techniques to recover these bioactive compounds is a must nowadays.This work presents an overview on the use of compressed fluid-based extraction techniques to obtain bioactive compounds from microalgae that can be seen also as a first step towards its recovery at larger scale.When relevant,the description of the analytical procedure used to chemically characterize the bioactive compounds is also included.

Appendiceal neuroendocrine tumors(aNETs)are an uncommon neoplasm that is relatively indolent in most cases.They are typically diagnosed in younger patients than other neuroendocrine tumors and are often an incidental finding after an appendectomy.Although there are numerous clinical practice guidelines on management of a NETs,there is continues to be a dearth of evidence on optimal treatment.Management of these tumors is stratified according to risk of locoregional and distant metastasis.However,there is a lack of consensus regarding tumors that measure 1-2 cm.In these cases,some histopathological features such as size,tumor grade,presence of lymphovascular invasion,or mesoappendix infiltration must also be considered.Computed tomography or magnetic resonance imaging scans are recommended for evaluating the presence of additional disease,except in the case of tumors smaller than 1 cm without additional risk factors.Somatostatin receptor scintigraphy or positron emission tomography with computed tomography should be considered in cases with suspected residual or distant disease.The main point of controversy is the indication for performing a completion right hemicolectomy after an initial appendectomy,based on the risk of lymph node metastases.The main factor considered is tumor size and 2 cm is the most common threshold for indicating a colectomy.other factors such as mesoappendix infiltration,lymphovascular invasion,or tumor grade may also be considered.on the other hand,potential complications,and decreased quality of life after a hemicolectomy as well as the lack of evidence on benefits in terms of survival must be taken into consideration.In this review,we present data regarding the current indications,outcomes,and benefits of a colectomy.

Pressurized liquid extraction using green solvents(ethanol and water)was applied to extract photosynthetic pigments from the microalgae Chlamydomonas sp.An experimental design is employed to determine the effect of the extraction conditions on the extraction yield and the antioxidant capacity of the different extracts determined using 1,1-diphenyl-2-picryl-hydrazyl and TEAC tests.All the extracts show significant antioxidant activity in all assays.Besides,in this work,the carotenoid and chlorophyll composition of the Chlamydomonas extracts was analyzed by HPLC–DAD–APCI–MS/MS for the first time,being the main compounds lutein and pheophythin a′.

Background:In-hospital mortality in patients with coronavirus disease 2019(CoVID-19)is high.Simple prognostic indices are needed to identify patients at high-risk of CoVID-19 health outcomes.We aimed to determine the usefulness of the CoNtrolling NUTritional status(CoNUT)index as a potential prognostic indicator of mortality in CoVID-19 patients upon hospital admission.Methods:our study design is of a retrospective observational study in a large cohort of CoVID-19 patients.In addition to descriptive statistics,a Kaplan-Meier mortality analysis and a Cox regression were performed,as well as receiver operating curve(roc).Results:From February 5,2020 to January 21,2021,there was a total of 2969 admissions for CoVID-19 at our hospital,corresponding to 2844 patients.overall,baseline(within 4 days of admission)CoNUT index could be scored for 1627(57.2%)patients.Patients’age was 67.3±16.5 years and 44.9%were women.The CoNUT severity distribution was:194(11.9%)normal(0-1);769(47.2%)light(2-4);585(35.9%)moderate(5-8);and 79(4.9%)severe(9-12).Mortality of 30 days after admission was 3.1%in patients with normal risk CoNUT,9.0%light,22.7%moderate,and 40.5%in those with severe CoNUT(Pof death associated with a greater baseline CoNUT stage was sustained in a multivariable Cox regression model(PoNUT stage was associated with a longer duration of admission,a greater requirement for the use of non-invasive and invasive mechanical ventilation,and other clinical outcomes(all Proc of CoNUT for mortality had an area under the curve(AUC)and 95%confidence interval of 0.711(0.676-0746).Conclusion:The CoNUT index upon admission is potentially a reliable and independent prognostic indicator of mortality and length of hospitalization in CoVID-19 patients.

193 members of the United Nations approved the 17 Sustainable Development Goals in September 2015.According to the 2030 Agenda,the SDGs contemplated the ending of poverty,the protection of the Earth and the promotion of prosperity for all.Sustainable Development Goal 17(SDG 17)deals specifically with the creation of global alliances for development.The underlying assumption respecting this point is that these stakeholder partnerships encourage the interchange of knowledge,experience,technology,and other resources to administer efficiently the other sixteen SDGs.Although SDG 17 is very well established in theory,in practice there are still appreciable downfalls as to how to successfully make this theory become a reality.This short review will analyse the potential viability of SDG 17“partnerships for the goals”with respect to SDG 7(affordable and clean energy),and thereupon SDG 13(associated with climate action)utilising two south-western France two wind farm initiatives.

Transcriptome‑wide mapping of milk somatic cells upon subclinical mastitis infection in dairy cattle

Background Subclinical intramammary infection(IMI)represents a significant problem in maintaining dairy cows’health.Disease severity and extent depend on the interaction between the causative agent,environment,and host.To investigate the molecular mechanisms behind the host immune response,we used RNA-Seq for the milk somatic cells(SC)transcriptome profiling in healthy cows(n=9),and cows naturally affected by subclinical IMI from Proto-theca spp.(n=11)and Streptococcus agalactiae(S.agalactiae;n=11).Data Integration Analysis for Biomarker discov-ery using Latent Components(DIABLo)was used to integrate transcriptomic data and host phenotypic traits related to milk composition,SC composition,and udder health to identify hub variables for subclinical IMI detection.Results A total of 1,682 and 2,427 differentially expressed genes(DEGs)were identified when comparing Prototheca spp.and S.agalactiae to healthy animals,respectively.Pathogen-specific pathway analyses evidenced that Proto-theca’s infection upregulated antigen processing and lymphocyte proliferation pathways while S.agalactiae induced a reduction of energy-related pathways like the tricarboxylic acid cycle,and carbohydrate and lipid metabolism.The integrative analysis of commonly shared DEGs between the two pathogens(n=681)referred to the core-mastitis response genes,and phenotypic data evidenced a strong covariation between those genes and the flow cytometry immune cells(r2=0.72),followed by the udder health(r2=0.64)and milk quality parameters(r2=0.64).Variables with r≥0.90 were used to build a network in which the top 20 hub variables were identified with the Cytoscape cyto-hubba plug-in.The genes in common between DIABLo and cytohubba(n=10)were submitted to a roc analysis which showed they had excellent predictive performances in terms of discriminating healthy and mastitis-affected animals(sensitivity>0.89,specificity>0.81,accuracy>0.87,and precision>0.69).Among these genes,CIITA could play a key role in regulating the animals’response to subclinical IMI

Surrogacy in Reproductive Medicine: Ethical and Legal Analysis of the Procedure

This text sets out the different arguments regarding surrogacy as a reproductive medicine tech-nique in Colombia. The procedure is described, as well as the legal matters that this issue generates. Gestational carriers appear as an alternative for couples who want to have a child but they are not able to conceive because of infertility issues and do not want to go through an adoption process. Therefore, they adhere to surrogacy as an alternative of assisted human reproduction;a procedure that in Colombia faces serious legal gaps and causes ethical dilemmas in the collective imagery. These situations suggest an open discussion through a dialectics between the scientific progress advances, the lack of legal regulation and the constants against ethical positions that the surrogacy procedure causes in Colombia and other parts of the world.

objective:To relate the presence of active metabolites with therapeutic effects of the Allium sativum L whose common name is Garlic,as well as its contraindications and adverse reactions and its use as condiment.Methods:Searching about the topic in the last 5 years and present a general view of the Allium sativum L whose common name is Garlic.Conclusions:Technological prevision was based on the search of articles in the Virtual Library in Health(BVS)and using the key words“medicinal plants”/“spice”/Allium sativum L/garlic/adverse reactions and contraindications,and the association among them.

Gut dysbiosis is associated with poorer long-term prognosis in cirrhosis

BACKGRoUND Gut dysbiosis is common in cirrhosis.AIM To study the influence of gut dysbiosis on prognosis in cirrhosis.METHoDS The case-control study included 48 in-patients with cirrhosis and 21 healthy controls.Stool microbiome was assessed using 16S ribosomal ribonucleic acid gene sequencing.We used modified dysbiosis ratio(MDR):[Bacilli(%)+Proteobacteria(%)]/[Clostridia(%)+Bacteroidetes(%)].Patients with MDR more the median made up the group with severe dysbiosis,others did the group with nonsevere dysbiosis.The follow-up period was 4 years.RESULTS The mortality rate of patients with severe dysbiosis was significantly higher than that of patients with non-severe dysbiosis(54.2%vs 12.5%;P=0.001).The presence of severe dysbiosis was independent risk factors for death[hazard ratio=8.6×(1.9-38.0);P=0.005].The abundance of Enterobacteriaceae(P=0.002),Proteobacteria(P=0.002),and Lactobacillaceae(P=0.025)was increased and the abundance of Firmicutes(P=0.025)and Clostridia(P=0.045)was decreased in the deceased patients compared with the survivors.The deceased patients had a higher MDR value than the survivors[0.131×(0.069-0.234)vs 0.034×(0.009-0.096);P=0.004].If we applied an MDR value of 0.14 as the cutoff point,then it predicted patient death within the next year with a sensitivity of 71.4%and a specificity of 82.9%[area under the curve=0.767×(0.559-0.974)].MDR was higher in patients with cirrhosis than in health controls[0.064×(0.017-0.131)vs 0.005×(0.002-0.007);Pompensated cirrhosis than in patients with compensated cirrhosis[0.106×(0.023-0.211)vs 0.033×(0.012-0.074);P=0.031].MDR correlated negatively with prothrombin(r=-0.295;P=0.042),cholinesterase(r=-0.466;P=0.014)and serum albumin(r=-0.449;P=0.001)level and positively with Child–Turcotte–Pugh scale value(r=0.360;P=0.012).CoNCLUSIoN Gut dysbiosis is associated with a poorer long-term prognosis in cirrhosis.
