Nodular Pulmonary Amyloidosis with Interstitial Lung Disease—Case Report and Literature Review

Amyloidosis is a rare spectrum of disease which involves deposition of misfolded extracellular proteins (amyloids) in various body organs leading to progressive organ dysfunction. Clinical presentation can be variable depending on the organ involved and type of protein. Amyloidosis can be classified based on quantity, type, and location of these proteins. Amyloid light-chain amyloidosis develops in the bone marrow, producing abnormal forms of light-chain proteins, which cannot be broken down. These proteins transform into amyloid fibrils and form amyloid deposits in different organs. Pulmonary amyloidosis is uncommonly diagnosed since it is rarely symptomatic. Diagnosis of pulmonary amyloidosis is usually made in the setting of systemic amyloidosis;however, it may present as localised pulmonary disease. Localized pulmonary Amyloidosis can present as nodular, cystic, or tracheobronchial amyloidosis. Depending on the degree of the interstitial involvement, it may affect alveolar gas exchange and cause respiratory symptoms. This is a case of a 47-year-old female with background history of interstitial lung disease presenting with progressive shortness of breath. Computed tomography scan revealed bilateral pulmonary nodules. The patient was referred to our thoracic surgery team with the suspicion of bronchogenic malignancy with metastasis. Diagnostic video assisted wedge resection was performed for this patient, and histology confirmed pulmonary amyloidosis of nodular type. Amyloid deposition simulates both inflammatory and neoplastic conditions. Definitive diagnosis requires biopsy confirmation therefore early detection and commencing the patient on appropriate treatment pathway may help in symptomatic relief and better outcome.

Subsea Maintenance Service Delivery:Mapping Factors Influencing Scheduled Service Duration

The oil and gas （O＆G） industry on the Norwegian continental shelf （NCS） leads the world in terms of the number of subsea O＆G installations. Ensuring the dependability of these assets is critical. Non-intrusive inspection, maintenance and repair （IMR） services are therefore needed to reduce risks. These services are planned and executed using a mono-hull offshore vessel complete with remotely operated vehicles （ROVs）, a module handling system and an active heave compensated crane. Vessel time is shared between competing jobs, using a prioritized forward-looking schedule. Extension in planned job duration may have an impact on O＆G production, service costs and health, safety, and environmental （HSE） risks. This paper maps factors influencing the job schedule efficiency. The influence factors are identified through reviews of literature as well as interviews with experts in one of the large IMR subsea service providers active on the Norwegian Continental Shelf. The findings show that the most obvious factors are weather disruption and water depth. Other factors include job complexity, job uncertainty, IMR equipment availability, as well as the mix of job complexity.

Serum immunoglobulin E level of children infected with intestinal parasite in Okada, Nigeria

Children especially in rural areas of Okada have high rates of intestinal parasitosis with a prevalence of 50%. Stool and blood sample were collected of 334 children which comprised of 152 females and 185 males. Intestinal parasitosis was confirmed by direct smear technique and formol-ether concentration method. Serum IgE level was estimated by ELISA method. It was revealed that children between the ages of 11 - 15 years had the highest incidence of intestinal parasite in both sex (73.3% of the males and 62.5% of the females). About 2-fold elevation in serum IgE level was demonstrated. Intervention program including early introduction of health education to children is advocated and environmental sanitation should be encouraged.

Sustaining Increase in Life Expectancy in Africa Requires Active Preventive Measures against Non-Communicable Diseases

It is projected that aged population (≥60 years) will continue to increase globally, including in Africa. This is due to reduced population growth, decreased fecundity and improved medical interventions;factors which increase life expectancy. While this is typical for developed countries, it is not the same for Africa and similar developing regions. In these regions, a significant proportion of death is due to non-communicable diseases (NCD’s) such as hypertension, cerebrovascular accident, coronary heart disease, diabetes mellitus, chronic renal disease and cancer, among others. Rising prevalence of NCD’s due mainly to western style diets and sedentary living is made worse by inadequate nutrition education, high prevalence of low birth weight, poor health services, lack of efficient tobacco control and deficient planning of built environment. In order to halt the possible reduction in life expectancy occasioned by NCD’s, efforts by the community, health planners and governments in Africa to address relevant NCD’s, must be put in place. Suggested measures are: nutrition education, regular community directed physical exercise, improved environmental planning and development. Others are review of present health service model, early detection, prevention and treatment of NCD’s, including improved antenatal care to reduce low birth weights, and establishment of policies and measures that decreased access to tobacco especially by women of childbearing age. Africa and similar developing regions cannot fund the health bill due to NCD’s and their complications;hence it is important that this scourge is attended to with all seriousness.

Serum C reactive protein level in type 2 diabetes mellitus patients attending diabetic clinic in Benin City, Nigeria

C reactive protein is sensitive physiological biomarkers of sub clinical inflammation associated with hyperglycemia. The aim of this study is to determine the fasting serum C reactive protein level in type 2 diabetes mellitus patients attending diabetic clinic in Benin City, Nigeria. The population sample consists of 142 subjects. 71 patients were known type 2 diabetes mellitus, while the other 71 were age matched control subjects. Fasting glucose and C reactive protein were estimated using glucose oxidase method and ELISA method respectively. The age group that has the highest number of type 2 diabetes mellitus was 41 - 50 (64% of males and 36% of the females). Our finding revealed that C reactive protein and serum glucose level of type 2 Diabetes mellitus in both females and males show a statistically significant increase as compared with age matched Control subjects,

There are limited studies examining the relationship between serum reproductive hormone levels and sexual functions among prostate cancer (PCa) patients after rapid prostatectomy (RP). Aim: The present study aimed at evaluating the correlation between post-operative serum levels of some male reproductive hormones of PCa patients and their sexual functions in the months following RP. A total of 20 male patients with clinically localized PCa who had undergone RP within the last 12 months prior to commencement of the study, were invited to participate. The postoperative serum levels of their luteinizing hormone (LH), follicle stimulating hormone (FSH) and total testosterone (Te) were measured. They also completed a structured health and lifestyle questionnaire to obtain information on their demographic characteristics and detailed medical history. Our data indicated normal mean levels of post-operative Te (4.34 ± 4.38 ng/ml) in the patients following RP, while the mean levels of FSH (39.24 ± 34.12 miu/ml) and LH (21.67 ± 25.73 miu/ml) were on the increase and far above normal ranges for healthy men. Background: Benign Prostatic Hyperplasia (BPH) is a common condition which occurs as men get older. Aim: We aimed at investigating the reproductive hormone profile of men with BPH and the relationship of these hormones with age. Methods: Twenty-nine men of age > 40 years were screened for prostate symptoms. The prostate examination included digital rectal examination and measurement of serum prostate-specific antigen levels. Demographic and clinical characteristics of subjects were obtained using a structured questionnaire. The serum levels of Testosterone (Te), Luteinizing hormone (LH), Follicle stimulating hormone (FSH) and Prolactin (PRL) were measured using ELISA method. Results: Our study indicated that age and levels of prostate specific antigen (PSA), LH and FSH were significantly (p Conclusion: This study suggests that the rise in serum levels of the hypothalamic-pituitary-testicular axis hormones in BPH patients as demonstrated by the present findings may be age-related since testicular dysfunction in aging men is associated with the decrease in androgen production which may stimulate an increased production of gonadotropins by negative feedback.
