Backgr.und: Asymptomatic bacter.ur.a (ASB) and ur.nar. tr.ct infections (UTI) dur.ng pr.gnancy may contr.bute to adver.e pr.gnancy outcomes. Diabetes mellitus (DM) and gestational diabetes mellitus (GDM) ar. consider.d to be impor.ant additional r.sk factor.for.ASB and UTI dur.ng pr.gnancy. Aims: To investigate differ.nces in pr.valence of ASB and incidence of UTI in pr.gnant women with and without DM and GDM to infor. ASB scr.ening and tr.atment policies. Methods: Data fr.m 214 pr.gnant women who gave bir.h dur.ng 2010 at the Women’s and Childr.n’s Hospital, Adelaide, Austr.lia wher. cases wer. women with a clinical diagnosis of (G)DM and contr.ls wer. matched on date of bir.h. ASB was defined as the gr.wth of at least 10e5 colony for.ing units/ml of one or.anism or.any pr.sence of gr.up B str.ptococcus (GBS) at the fir.t ur.ne cultur. collected dur.ng pr.gnancy without complaints of a UTI. A clinical UTI was diagnosed by the tr.ating physician, in combination with a positive ur.ne cultur. it was defined as cultur.-confir.ed UTI. r.sults: No significant differ.nces in pr.valence of ASB (5.6% and 3.7%;r.lative r.sk (r.) 1.50;95% confidence inter.als (CI) 0.44 - 5.17), incidence of clinical UTI (4.7% and 11.2%;r. 0.42;95% CI 0.15 - 1.14) or.cultur.-confir.ed UTI (2.8% and 3.7%;r. 0.75;95% CI 0.17 - 3.27) between pr.gnant women with and without (G)DM wer. pr.sent. No association was found between ASB and UTI. GBS was the most common causative or.anism of ASB in women with and without DM (66.7% and 50.0%). Conclusion: In contr.st with ear.ier.r.sear.h, no significant differ.nces in pr.valence of ASB or.incidence of UTI was found between pr.gnant women with and without (G)DM.

Setting Maximum Mor.ality Limits for.Har.our.Por.oises in Dutch Water. to Achieve Conser.ation Objectives

Har.our.por.oises in the Nor.h Sea ar. pr.tected under.national and EU legislation, and under.the r.gional agr.ement ASCOBANS, within the UN Convention on Migr.tor. Species. As yet, no methods exist to quantify the cumulative impacts of dir.ct anthr.pogenic mor.ality (e.g. by-catch) and indir.ct effects of human activities (e.g. contaminants) on por.oise populations. However. pr.cedur.s do exist for.assessing whether.total anthr.pogenic mor.ality may r.sult in failur. to meet conser.ation objectives. While the ultimate aim is to r.duce the number.of such deaths to zer., ASCOBANS has an inter.m objective to r.duce anthr.pogenic mor.ality to a level that will allow r.cover. of populations. Sever.l differ.nt cr.ter.a have been pr.posed as limits to anthr.pogenic mor.ality that may still allow conser.ation objectives to be met. These cr.ter.a include simple per.entages of the best population abundance estimate and mor. complex pr.cedur.s that account for.uncer.ainty and other.infor.ation about the population. In this paper. we r.por. new estimates of abundance for.por.oises in Dutch water., and we apply sever.l methods to calculate maximum anthr.pogenic mor.ality limits fr.m these estimates. We consider.d whether.these mor.ality limits would meet the objective of the ASCOBANS agr.ement and other.inter.ational obligations, and how these limits may be applied at a national level r.ther.than the biological population level. The cur.ent simple appr.ach of a limit of 1.7% of the best abundance estimate may not satisfy conser.ation objectives. We r.commend the use of management pr.cedur.s for.setting mor.ality limits that take into account available data including associated uncer.ainties and biases, and whose per.or.ance has been extensively tested thr.ugh simulation.

It is suggested that unicompar.mental knee r.placement (UKr. offer. the potential to r.stor. nor.al knee kinematics better.than total knee r.placement (TKr. because of r.taining the cr.ciate ligaments, and better.pr.ser.ation of the over.ll geometr.. It was hypothesized that patient-specific UKr.would r.stor. nor.al knee kinematics even better.because of a customised ar.icular.shape. A compar.tive kinematics study was conducted on thr.e cadaver.limbs using two differ.nt test setups, a loaded ankle r.g and an unloaded ankle r.g. Kinematics was compar.d between a patient-specific UKr.and a conventional fixed-bear.ng UKr. Both the UKr. showed similar.kinematic patter.s to the nor.al knee using both the test appar.tus. The patient-specific UKr.showed good r.sults and with the other.benefits it shows potential to dr.matically impr.ve clinical outcomes of knee r.placement sur.er..

Backgr.und: Mechanical ventilation (hit one) dur.ng sur.er. (hit two) is often needed and both induce an inflammator. r.sponse. Dysr.gulation of the inflammator. r.sponse can cause chr.nic postoper.tive pain. Methods: Healthy C57BL6 mice (n = 56) wer. mechanically ventilated (MV) and allocated to r.ceive sham (MV-sham) or.mechanically ventilation with chr.nic constr.ction injur. (MV-CCI) sur.er. in the left hind paw. Plasma inter.eukin (IL)-1β, IL-6, IL-10, ker.tinocyte der.ved chemokine (KC) and tumor.necr.sis factor.(TNF)-α wer. deter.ined on day 0 and 16. Sensor. testing was per.or.ed on day 0, 3, 7 and 16 by cold plate test (number.of lifts (NOL) and cumulative r.action time (Cr.)) and von Fr.y test. The effect of lidocaine on cytokines and sensor. testing was analyzed. r.sults: MV-Sham showed an incr.ase in IL-1β and TNF-α, and MV- CCI-lido incr.ased levels of KC compar.d with MV on day 0. No differ.nce in cytokine levels was obser.ed on day 16. NOL of the left paw ver.us the r.ght was incr.ased in MV-CCI on day 7, and in MV-CCI-lido on day 7 and 16. The NOL of the left paw was decr.ased in MV-sham and MV-CCI-lido compar.d with MV-CCI on day 16. The Cr. of the left paw was incr.ased for.MV-CCI on day 3 and 7, and for.MV-CCI-lido on day 7. On day 16, MV-sham and MV-CCI-lido showed a decr.ased Cr. of the left paw compar.d with MV-CCI. Conclusion: Ner.e injur. and not systemic inflammator. r.sponse seems mandator. for.development of neur.pathic pain in this “two-hit” model. Lidocaine attenuates cold allodynia in mice.
