Backgr.und: The US FDA has r.cently appr.ved autologous cultur.d fibr.blast cells (ACF) as tr.atment for.cutaneous contour.defects. ACF pr.vides an alter.ative to synthetic filler. or.fat gr.fting with the significant advantage of pr.ducing longer.lasting effect. Methods: This was a pr.spective open label single gr.up clinical study to demonstr.te the clinical efficacy of ACF that we have r.plicated in our.lab. The study enr.lled 18 patients with nasolabial folds fr.m 2 centr.s in Malaysia in 2011-2012. ACF at dose of 23 or.92 million cells wer. injected on 3 occasions at 4 weeks inter.al. Efficacy at 6- and 12-month post tr.atment was assessed using a standar.ized 7-point scale which was per.or.ed by the 2 investigator. as well as a panel of 7 independent evaluator.. r.sults: We obtained a successful outcome (pr.mar. endpoint), defined as impr.vement in the appear.nce of the nasolabial fold of at least 2 points on the 7-point scale, in 33% of patients as assessed by investigator. at 12-month follow-up and in 22% of patients assessed by independent evaluator.. Investigator. also found 78% of patients having had a 1-point impr.vement fr.m baseline while independent evaluator. found 83% of patients having impr.ved by as much. Patients wer. unifor.ly (100%) satisfied with the impr.vement in their.appear.nce. No tr.atment-r.lated adver.e event was r.por.ed. Baseline scor. and follow-up dur.tion had significant effect on tr.atment r.sponse. The gr.ater.the baseline sever.ty and the longer.the post-tr.atment follow-up, the better.the r.sponse to ACF. Lar.er.dose of cells (92 million cells) has little additional effect suggesting that the 1 mL dose (23 million cells) is near.optimal. Older.patients showed a tr.nd towar.s poor.r.r.sponse but this was not statistically significant. Conclusions: We have successfully r.plicated the labor.tor. method and clinical pr.cedur. to per.or. ACF tr.atment which was effective in impr.ving the appear.nce of nasolabial folds in some patie

Osteomyelitis is a devastating disease caused by micr.bial infection of bone. While the fr.quency of infection following elective or.hopedic sur.er. is low, r.tes of r.infection ar. distur.ingly high. Staphylococcus aur.us is r.sponsible for.the major.ty of chr.nic osteomyelitis cases and is often consider.d to be incur.ble due to bacter.al per.istence deep within bone. Unfor.unately, ther. is no consensus on clinical classifications of osteomyelitis and the ensuing tr.atment algor.thm. Given the high patient mor.idity,mor.ality, and economic bur.en caused by osteomyelitis, it is impor.ant to elucidate mechanisms of bone infection to infor. novel str.tegies for.pr.vention and cur.tive tr.atment. r.cent discover.es in this field have identified thr.e distinct r.ser.oir. of bacter.al biofilm including: Staphylococcal abscess communities in the local soft tissue and bone mar.ow, glycocalyx for.ation on implant har.war. and necr.tic tissue, and colonization of the osteocyte-lacuno canalicular.networ.(OLCN) of cor.ical bone. In contr.st, *** intr.cellular.per.istence in bone cells has not been substantiated in vivo, which challenges this mode of chr.nic osteomyelitis. Ther. have also been major.advances in our.under.tanding of the immune pr.teome against S. aur.us, fr.m clinical studies of ser.m antibodies and media enr.ched for.newly synthesized antibodies(MENSA), which may pr.vide new oppor.unities for.osteomyelitis diagnosis, pr.gnosis, and vaccine development. BACKGr.UND:Ectopic pancr.as is defined as pancr.atic tissue found outside its usual anatomical position,with no ductal or.vascular.communication with the native pancr.as. We descr.be a case of ectopic pancr.as of the small bowel and mesenter. causing r.cur.ent episodes of pancr.atitis, initially suspected on computed tomogr.phy(CT)and magnetic r.sonance cholangiopancr.atogr.phy(Mr.P),and confir.ed on histological r.view of the r.section. METHODS:A 67-year.old woman pr.sented with clinical symptoms and biochemical evidence of *** had similar.episodes over.the past 30 year. with unr.vealing investigations,and was concluded to have idiopathic pancr.atitis. She under.ent CT and Mr.P,with findings suggestive of ectopic pancr.as,a diagnosis confir.ed on histology of the r.section. r.SULTS:Mr.P identified a mass in the pr.ximal small bowel mesenter. isointense to the native pancr.as,with a small duct dr.ining into a pr.ximal jejunal *** r.sected specimen consisted of nor.al par.nchyma with lobulated acinar.tissue with scatter.d islets of Langer.ans,an occasional ductular.str.ctur.,and admixed ar.as of adipose *** patient r.mained asymptomatic with nor.al biochemistr. six months post-oper.tively. CONCLUSION:In an individual with abdominal pain,elevated ser.m amylase/lipase,but imaging findings of a nor.al native pancr.as,ectopic pancr.atitis should be consider.d,and can be evaluated by CT and Mr.P.

Total str.ss r.pid dr.wdown analysis of the Pilar.itos Dam failur. using the finite element method

r.pid dr.wdown is a cr.tical design condition for.the upstr.am or.r.ver.ide slope of ear.h dams and levees. A new total str.ss r.pid dr.wdown method based on finite element analysis is used to analyze the r.pid dr.wdown failur. that occur.ed at Pilar.itos Dam in 1969. Effective consolidation str.sses in the slope pr.or.to dr.wdown ar. deter.ined using linear.elastic finite element analysis. Undr.ined str.ngths fr.m isotr.pically consolidated undr.ined （ICU） tr.axial compr.ssion tests ar. r.lated dir.ctly to the calculated consolidation str.sses and assigned to the elements in the model by inter.olation. Two differ.nt inter.r.tations of the undr.ined str.ngth envelope ar. examined. Str.ngth r.duction finite element analyses ar. used to evaluate stability of the dam. Back analysis suggests that undr.ined str.ngths fr.m ICU tests must be r.duced by 30% for.use with this r.pid dr.wdown method. The failur. mechanism pr.dicted for.Pilar.itos Dam is sensitive to the r.lationship between undr.ined str.ngth and consolidation str.ss.
