Weight regain after intragastric balloon for pre-surgical weight loss

Surgeon preferences in the treatment of thumb carpometacarpal osteoarthritis

A mimic of soft tissue infection: intra-arterial injection drug use producing hand swelling and digital ischemia

BACKGROUND: Inadvertent intra-arterial injection of illicit substances is a known complication of injection drug use and can lead to severe complications, including infection, ischemia and compartment syndrome. Identifying complications of intra-arterial injection can be difficult, as clinical manifestations overlap with other more common conditions such as cellulitis and soft tissue infection, and a history of injection drug use is frequently not ***: A 37-year-old male patient presented with 24 hours of right hand pain, erythema and swelling. Despite classic "track marks", he denied a history of injection drug use, and vascular insults were not initially considered. After failing to respond to three days of aggressive treatment for suspected deep-space infection, an arteriogram demonstrated findings consistent with digital ischemia of embolic ***: As a result of the delay in diagnosis, the lesion was not amenable to reperfusion and the patient required amputation of the distal ***: Practitioners should be alert to the possibility of intra-arterial injection and resulting complications when evaluating unusual extremity infections or unexplained ischemic symptoms, even in the absence of a definite history of injection drug use.

Heat and Mass Transfer for a Nanofluid Flow in Fluidized Bed Dryer in Presence of Induced Magnetic Field

This research entails the study of heat and mass transfer of nanofluid flow in a fluidized bed dryer used in tea drying processes in presence of induced magnetic field. A mathematical model describing the fluid flow in a Fluidized bed dryer was developed using the nonlinear partial differential equations. Due to their non-linearity, the equations were solved numerically by use of the finite difference method. The effects of physical flow parameters on velocity, temperature, concentration and magnetic induction profiles were studied and results were presented graphically. From the mathematical analysis, it was deduced that addition of silver nanoparticles into the fluid flow enhanced velocity and temperature profiles. This led to improved heat transfer in the fluidized bed dryer, hence amplifying the tea drying process. Furthermore, it was noted that induced magnetic field tends to decrease the fluid velocity, which results in uniform distribution of heat leading to efficient heat transfer between the tea particles and the fluid, thus improving the drying process. The research findings provide information to industries on ways to optimize thermal performance of fluidized bed dryers.

The association between radiological spreading pattern and clinical outcomes in necrotizing external otitis

Objectives: Necrotizing external otitis(NEO) is a rare infectious disease of the skull base. The purpose of this study was to determine whether clinical outcomes of NEO can be correlated to different infectious spread ***: Retrospective chart review from 2010 to 2019 with NEO patients, who were divided into two cohorts: single spreading patterns(group A) or complex spreading patterns(group B) as diagnosed by *** symptoms, diagnostic and treatment delay, course of disease, complications, and duration of antibiotic exposure were retrospectively collected from patient ***: 41 NEO patients were included, of which 27 patients belonged to group A(66%). The diseaserelated mortality rate was 12.2% among the entire cohort, no differences were found between group A and B. Higher rates of ***(42.9% vs 14.8% P = 0.047) and N. IX palsies were found in group B compared to group A(28.6% vs 3.7%, P = 0.039). The median duration of antibiotic use was significantly different for a complex spreading pattern, clinical recovery and hospitalizations. Complications were associated with higher diagnostic delay and with a complex spread pattern. The median duration of follow-up was 12.0(IQR 6.0-19.5) ***: NEO is a severe disease, with significant mortality and morbidity(cranial nerve palsies). The radiological spread pattern may assist in predicting clinical outcome. Furthermore, complex spread patterns are associated with higher rates of clinical nerve palsies(N. VII and ***), complications, surgery rates and longer duration of antibiotic use. Diagnostic delay was associated with mortality, complications and facial palsies.
