Before the 70s, in Brazil, each city had its morbidly obese, considered as the “excessive fats”, in very small numbers if we compare it with the current percentage. There was no classification of the degree of obesity by the body mass index (BMI) as we have today. By chance, on a Saturday in June 1975, at the Outpatient Clinic of the 23rd Infirmary of Santa Casa da Misericórdia Hospital in Rio de Janeiro, arrived the patient I. S., 41 years old, 1.70 m tall, supported by her two children, weighing 210 kg in weight body. Knowing that bariatric surgery only appeared in the 1980s, before that, patients with morbid obesity were left to their own devices, with hypertension and diabetes. The patient I. S. was hospitalized for 3 years in our Plastic Surgery Service, having received nutritional monitoring, had sporadic discharges and undergone 9 reparative plastic surgeries. She was discharged weighing 71 kg, with self-esteem recovered and happy to start a new life, without hypertension and diabetes.

Breast cancer is a heterogeneous and complex disease with different situations of threat to their patients, which can cause psychological discomfort, anxiety and a depressed state in women. The aim of this study was to analyze the performance of nursing to psychological repercussions on living with breast cancer. This is a descriptive and cross-sectional study with qualitative characteristics. It was conducted with professional nurses in Basic Health Units (UBS) in the municipality of Juazeiro do Norte-CE, Northeast of Brazil, located in the region of Cariri. The study began to be developed after the approval of the Research Ethics Committee, CAAE: 50809315.8. 0000.5624. data collection occurred through a semi-structured interview. For the interpretation of the results, the technique of content analysis was used. The results showed that nurses realize that emotional support, care and assistance in coping, improve the construction of a good performance of the professional nurse. However, it is possible to realize the need for information and qualified assistance by multidisciplinary team to these patients and even their family members. It was identified as difficulty in assisting the denial of patient to start the treatment, but the facility is in the intimacy that the professional nurse can engage with the customer. It highlighted the care through dialogue and health education. And, finally, the nurse is aware of such importance of his/her role when supporting the patient. Thus, the improvement of professional assistance was evidenced as regards emotional support, reception, qualified listening, health education measures leading to self-knowledge, self-esteem and acceptance of the disease and the creation of bonds with the patients.

Objective: To characterize the scientific production related to the impact of urinary incontinence on quality of life of pregnant women. Method: An integrative review carried out from September 2014 to May 2015. The search was conducted in six databases, and a final sample of seventeen publications was presented. The data were presented in tables. Results: Incontinence related factors are: Being older, second pregnancy, multiparity, constipation, smoking, and obesity. Stress incontinence is the most occurring in pregnant women, causing an adverse impact on quality of life. Discussion: It is for health professionals who try to create effective coping strategies for minimizing consequences generated by the accidental loss of urine. Conclusion: Urinary incontinence affects the mother’s life, resulting in social, behavioral, physical and emotional harm, taking this woman to be excluded from society and feel insecure about their health.

Introduction: Recent studies showed relation between mortality and bleeding in acute coronary syndromes. Objective: The objective is to analyze the prognosis of patients with acute coronary syndromes with or without in-hospital major and/or minor bleedings. Methods: This was a pro-spective data bank analysis study with 546 patients (39 with bleeding (group I) and 507 without bleeding (group II)) with acute coronary syndromes included between May 2010 and May 2013. Besides, Mehran bleeding risk score was calculated to all patients. The primary endpoint was all causes of in-hospital death and combined events. Comparison between groups was made by Anova and Q-square. Multivariate analysis was determined by logistic regression and was considered significant when p p = 0.005) and combined events (35.9% vs 11.4%, p < 0.001). The same results were observed in multivariate and long-term analysis. Con-clusions: Almost half of patients with acute coronary syndromes had higher risk of bleeding, and that with major or minor bleedings had greater deaths and combined events.

Objective: This study aims at assessing maternal and child care provided from prenatal to birth, so that it can reveal the weaknesses and potentialities of prenatal program in order to guide the development of strategic plans. Methods: This is an exploratory descriptive study with a quantitative approach, conducted in the city of Santa Cruz, state of Rio Grande do Norte, Brazil, from November 2012 to February 2013. The sample consisted of 200 women, chosen by draw, mothers of children under two years old who had attended the Primary Health Care System. The study used a semi-structured questionnaire used by the Ministry of Health. Our research project was approved by the Research Ethics Committee of the Federal University of Rio Grande do Norte, under the approval No. 311.613. Results: It was found that most mothers were rated from good to very good in the quality of care during prenatal and birth. As to the performance of prenatal and birth care services, most women performed those services under the municipality’s Public Health System, and the prenatal care primarily under Basic Health Care Units. Conclusion: In this assessment study, indicators regarding maternal and child care provided during the prenatal period were found to be positive, which contributes to the reduction of maternal and child mortality. The study also shows that Primary Health Care is essential to ensuring an effective, humane and comprehensive health care.

Neoplasms of the digestive system are uncommon in dogs, and may be of epithelial, neuroendocrine, hematopoietic and mesenchymal origin. The leiomyosarcoma is a tumor of mesenchymal origin of smooth, malignant, slow-growing muscles. Paraneoplastic syndromes can affect different systems, being the most commonly found: hypercalcemia, hypoglycemia, cachexia and anemia. There are rare cases of mesenchymal tumors of the gastrointestinal tract in dogs that result in hypoglycaemia with clinical signs. The objective of this study is to report a case of gastric leiomyosarcoma associated with clinical hypoglycemia. The patient had neurological signs such as ataxia and seizures associated with hypoglycemia, which did not recur after surgical excision of the tumor. The definitive diagnosis was based on the histopathological and immunohistochemical examination of the tumor.

AIM To determine the best option for bowel preparation [sodium picosulphate or polyethylene glycol(PEG)] for elective colonoscopy in adult *** A systematic review of the literature following the PRISMA guidelines was performed using Medline, Scopus, EMBASE, Central, Cinahl and Lilacs. No restrictions were placed for country, year of publication or language. The last search in the literature was performed on November 20th, 2017. Only randomized clinical trials with full texts published were included. The subjects included were adult outpatients who underwent bowel cleansing for elective colonoscopy. The included studies compared sodium picosulphate with magnesium citrate(SPMC)and PEG for bowel preparation. Exclusion criteria were the inclusion of inpatients or groups with specific conditions, failure to mention patient status(outpatient or inpatient) or dietary restrictions, and permission to have unrestricted diet on the day prior to the exam. Primary outcomes were bowel cleaning success and/or tolerability of colon preparation. Secondary outcomes were adverse events, polyp and adenoma detection rates. data on intention-totreat were extracted by two independent authors and risk of bias assessed through the Jadad scale. Funnel plots, Egger's test, Higgins' test(I2) and sensitivity analyses were used to assess reporting bias and heterogeneity. The meta-analysis was performed by computing risk difference(RD) using MantelHaenszel(MH) method with fixed-effects(FE) and random-effects(RE) *** Manager 5(RevMan 5) version 6.1(The Cochrane Collaboration) was the software chosen to perform the ***662 records were identified but only 16 trials with 6200 subjects were included for the meta-analysis.
