Acute exacerbations of asthma are one of the leading causes of emergency room visits and hospitalizations in children. Asthma exacerbations also demonstrate a seasonal pattern in which high rates can be seen at a particular season and knowledge of the seasonal pattern of asthma exacerbation in any locality can improve asthma management. Objective: To determine the prevalence, management outcome and seasonal pattern of emergency department visits for acute exacerbation of asthma. Methods: This was a retrospective review of the emergency department register of all asthma admissions in the Rivers State University Teaching Hospital, Portharcourt, Nigeria over a five year period from January 2014 to December 2018. Results: A total of 205 (2.9%) asthma-related visits were recorded out of 7046 emergency room visits. The mean age of the patients was 6.1 (±4.2) yrs, with an M:F ratio of 1.97:1. The median length of hospital stay was 24 hrs. Children older than 11 yrs were more likely to be admitted for >48 hrs (OR 4.18, 95% CI;1.67, 10.39, P;0.003). Emergency department visits for asthma were more in the rainy season April-September 129 (63.9%) with a peak in May. There is a significant variation in the seasonal pattern of emergency room asthma admission with age (P = 0.018). Children >3 yrs have their highest admission rates in May while younger children experience two peaks in February and November. Conclusion: Asthma exacerbation is more in rainy season among children in Portharcourt although there is a variation in seasonal pattern of asthma exacerbation with age.

进入21世纪以来,中国对外直接投资快速扩张,投资国家数量和投资规模都持续扩大。2015年中国对外直接投资总量居世界第二,首次超过同期吸引外资的总量,实现资本的净输出。而随着中国对外经济合作的不断加深,其对非洲国家的直接投资规模也在逐渐扩大。其中,作为中国在非洲的坚实伙伴,加纳已与中国建立了数十年的友谊,越来越多的中国企业也选择在加纳进行直接投资,同时中国政府还一直不断对加纳进行大量的经济援助。近年来,在中国政府积极鼓励企业“走出去”,并提出“一带一路”倡议和中非合作论坛等一系列新政策的大背景下,更多的中国企业选择进入加纳并进行投资。从加纳方面来看,国内的外商直接投资以及注册数量最多的外资企业都来自中国,这对加纳制造业发展有十分积极的影响。在梳理相关文献的基础上,本文首先分析了加纳制造业的发展历史与现状,以及目前存在的一系列问题,同时也分析了中国对加纳直接投资的现实特征和驱动因素。随后,本文从资本积累、技术进步、劳动力就业和贸易结构四个方面入手,详细分析了中国对外直接投资对加纳制造业发展的影响机理,为进一步的实证研究提供理论基础。再次,根据所提出的影响机理,本文通过构建相应模型对中国对外直接投资对加纳制造业的影响进行了研究和分析,结果表明中国的OFDI对加纳制造业的总体发展有显著的促进作用,同时也有助于改善加纳制造业的资本积累、技术进步和就业增长,但并不利于改善加纳制造业的贸易结构调整。最后,本文认为加纳政府也应当采取相应措施,加强对外国直接投资流入的正确管理,从而提高制造业的产出,并扭转之前存在的问题。另一方面,根据所得出的结论,本文建议加纳政府应当制定相关政策,确保市场正常运行,吸引投资目标明确的外国投资者,同时不断进行自主创新,加强国内教育水平,从而从多个方面促进加纳制造业的快速发展。

Adequacy of Water Use Resources for Drinking and Irrigation, Study Case of Sarh City, Capital of Moyen-Chari Province, CHAD

Our objective is to evaluate adequacy of water for consumption and irrigation. We collected nineteen water samples from 21 to 26, May 2017. Twenty-two physico-chemical and bacteriological parameters were taken into account, electrical conductivity, pH, total hardness, bicarbonate, nitrates, sulfates, chlorides, calcium, sodium, magnesium, potassium, dissolved oxygen and biochemical oxygen demand, chemical oxygen demand, total coliforms, Escherichia Coli, and fecal coliforms. To evaluate water intended for consumption, average values measured parameters were compared to [1] for quality water drinking and we evaluated overall quality of water by means of simplified SEQ-Water grid. Concerning irrigation water, evaluation is based on interpretation of quality parameters of irrigation water that we have calculated;among these parameters we can mention the percentage of sodium, rate of sodium absorption, ratio of magnesium adsorption, residual sodium carbonate. Kelly ratio, Corrosivity ratio and Permeability Index. Results show that surface water is weakly mineralized compared to groundwater and presents a good chemical quality;however, it is slightly altered by presence of organic pollutants. The overall assessment of groundwater quality shows that about 47% of water sampled varies between average quality and very poor quality for consumption. This poor quality is mainly due to presence of nitrates in water, whose content sometimes exceeds the limit authorized by [1]. From a bacteriological point of view, germs of faecal origin such as Total Coliforms, Escherichia Coli and Faecal Coliforms contaminate all groundwater points sampled. They are of poor quality and unfit for human consumption without prior treatment. We also evaluated the quality of water used for irrigation using 6 parameters. BACKGROUND Dental fluorosis is caused by excessive fluoride ingestion during tooth *** a consequence,there is a higher porosity on the enamel surface,which causes an opaque *** The aim of this study was to identify a dental intervention to improve the smile in patients with tooth *** aims were to relate the stain size on fluorotic teeth with the effectiveness of stain removal,enamel loss and procedure time using a manual microabrasion technique with 16%hydrochloric acid(HCL).METHODS An experimental study was carried out on 84 fluorotic teeth in 57 adolescent patients,33 females and 24 males,with moderate to severe *** means,standard deviations and percentages were analyzed using nonparametric statistics and ArchiCAD 15 software was used for the variables including stain size and effectiveness of stain *** The average enamel loss was 234μm and was significantly related to the procedure time categorized as 1-4 min and 4.01-6 min,resulting in a P>*** microabrasion technique using 16%HCL was effective in 90.6%of patients and was applied manually on superficial stains in moderate and severe *** time was less than 6 min and enamel loss was within the acceptable *** Microabrasion is a first-line treatment;however,the clinician should measure the average enamel loss to ensure that it is within the approximate range of 250μm in order to avoid restorative treatment.

Three monogenetic cones in the Baossi–Warack area, Ngaoundéré, Adamawa Plateau forming part of the Cameroon Volcanic Line(CVL) are documented in this study. Basaltic lavas(<1 km^3) scattered around these vents and restricted volcaniclastic deposits were emplaced by Hawaiian and mild strombolian style eruptions. The lavas are porphyritic, mainly composed of olivine(chrysolite) and clinopyroxene(diopside and augite) phenocrysts and plagioclase(andesine) microphenocrysts. Accessory minerals include titano-magnetite and titano-hematite, nepheline,apatite and amphibole xenocrysts. Sanidine occurs in some samples and sodi-potassic albite in others. Some olivines and clinopyroxenes exhibit resorbed margins and thin reaction rims while plagioclase displays oscillatory zoning, and sieved textures as a result of magma mixing. Whole-rock geochemistry data indicates that the lavas are silica-undersaturated, composed of basanites and basalts, showing little compositional variations(SiO_2: 39.20 wt.%–48.01 wt.%,MgO: 5.29 wt.%–9.70 wt.%). Trace elements patterns of these lavas suggest they are enriched in LILE including Pb,probably due to crustal contamination. REE patterns suggest cogenetic magmas below Baossi 1 and Baossi 2 volcanoes,and distinct sources below Warack volcano and nearby *** lavas studied show affinity to high-μ(HIMU), enriched type Ⅰ(EM1) and Oceanic Island Basalt(OIB)-like mantle signatures and thus indicate a heterogeneous mantle source underneath the vents as noted at other monogenetic and polygenetic volcanoes along the CVL. Primary melts derived from low degrees of partial melting(0.5%–2%) and encountered low rates of fractionation, and crustal contamination coupled with magma mixing. These melts evolved independently through structural weaknesses in the basement.

The metabolic syndrome(MS), which includes obesity,dyslipidaemia, hypertension and hyperglycaemia according to the most widely accepted definitions now used, is one of the most common post-transplant complications, with a prevalence of 44%-58%. The MS, together with the immunosuppression, is considered the main risk factor for the development of cardiovascular disease(CVD) in transplant recipients, which in turn accounts for 19%-42% of all deaths unrelated to the graft. The presence of MS represents a relative risk for the development of CVD and death of 1.78. On the other hand, non-alcoholic fatty liver disease(NAFLD), considered as the manifestation of the MS in the liver, is now the second leading reason for liver transplantation in the United States after hepatitis C and alcohol. NAFLD has a high rate of recurrence in the liver graft and a direct relation with the worsening of other metabolic disorders, such as insulin resistance or diabetes mellitus. Consequently, it is vitally important to identify and treat as soon as possible such modifiable factors as hypertension, overweight, hyperlipidaemia or diabetes in transplanted patients to thus minimise the impact on patient survival. Additionally, steroid-free regimens are favoured, with minimal immunosuppression to limit the possible effects on the development of the MS.

研究背景:大剂量马法兰（high-dose melphalan,HDM）是多发性骨髓瘤（multiple myeloma,MM）自体造血干细胞移植（autologous hematopoietic stem cell transplantation,autoHSCT）前的标准预处理方案。尽管患者的反应率和存活率有所提高,但复发率和与方案相关的安全性仍然是一个挑战。目的:本研究主要目的是比较接受白消安、环磷酰胺和依托泊苷（BVC）和HDM作为自体造血干细胞移植预处理方案的MM患者的疗效和预处理毒副作用的差异。方法:对2015年8月至2020年12月期间来自山东省3个主要移植中心（山东大学齐鲁医院、山东省立医院和山东省千佛山医院）的29-65岁（中位年龄52岁）接受自体造血干细胞的78例MM患者进行回顾性分析。37名患者接受BVC预处理方案,而41名患者接受HDM预处理方案。多发性骨髓瘤的诊断使用IMWG标准进行,并使用国际分期系统进行分期。纳入标准包括肌酐低于***/dL、无活动性感染或经肺功能检查确定的严重阻塞性和/或限制性肺疾病（即DLCO＜50%和/或FEV1＜50%和/或FVC＜50%）且心脏射血分数（EF）大于40%的成人（年龄在18-65岁之间）。患者首先接受若干个周期的诱导化疗（通常为3-6个周期）,对疗效达部分缓解以上的患者进行干细胞动员和收集。自体外周血造血干细胞（PBHSCs）用作自体干细胞移植细胞。76例患者接受大剂量环磷酰胺联合粒细胞集落刺激因子（G-CSF）化疗动员采集外周血造血干细胞。2名患者使用plerixafor联合G-CSF稳态动员采集干细胞。CD34+细胞平均采集量为4.61 × 106/kg。CD34+细胞最小采集量为2×1 06个/kg。37名MM患者接受BVC预处理方案:白消安在-7天、-6天和-5天以0.8mg/kg/天静脉注射。依托泊苷在-5天和-4天以400mg/m2/天静脉给药,环磷酰胺在-3天和-2天以50mg/m2/天静脉内给药。41名MM患者接受HDM预处理方案:马法兰总剂量为200mg/m2,分成-3天和-2天静脉注射,然后进行自身HSCT。预处理方案对待评估的移植受者中发生的毒副作用毒性的影响如下:a.方案相关的毒性;包括血液学毒性和非血液学毒性。b.中性粒细胞和血小板植入的时间。c.住院时间。要分析的结果包括:a.移植后疾病的反应状态。b.移植1年时复发率。c.移植后100天内的移植相关死亡率。d.移植 1 年时无进展生存期（progression-free survival,PFS）。e.移植1年时的总生存期（overall survival,OS）。使用SPSS版本26对两组之间的数据进行分析和比较。描述性统计用于报告结果,包括人口统计学,疾病相关因素和移植相关因素。使用Kaplan-Meier方法估计PFS和OS的概率,并使用Log-rank检验对组进行比较。使用Cox比例风险回归模型对OS,PFS和累积复发发生率进行多变量分析,该模型用于估计与HDM相比BVC的风险比（HR）。结果:本研究共纳入MM患者78例,其中37名患者在接受BVC 预处理方案后接受autoHSCT,而41名患者在接受HDM预处理方案后接受autoHSCT。两组的总缓解率（ORR）、PFS、OS和复发率在统计学上相似。BVC组的中位随访时间为540天,HDM组为436天。HDM组肺毒性发生率显著增加（BVC为36.6%对5.4%,P=0.002）。BVC组患者的住院时间明显长于HDM组（中位数20天,范围:BVC为18-27;与中位18天范围:HDM为13-27,P＜0.0001）。BVC组（7天,范围:4-12）中性粒细胞减少的中位持续时间明显长于HDM组(6天,范围:（2-12）;P=0.002。有两例TRM病例,每组一例。在多变量亚组分析中,BVC调理在PFS、OS和复发方面与HDM调理有统计学相似。但是,低风险疾病分期显示可以预测PFS和复发的累积发生率,ISSⅠ和ISSⅡ疾病阶段的患者PFS明显更长;（ISS Ⅰ:HR=0.186,P=0.006,ISS Ⅱ:HR=0.240,P=0.022）;并延迟复发时间;（ISSⅠ:HR=0.220,P=0.013,ISS Ⅱ:HR=0.262,P=0.036）。β-2 微球蛋白预测 OS,β-2微球蛋白水平升高显示显著恶化OS（HR=1.356,P=0.041）。年龄,性别,MM亚型,KPS,移植前使用的化疗方案/周期数或移植时的疾病状况与两组的PFS,OS或复发率无关。结论:尽管两种方案的反应率和生存率似乎相似,但与HDM方案相比,BVC方案在非血液学毒性（肺毒性）方面似乎更优越,但在血液学毒性（血细胞减少）方面较差。本研究显示BV

Objective: To investigate maternal and neonatal complications resulting from inadvertent immunization against rubella-measles during the first trimester of pregnancy. Methods: A prospective and descriptive study was carried out, including a total of 1,924 pregnant women, 175 (9.1%) of which were classified as non responding to infection by the rubella virus. They underwent clinical and ultrasonographic follow-up to dismiss maternal or fetal complications and complications at the time of delivery. The infant was checked to determine demographic, anthropometric, serological and clinical features at the time of birth. Results: No women had complications during the pregnancy, including exanthematic symptoms. 174/175 newborns were studied;one pregnancy was interrupted based on non-medical arguments. The findings in terms of the analyzed patients suggest a benign evolution after inadvertently immunizing the pregnant women, which support studies with similar results. No complications during the course of the pregnancy or phenotypic alterations of the infant at the time of birth are suggested.

Dengue is a global health problem without current specific treatment nor safe vaccines *** severe dengue is related to pre-existing non-neutralizing dengue virus(DENV)antibodies,the role of T cells in protection or pathology is *** cutaneous DENV infection in immunocompetent mice we previously showed the generation of PNA+germinal centers(GCs),now we assessed the activation and proliferation of B and T cells in draining lymph nodes(DLNs).We found a drastic remodelling of DLN compartments from 7 to 14 days post-infection(dpi)with greatly enlarged B cell follicles,occupying almost half of the DLN area compared to*24%in na?ve *** clusters of proliferating(Ki-67+)cells inside B follicles were found 14 dpi,representing*33%of B cells in DLNs but only*2%in noninfected *** GCs,we noticed an important recruitment of tingle body macrophages removing apoptotic *** contrast,the percentage of paracortex area and total T cells decreased by 14–16 dpi,compared to *** randomly distributed Ki-67+T cells were found,similar to non-infected ***69 expression by CD4+and CD8+T cells was minor,while it was remarkable in B cells,representing 1764.7%of change from basal levels 3 *** apparent lack of T cell responses cannot be attributed to apoptosis since no significant differences were observed compared to noninfected *** study shows massive B cell activation and proliferation in DLNs upon DENV *** contrast,we found very poor,almost absent CD4+and CD8+T cell responses.
