Epidemiological, Clinical Aspects and Outcome of Measles in a Low-Income Country in 2023

Introduction: Measles remains a worrying health problem in sub-Saharan African countries. There have been measles outbreaks in Cameroon with the most recent occurring between October 2022 and September 2023. The Centre region of Cameroon was the most affected in the country and being at the frontline of the fight against this illness, we conducted this study in order to determine the epidemiological and clinical characteristics as well as the outcome of children who contracted the measles and were hospitalized at the Yaoundé Gynaeco-Obstetric and Pediatric Hospital. Methodology: We conducted a cross-sectional descriptive and prospective study for one year from October 2022 to November 2023 corresponding to the duration of the outbreak. We included all children admitted in the pediatric unit for measles and its complications during this period. Results: In total, 60 children were enrolled. Their caregivers were mostly their mothers who had a mean age of 34.71 ± 9.55 years living in significant precarious conditions for the majority. The median age of children was 16 months (09 - 30 months). Twenty six percent of children (16) were less than 9 months. Girls were predominant (55%). Most children were not up to date with their routine EPI vaccination (80%) and most did not receive the measles and Rubella vaccine (76.67%). Clinical manifestations, included fever, conjunctivitis, and cough. Skin rash and catarrh (98.33% and 86.67% respectively) are the case definition signs of measles. The outcome was unfavorable for 4 children. One had a neurological disorder: coma and three presented with pneumonia and severe respiratory distress (6.67%). Conclusion: Measles is still being a reality and claiming lives in our context, emphasis should be made on immunization coverage and if possible, advocacies should be formulated to decrease the age of measles vaccine administration.

Background: Human African trypanosomiasis (HAT) occurs in three historical foci in the Central African Republic. Objective: To describe the morbi-mortality of childhood HAT in rural Central African Republic with the aim of early management. Methods: Descriptive and analytic cross-sectional study conducted from January 1, 2017 to March 30, 2018 at Nola prefectural hospital. This study was included all children seen as outpatients and/or hospitalized in the pediatric wards, in whom the diagnosis of HAT was confirmed. Statistical analysis was performed using Epi-info software. Results: Forty children were included from Bilolo (60%), Nola (30%) and Salo (10%). The sex ratio was 0.66 with a median age of 8.65 ± 12.48 years. Fever (82.5%), nocturnal insomnia (75%), daytime somnolence (67.5%), headache (65%), polyarthralgia (62.5%), convulsions 52.5% (n = 21), tremor (27.5%), trypanidism (7.5%) and delirium (2.5%) were the main functional signs. Examination signs were adenopathy (52.5%), paresthesia (30%), decreased cutaneous-abdominal reflexes (25%) and osteoarticular reflexes (17.5%), hyperesthesia (20%), extrapyramidal hypertonia (15%) and depression (2.5%). The children were in the lymphatic-blood phase in 65% of cases, and meningoencephalitis in 35%. Pentamidine was administered in 65% of cases. Four children died and 6 had neurological sequelae. There was an association between age under 5, Bilolo’s focus, the children’s history and the severity of the disease. Age under 5, gender, household, children’s activity, history and occurrence of sequelae were also associated. Conclusion: HAT remains a permanent threat to Central African children. Any clinical presentation combining long-term infectious signs and unexplained neurological or neuropsychological disorders must be treated with caution.

The evolution in knowledge and application of disaster risk reduction in the 25 years of global cooperation on this issue has been uneven. While advances in knowledge have improved our understanding of the full nature of risk—the combination of hazards meeting vulnerability—the application of such knowledge has not been conducive to the development of institutional and technical mechanisms to address the full range of risk elements. Governance of risk(policies, legislation, and organizational arrangements) still focuses largely on preparing to respond to the hazards and planning for recovery. This leaves largely unattended the vulnerability component of risk, which is the only component on which change can be *** arrangements, risk assessments, early warning systems, and other institutional and technical capacities still concentrate on natural hazards and this is the main change that remains to be substantively addressed.

Recent shifts in shade tolerance and disturbance traits in forests of the eastern United States

Background:Current forests of the eastern USA have the potential to succeed in composition to more shade-tolerant ***,long-term processes of transition from fire-tolerant tree species to fire-sensitive species and effects of current land use on forests may interfere with successional ***:I examined if forests in three regions have increased in shade tolerance and if life history strategy groups that represent response to disturbance(i.e.,fire-tolerance,early-successional species based on low shade tolerance,mid-successional species,late-successional species,and trees valued for traits related to short harvest rotations)have changed,using Forest Inventory and Analysis surveys,adjusted for comparison,and generalized linear mixed models to assess approximately 30 year trends,with adjustments to equalize different survey ***:Although statistically significant,a slight increase of 2 to 4%in regional mean shade tolerance may not be ecologically *** the central East,mid-successional species replaced early-successional species and early-successional species replaced fire-tolerant oaks,resulting in an overall shift from fire-tolerant oaks to mid-successional *** fire-tolerant pine species and increased planted pine species were the major changes in the northern Southeast and Coastal ***:The successional process of increased composition by shade-tolerant species over time was overshadowed by land use changes that resulted in decreased fire-tolerant species and increased planted ***,frequent land use disturbance may continue to prevent the slow progress of compositional succession to very shade-tolerant species.

Trajectory from beech and oak forests to eastern broadleaf forests in Indiana, USA

Background:Historical forests(circa 1799 to 1846)of Indiana were predominantly composed of American beech(25%of all trees)and upland oaks(27%of all trees).I compared historical forest composition,using studies of smaller areas to approximate composition for uncommon species or genera(<4.5%of all trees)and forest types,to current forest composition and forest types in Indiana.I also compiled published historical density estimates from small studies and estimated current density for the ***:Current forests are diverse with an even composition of many eastern broadleaf *** addition to upland oaks,which decreased to 13%of all trees,only sugar maple currently comprises greater than 10%of all *** increasing species were yellow-poplar,red maple,black cherry,and eastern *** redcedar increased from no presence to one of the most abundant ten species(≥3.5%of composition),similarly to the pine genus,becoming more common currently than American *** or oak or beech-oak forest types(≥24%of all trees)became eastern broadleaf forests,with no dominant species,and black cherry,red maple,and eastern redcedar forest types now *** of historical oak or beech forests ranged from 28 trees per hectare to 175 trees per hectare,which probably represent savannas and *** mean density of Indiana forests is 385 trees per hectare,ranging from 180 trees per hectare to 450 trees per *** forests likely contain dense layers of woody vegetation,filling the midstory and replacing herbaceous vegetation in the ground ***:Historically dominant beech and oak forests in the eastern United States have transitioned to an alternative state of closed eastern broadleaf forests due to uncontrolled establishment of many tree species.

Pig Reproductive and Respiratory Syndrome Virus (PRRSV) Does Not Attach to Boar Sperm;It Affects Only the Velocity Pattern and the Mobility Pattern

The purpose of the study was to evaluate the sperm viability of semen infected with PRRSV viral particles, observing the effect of the Virus on the motility of boar sperm. The work was carried out at the FMVZ-BUAP Genetics and Reproduction Laboratory. 5 stallions were used. Each sample contained 1 × 106 sperm, the PRRS virus strain was ATCC-VR-2332 (0, 102, 104 and 106 copies of RNA/mL in triplicate), it was observed daily at the CASA;Hamilton Thorne®. Cells with MT (P 2 and 10X6 (P 2) 162.81 ± 10.65 and (106) 177.12 ± 5.77 vs 193.04 ± 4.62 of control. This indicates that altering these parameters would be related to fertility and the PRRS virus affects the LCV. Regarding the VSL, it was observed that the sperm infected with viruses 102, 104 and 106 of 48.00 ± 3.38, 49.88 ± 1.83 and 50.55 ± 2.24 Vs. 56.66 ± 1.68 of control respectively, the control would have greater possibilities of fertilizing the oocyte. In this study, it was found (P ≤ 0.01) in the VAP with 102 of 77.26 ± 5.16, 104 with 83.35 ± 2.41 and 106 with 81.29 ± 3.14 vs the control with 90.56 ± 2.07. Regarding the ALH there is (P 4 with 8.70 ± .26 and 106 with 9.64 ± 0.23 vs control 8.50 ± 0.27. Management of Testicular Cancers in Brazzaville

Introduction: Testicular cancer accounts for 5% of urological tumors, predominantly affecting young men. The aim of our study was to report the diagnostic and evolutionary aspects of testicular cancer cases treated in our center. Patients and Methods: A retrospective study conducted over a 15-year period involving 12 patients treated for testicular cancer at the University Hospital of Brazzaville. Results: The median age was 31 years (range 11 to 49 years), with a median consultation delay of 10.6 months (range 3 to 27 months). Scrotal mass was the most common reason for consultation. Cancer was bilateral in two patients. Two patients were admitted with metastatic disease. Histopathological examination favored germ cell tumors in 7 cases, two cases of non-Hodgkin’s malignant lymphoma, and one case of epididymo-testicular adenocarcinoma. Adjuvant chemotherapy resulted in complete remission in patients with germ cell tumors. However, neoadjuvant chemotherapy was not effective in patients admitted with advanced-stage disease. Conclusion: Testicular cancer is a rare condition that is curable in the majority of cases, but its management is often complicated in our setting due to delayed diagnosis caused by taboos surrounding genital organ pathologies.

The objective of this work was to carry out a morphological characterization of arbuscular mycorrhizal fungi in the rhizosphere of Xanthosoma sagittifolium L. Schott plants. The plant material used was the white and red cultivars of X. sagittifolium, belonging to age intervals of 3 - 6, 6 - 9, and 9 - 12 months. Three harvest sites were chosen in the Central Region of Cameroon. In each site, soil from the rhizosphere and plant roots was collected in a randomized manner. In the field, the agronomic parameters were evaluated. The physicochemical characteristics of the soils, the mycorrhization index, and the morphological characterization of the mycorrhizal types of each site were carried out. The results obtained show that the agronomic growth parameters varied significantly using the Student Newman and Keuls Test depending on the harvest sites. The soils’ pH in all sites was acidic and ranged between 4.6 and 5.8. The Nkometou site has a loamy texture while the Olembe and Soa sites have loam-clay-sandy and loam-clay textures respectively. The highest mycorrhization frequencies appeared at the Nkometou site, with 75 and 87.33% of the white and red cultivars plant roots at 6 - 9 and 3 - 6 months. The relative abundance of AMF arbuscular mycorrhizal fungal spores in the rhizosphere of X. sagittifolium plants varied with age and cultivar. There were 673 spores between 9 - 12 months in Nkometou in the red cultivar. Six AMF genera were identified in all the different soils collected: Acaulospora sp., Funneliformis sp., Gigaspora sp., Glomus sp., Scutellospora sp., and Septoglomus sp. The genus Glomus sp. was the most present at all age intervals in both cultivars.

Synthesis of Water-soluble, Polyester-based Dendrimer Prodrugs for Exploiting Therapeutic Properties of Two Triterpenoid Acids

Dendrimers are macromolecules characterized by high controlled size, shape and architecture, presence of inner cavities able to accommodate small molecules and many peripheral functional groups to bind target entities. They are of eminent interest for biomedical applications, including gene transfection, tissue engineering, imaging, and drug delivery. The well-known pharmacological activities of ursolic and oleanolic acids are limited by their small water solubility, non-specific cell distribution, low bioavailability, poor pharmacokinetics, and their direct administration could result in the release of thrombi. To overcome such problems, in this paper we described their physical incorporation inside amino acids-modified polyester-based dendrimers which made them highly water-soluble. IR, NMR, zeta potential, mean size of particles, buffer capacity and drug release profiles of prepared materials were reported. The achieved water-soluble complexes harmonize a polycationic character and a buffer capacity which presuppose efficient cell penetration and increased residence time with a biodegradable cell respectful scaffold, thus appearing as a promising team of not toxic prodrugs for safe administration of ursolic and oleanolic acids.
