Quality audit of colonoscopy reports amongst patients screened or surveilled for colorectal neoplasia

AIM： To complete a quality audit using recently pub- lished criteria from the Quality Assurance Task Group of the National Colorectal Cancer Roundtable. MeTHODS： Consecutive colonoscopy reports of patients at average/high risk screening, or with a prior col6rectal neoplasia （CRN） by endoscopists who perform 11 000 procedures yearly, using a commercial computerized endoscopic report generator. A separate institutional da- tabase providing pathological results. Required documen- tation included patient demographics, history, procedure indications, technical descriptions, colonoscopy findings, interventions, unplanned events, follow-up plans, and pathology results. Reports abstraction employed a stan- dardized glossary with 10% independent data validation. Sample size calculations determined the number of re- ports ***： Two hundreds and fifty patients （63.2± 10.5 years, female： 42.8%, average risk： 38.5%, per- sonal/family history of CRN： 43.3%/20.2%） were scoped in June 2009 by 8 gastroenterologists and 3 surgeons （mean practice： 17.1 ± 8.5 years）. Procedural indica- tion and informed consent were always documented. 14% provided a previous colonoscopy date （past polyp removal information in 25%, but insufficient in most to determine surveillance intervals appropriateness）. Most procedural indicators were recorded （exam date： 98.4%, medications： 99.2%, difficulty level： 98.8%, prep quality： 99.6%）. All reports noted extent of visualization （cecum： 94.4%, with landmarks noted in 78.8% - photodocu- mentation： 67.2%）. No procedural times were recorded. One hundred and eleven had polyps （44.4%） with ana- tomic location noted in 99.1%, size in 65.8%, morphol- ogy in 62.2%; removal was by cold biopsy in 25.2% （cold snare： 18%, snare cautery： 31.5%, unrecorded： 20.7%）, 84.7% were retrieved. Adenomas were noted in 24.8% （advanced adenomas： 7.6%, cancer： 0.4%） in this population with varying previous colonic investigations. CONCLUSION： This audit reveals lacking repo

A new theoretical approach to language has emerged in the past 10-15 years that allows linguistic observations about form-meaning pairings constructions — to be stated directly. Constructionist approaches aim to account for the full range of facts about language, without assuming that a particular subset of the data is part of a privileged 揷ore? Researchers argue that unusual constructions shed light on more general issues, and serve to illuminate what is required for a complete account of language.

采用整体退火遗传算法搜索3n混合极性Reed-Muller表达式,获得最优解从而达到简化逻辑电路目的.并有效地结合遗传算法的全局搜索和模拟退火算法的局部搜索能力,在退火阶段将父代中最优的2/3种群的染色体和子代中最优的2/3种群的染色体选中形成中间阶段种群,然后对该种群进行退火选择,选出染色体组成新的种群,再对新种群进行选择、交叉和变异操作.为了进一步加快整体退火遗算法的执行效率,本文采用数据不相关的方法计算适应函数.实验结果证明,该算法在保证最优结果的同时,可有效缩短CPU运行时间.

AIM： To evaluate the effects of chlorella crude extract （CCe） on intestinal adaptation in rats subjected to short bowel syndrome （SBS）. MeTHODS： Wistar rats weighing 230-260 g were used in the study. After anesthesia a 75% small bowel resection was performed. Rats were randomized and divided into groups. Control group （n = 10）： where 5% dextrose was given through a gastrostomy tube, enteral nutrition （eN） group （n = 10）： Isocaloric and isonitrogen eN （Alitraq, Abbott, USA）, study group （n = 10）： CCe was administrated through a gastrostomy tube. Rats were sacrificed on the fifteenth pcstoperative day and blood and tissue samples were taken. Histopathologic evaluation, intestinal mucosal protein and DNA levels, intestinal proliferation and apoptosis were determined in intestinal tissues, and total protein, albumin and citrulline levels in blood were studied. ReSULTS： In rats receiving CCe, villus lengthening, crypt depth, mucosal DNA and protein levels, intestinal proliferation, and serum citrulline, protein and albumin levels were found to be significantly higher than those in control group. Apoptosis in CCe treated rats was significantly reduced when compared to eN group rats. CONCLUSION： CCe has beneficial effects on intestinal adaptation in experimental SBS.

Increasing the detail of european land use/cover data by combining heterogeneous data sets

Data on land use and land cover(LULC)are a vital input for policy-relevant research,such as modelling of the human population,socioeconomic activities,transportation,environment,and their *** europe,CORINe Land Cover has been the only data set covering the entire continent consistently,but with rather limited spatial *** data sets have provided much better detail,but either have covered only a fraction of europe(*** Atlas)or have been thematically restricted(*** High Resolution Layers).In this study,we processed and combined diverse LULC data to create a harmonised,ready-to-use map covering 41 *** doing so,we increased the spatial detail(from 25 to one hectare)and the thematic detail(by seven additional LULC classes)compared to the CORINe Land ***,we decomposed the class‘Industrial and commercial units’into‘Production facilities’,‘Commercial/service facilities’and‘Public facilities’using machine learning to exploit a large database of points of *** overall accuracy of this thematic breakdown was 74%,despite the confusion between the production and commercial land uses,often attributable to noisy training data or mixed land *** learnt from this exercise are discussed,and further research direction is proposed.

Multi-scenario-based hazard analysis of high temperature extremes experienced in China during 1951-2010

China is physically and socio-economically susceptible to global warming-derived high temperature extremes because of its vast area and high urban population density. This article presents a scenario-based analysis method for high temperature extremes aimed at illustrating the latter＇s hazardous potential and exposure across China. Based on probability analysis, high temperature extreme scenarios with return periods of 5, 10, 20, and 50 years were designed, with a high temperature hazard index calculated by integrating two differen- tially-weighted extreme temperature indices （maximum temperature and high temperature days）. To perform the exposure analysis, a land use map was employed to determine the spatial distribution of susceptible human activities under the different scenarios. The results indicate that there are two heat-prone regions and a sub-hotspot occupying a relatively small land area. However, the societal and economic consequences of such an environmental im- pact upon the North China Plain and middle/lower Yangtze River Basin would be substantial due to the concentration of human activities in these areas.

Predictability of South China Sea Summer Monsoon Onset

Predicting monsoon onset is crucial for agriculture and socioeconomic planning in countries where millions rely on the timely arrival of monsoon rains for their livelihoods. In this study we demonstrate useful skill in predicting year-to-year variations in South China Sea summer monsoon onset at up to a three-month lead time using the GloSea5 seasonal forecasting system. The main source of predictability comes from skillful prediction of Pacific sea surface temperatures associated with el Ni?no and La Ni?na. The South China Sea summer monsoon onset is a known indicator of the broadscale seasonal transition that represents the first stage of the onset of the Asian summer monsoon as a whole. Subsequent development of rainfall across east Asia is influenced by subseasonal variability and synoptic events that reduce predictability, but interannual variability in the broadscale monsoon onset for east Asian summer monsoon still provides potentially useful information for users about possible delays or early occurrence of the onset of rainfall over east Asia.

Genetics of inflammatory bowel disease: The role of the HLA complex

The human leucocyte antigen （HLA） complex on chromosome 6p21.3 is the most extensively studied genetic region in Inflammatory bowel disease （IBD）. Consistent evidence of linkage to IBD3 （6p21.1-23）, an area which encompasses the HLA complex, has been demonstrated for both Crohn＇s disease and ulcerative colitis, and a number of replicated associations with disease susceptibility and phenotype have recently emerged. However, despite these efforts the HLA susceptibility gene （s） for IBD remain elusive, a consequence of strong linkage disequilibrium, extensive polymorphism and high gene density across this region. This article reviews current knowledge of the role of HLA complex genes in IBD susceptibility and phenotype, and discusses the factors currently limiting the translation of this knowledge to clinical practice.
