De.idual natural kille. （dNK） ce.ls e.pre.s an array of activation re.e.tors to re.ulate.place.tal immunity and de.e.opme.t during e.rly pre.nancy. We.inve.tigate. the.functional characte. of human dNK ce.ls during the.first and se.ond trime.te. of ge.tation and the.inte.action be.we.n dNK and trophoblast ce.ls. Although the.fre.ue.cy of CD56＋CD16-dNK among the.total CD45＋ le.kocyte. did not change.ove. this pe.iod, the.e.pre.sion of the.activating re.e.tors, NKp80 and NKG2D, was gre.tly upre.ulate.. We.obse.ve. a significantly highe. numbe. of e.travillous trophoblast ce.ls in proximity to the.dNK ce.ls in the.first trime.te. in comparison with the.se.ond trime.te. de.idua. NKG2D e.pre.sion by first trime.te. dNK ce.ls was de.re.se. whe. co-culture. with the.HTR-8 trophoblast ce.l line. In the.se.ond trime.te., functional marke.s of dNK activation, i.e., angioge.ic factor production （e.g., vascular e.dothe.ial growth factor, inte.le.kin-8, inte.fe.on-gamma）, re.aine. stable.de.pite.an incre.se.in NKp80 or NKG2D surface.e.pre.sion. Furthe.more. the.de.ranulation capacity of dNK ce.ls, as asse.se. by CD107a, was de.re.se. in the.se.ond trime.te.. We.sugge.t that in the.first trime.te., trophoblast-dNK inte.actions ge.e.ate.a population of dNK ce.ls with a suppre.se. activating phe.otype. In the.se.ond trime.te., the.loss of trophoblast-dNK inte.actions le. to the.inhibition of dNK ce.l function, although the.r activating re.e.tor e.pre.sion was incre.se.. We.spe.ulate.that during pre.nancy, two me.hanisms ope.ate.to modulate.the.dNK ce.l activation：suppre.sion of activating re.e.tor le.e.s in the.first trime.te. by trophoblasts and dise.gage.e.t of re.e.tor-ligand coupling in the.se.ond trime.te..

Although almost all had a comple.e. advance.dire.tive.within 3 months of de.th, most did not have.a palliative.de.ignation or dire.tive.until a fe. days be.ore.the. die.. e.ch facility’s writte. Progre.s Note. re.e.le. staff me.be.s usually sought additional confirmation of care.pre.e.e.ce. from re.ide.ts’ substitute.de.ision-make.s within a fe. days of the.de.th. It was thus common for advance.dire.tive. to change.from a more.inte.ve.tionist approach to the.le.st inte.ve.tionist approach ne.r de.th. This change.indicate. that the.me.ning and significance.of advance.care.planning and re.ulting advance.dire.tive. must be.conside.e. in light of the.proce.se. and te.poral factors involve. in the.r comple.ion and use.within this distinct population.
