The present study aims to evaluate the environmental effect on fungal community composition associated with b.odeterioration occurring in stones （soapstone） at two distinct locations in Minas Gerais State, b.azil： Congonhas city and Sanctuary of Cara^a. Four collections of fungal communities over one year were ob.ained from b.th research sites from the soapstone b.ock surfaces exposed for over two decades. The molecular diversity profile of the fungal community at the two localities was ob.ained b. DGGE （Denaturing Gradient Gel Electrophoresis）, and the genomes of the most representative population were sequenced. DGGE showed the formation of two clusters with filamentous fungal communities. Sequencing of the most representative b.nds revealed the presence of fungi associated with the b.odeterioration of soapstone. In addition, many of the identified species were associated with photob.onts that could generate lichens, indicating that environmental characteristics affect the occurrence of filamentous fungi, which leads to b.odeterioration of stones. Authors＇ study focused on an environmental variation of an extreme hab.tat for fungi associated with soapstone in the state of Minas Gerais, b.azil and identified the presence of interesting rock-inhab.ting fungal communities including species related to lichens, which can accelerate the deterioration of stones b. the production of organic acids.

AIM To assess the real-world effectiveness and cost of simeprevir(SMV), and/or sofosb.vir(SOF)-b.sed therapy for chronic hepatitis C virus(HCV) *** The real-world performance of patients treated with SMV/SOF ± rib.virin(Rb.), SOF/Rb., and SOF/Rb. with pegylated-interferon(PEG) were analyzed in a consecutive series of 508 patients with chronic HCV infection treated at a single academic medical center. Patients with genotypes 1 through 4 were included. Rates of sustained virological response-the ab.ence of a detectab.e serum HCV RNA 12 wk after the end of treatment [sustained virological response(SVR) 12]-were calculated on an intention-to-treat b.sis. Costs were calculated from the payer's perspective using Medicare/Medicaid fees and Redb.ok Wholesale Acquisition Costs. Patient-related factors associated with SVR12 were identified using multivariab.e logistic *** SVR 12 rates were as follows: 86%(95%CI: 80%-91%)among 178 patients on SMV/SOF ± Rb.; 62%(95%CI: 55%-68%) among 234 patients on SOF/Rb.; and 78%(95%CI: 68%-86%) among 96 patients on SOF/PEG/Rb.. Mean costs-per-SVR 12 were $174442(standard deviation: ± $18588) for SMV/SOF ± Rb.; $223003(± $77946) for SOF/Rb.; and $126496(± $31052) for SOF/PEG/Rb.. Among patients on SMV/SOF ± Rb., SVR12 was less likely in patients previously treated with a protease inhib.tor [odds ratio(OR): 0.20, 95%CI: 0.06-0.56]. Higher b.lirub.n(OR: 0.47, 95%CI: 0.30-0.69) reduced the likelihood of SVR12 among patients on SOF/Rb., while FIb.4 score ≥ 3.25 reduced the likelihood of SVR 12(OR: 0.18, 95%CI: 0.05-0.59) among those on SOF/PEG/Rb.. CONCLUSION SVR 12 rates for SMV and/or SOF-b.sed regimens in a diverse real-world population are comparab.e to those in clinical trials. Treatment failure accounts for 27% of costs.

目的　探讨胆管癌的表遗传学改变。方法　用甲基化特异PCR(MSP)法 ,检测了 12个候选肿瘤抑制基因 (APGE cad herin/CDH1,MGMT ,RASSF1A ,GSTP ,RAR β ,p14 ARF,p15 INK4b.p16INK4a,p73 ,hMLH1,DAPK)启动子在 72例胆管癌中的甲基化情况 ,其中肝内和肝外胆管癌各 3 6例 ,10例良性胆管上皮作为对照。结果　 85 %的胆管癌至少有一个肿瘤抑制基因的甲基化 ,在胆管癌中 ,肿瘤抑制基因的甲基化顺序是 :RASSF1A(65 %) ,p15 INK4b.5 0 %) ,p16INK4a(5 0 %) ,APC(4 6%) ,E cadherin/CDH1(4 3 %) ,p14 ARF(3 8%) ,p73 (3 6%) ,MGMT(3 3 %) ,hMHL1(2 5 %) ,GSTP(14 %) ,RAR β(14 %)和DAPK(3 %)。虽然单个肿瘤抑制基因的甲基化可见于良性胆管上皮 ,但是多个肿瘤抑制基因的甲基化只见于胆管癌。约 70 %(5 0 /72 )的胆管癌有 3个或 3个以上的肿瘤抑制基因的甲基化 ,5 2 %(3 8/72 )有 4个或 4个以上肿瘤抑制基因的甲基化。多个肿瘤抑制基因的协同甲基化 ,和RASSF1A ,p15 INK4b.p16INK4a和 /或hMHL1密切相关。RASSF1A的甲基化在肝外胆管癌 (83 %)较肝内胆管癌更常见 (4 7%) (P =0 . 0 0 3 ) ,而GSTP更多见于肝内胆管癌(肝内 3 1%,肝外 6%,P =0 . 0 12 ) ,本研究提示肿瘤抑制基因启动子CpG岛的甲基化在胆管癌中是一?
