Mechanical upper bowel obstruction caused by a large trichobezoar in a young woman: A very unusual case report

BACKGROUND Bezoars usually compile human fibers and debris.A special form of bezoar in case of psychologically altered individuals is the trichobezoar.It consists of voluntarily swallowed hair bulks and is normally removed via gastroscopy.Trichobezoars leading to ileus have rarely been reported.CASE SUMMARY A 24-year-old female patient presented to the emergency room with abdominal pain,nausea,and vomiting for 3 d.Her previous medical and psychiatric history was unremarkable.Laboratory analysis showed iron deficiency anemia,leukocytosis,and elevated liver enzymes.An abdominal CT scan revealed a dense structure in the patients’stomach which turned out to be a huge trichobezoar completely obstructing the pylorus.The trichobezoar had to be removed surgi-cally.During her postoperative course,a subcutaneous seroma formed.After a single puncture,the rest of the recovery process was unremarkable,and the patient recovered fully.CONCLUSION A mechanical bowel obstruction is a potentially life-threatening event for every patient.In our case a young female was suffering from severe symptoms of an obstruction which might have resulted in serious harm without successful surgical management.

Clear cell hepatocellular carcinoma(CCHCC)has hitherto been considered an uncommon, highly differentiated variant of hepatocellular carcinoma(HCC) with a relatively favorable prognosis. CCHCC is composed of mixtures of clear and/or acidophilic ground glass hepatocytes with excessive glycogen and/or fat and shares histology, clinical features and etiology with common HCCs. Studies in animal models of chemical, hormonal and viral hepatocarcinogenesis and observations in patients with chronic liver diseases prone to develop HCC have shown that the majority of HCCs are preceded by, or associated with, focal or diffuse excessive storage of glycogen(glycogenosis) which later may be replaced by fat(lipidosis/steatosis). In ground glass cells, the glycogenosis is accompanied by proliferation of the smooth endoplasmic reticulum, which is closely related to glycogen particles and frequently harbors the hepatitis B surface antigen(HBs Ag).From the findings in animal models a sequence of changes has been established, commencing with preneoplastic glycogenotic liver lesions, often containing ground glass cells, and progressing to glycogen-poor neoplasms via various intermediate stages, including glycogenotic/lipidotic clear cell foci, clear cell hepatocellular adenomas(CCHCA) rich in glycogen and/or fat, and CCHCC. A similar process seems to take place in humans, with clear cells frequently persisting in CCHCC and steatohepatitic HCC, which presumably represent intermediate stages in the development rather than particular variants of HCC. During the progression of the preneoplastic lesions,the clear and ground glass cells transform into cells characteristic of common HCC. The sequential cellular changes are associated with metabolic aberrations, which start with an activation of the insulin signaling cascade resulting in preneoplastic hepatic glycogenosis. The demand for industrial enzymes is continually rising,fueled by the growing need to shift towards more sustainable industrial processes.However,making efficient enzyme production strains and identifying optimal enzyme expression conditions remains a challenge.Moreover,the production of the enzymes themselves comes with unavoidable impacts,e.g.,the need to utilize secondary feedstocks.Here,we take a more holistic view of bioprocess development and report an integrative approach that allows us to rapidly identify improved enzyme expression and secretion conditions and make use of cyanobacterial waste biomass as feed for supporting Pichia pastoris fermentation.We demonstrate these capabilities by producing a phytase secreted by P.pastoris that is grown on cyanobacterium hydrolysate and buffered glycerol-complex(BMGY)medium,with genetic expression conditions identified by high-throughput screening of a randomized se-cretion library.When our best-performing strain is grown in a fed-batch fermentation on BMGY,we reach over 7000 U/mL in three days.

BACKGROUND Population of patients with inflammatory bowel disease(IBD)is burdened by various extraintestinal manifestations which substantially contribute to greater morbidity and mortality.Growth-differentiation factor-15(GDF-15)is often overexpressed under stress conditions,such as inflammation,malignancies,heart failure,myocardial ischemia,and many others.AIM To explore the association between GDF-15 and IBD as serum concentrations of GDF-15 were shown to be an independent predictor of poor outcomes in multiple diseases.An additional aim was to determine possible associations between GDF-15 and multiple clinical,anthropometric and laboratory parameters in patients with IBD.METHODS This cross-sectional study included 90 adult patients diagnosed with IBD,encompassing both Crohn’s disease(CD)and ulcerative colitis(UC),and 67 healthy age-and sex-matched controls.All patients underwent an extensive workup,including colonoscopy with subsequent histopathological analysis.Disease activity was assessed by two independent gastroenterology consultants specialized in IBD,employing well-established clinical and endoscopic scoring systems.GDF-15 serum concentrations were determined following an overnight fasting,using electrochemiluminescence immunoassay.RESULTS In patients with IBD,serum GDF-15 concentrations were significantly higher in comparison to the healthy controls[800(512-1154)pg/mL vs 412(407-424)pg/mL,P<0.001],whereas no difference in GDF-15 was found between patients with CD and UC[807(554-1451)pg/mL vs 790(509-956)pg/mL,P=0.324].Moreover,multiple linear regression analysis showed that GDF-15 levels predict CD and UC severity independent of age,sex,and C-reactive protein levels(P=0.016 and P=0.049,respectively).Finally,an association between GDF-15 and indices of anemia was established.Specifically,negative correlations were found between GDF-15 and serum iron levels(r=-0.248,P=0.021),as well as GDF-15 and hemoglobin(r=-0.351,P=0.021).Accordingly,in comparison to IBD patients with normal hemoglobin levels,GDF-15

Background Anthrax,a zoonotic disease caused by the spore-forming bacterium Bacillus anthracis,remains a major global public health concern,especially in countries with limited resources.Sierra Leone,a West African country historically plagued by anthrax,has almost been out of report on this disease in recent decades.In this study,we described a large-scale anthrax outbreak affecting both animals and humans and attempted to characterize the path-ogenusingmoleculartechniques.Methods The causative agent of the animal outbreak in Port Loko District,Sierra Leone,between March and May 2022 was identified using the nanopore sequencing technique.A nationwide active surveillance was implemented from May 2022 to June 2023 to monitor the occurrence of anthrax-specific symptoms in humans.Suspected cases were subsequently verified using quantitative polymerase chain reaction.Full-genome sequencing was accomplished by combining long-read and short-read sequencing methods.Subsequent phylogenetic analysis was performed based on the full-chromosome single nucleotide polymorphisms.Results The outbreak in Port Loko District,Sierra Leone,led to the death of 233 animals between March 26th and May 16th,2022.We ruled out the initial suspicion of Anaplasma species and successfully identified B.anthracis as the causative agent of the outbreak.As a result of the government's prompt response,out of the 49 suspected human cases identifed during the one-year active surveillance,only 6 human cases tested positive,all within the frst month after the official declaration of the outbreak.The phylogenetic analysis indicated that the BaSL2022 isolate responsible for the outbreak was positioned in the A.Br.153 clade within the TransEuroAsian group of B.anthracis.Conclusions We successfully identifed a large-scale anthrax outbreak in Sierra Leone.The causative isolate of B.anthracis,BaSL2022,phylogenetically bridged other lineages in A.Br.153 clade and neighboring genetic groups,A.Br.144 and A.Br.148,eventually confirming the spillover of anthrax f

目的:研究胰岛素样生长因子1(IGF1)在非小细胞肺癌组织中的表达,探讨其与肺癌临床病理学特征和生存时间的关系。方法:采用免疫组织化学和免疫印迹技术检测IGF1在29例肺鳞癌和22例肺腺癌(25例含癌旁肺组织)及12例良性肺病变组织中的表达。采用χ2检验检测其与临床病理学特征的关系,Log-rank检验IGF1表达与生存时间的关系,Cox模型作单因素和多因素预后分析。结果:IGF1在肺癌中的阳性表达率为51%(26/51),表达率和强度显著高于良性肺组织(P分别为0.0143和0.0039)。晚期肺癌(Ⅲ+Ⅳ)IGF1阳性表达率和强度显著高于早期肺癌(Ⅰ+Ⅱ)(P分别为0.0032和0.0094)。IGF1在伴有局部淋巴结转移肺癌组织中的阳性表达率显著高于无淋巴结转移肺癌组织中的表达(P=0.0074)。IGF1阳性表达患者的中位生存时间显著短于IGF1阴性者(12个月比35个月,P=0.0001),Cox模型分析表明,IGF1表达是非小细胞肺癌的独立预后因素。结论:本研究结果提示,IGF1过表达在非小细胞肺癌的发生和发展中可能起重要作用,检测IGF1表达有望成为判断非小细胞肺癌预后的参考指标之一。

Ten years into the revival of deep networks and artificial intelligence,we propose a theoretical framework that sheds light on understanding deep networks within a bigger picture of intelligence in general.We introduce two fundamental principles,Parsimony and Self-consistency,which address two fundamental questions regarding intelligence:what to learn and how to learn,respectively.We believe the two principles serve as the cornerstone for the emergence of intelligence,artificial or natural.While they have rich classical roots,we argue that they can be stated anew in entirely measurable and computable ways.More specifically,the two principles lead to an effective and efficient computational framework,compressive closed-loop transcription,which unifies and explains the evolution of modern deep networks and most practices of artificial intelligence.While we use mainly visual data modeling as an example,we believe the two principles will unify understanding of broad families of autonomous intelligent systems and provide a framework for understanding the brain.
