Background:Cutaneous squamous cell carcinoma (CSCC) on the auricle is believed to carry a higher risk of metastatic *** rates of lymphatic metastasis reported in the literature have varied *** are no established prognostic criteria to determine which of these tumors are higher risk and warrant prophylactic treatment of the associated lymphatic ***:To retrospectively evaluate outcomes after surgical treatment of auricular CSCC in our department,examining excision completeness,tumor recurrence,and lymphatic ***,to identify factors associated with lymphatic ***:One hundred and thirty‐eight consecutive cases of auricular SCC were excised from 126 patients in our department over a 7‐year period (january 2012–December 2018).Data were retrospectively collected on patient characteristics,tumor histology,surgical procedures,and follow‐***:Incomplete initial excision occurred in 17 cases (12.32%).Six patients (4.76%) had a local *** metastasis occurred in eight patients (6.35%),on average within 10.25 months after primary *** patients with metastasis died during follow‐up,with a mean survival of 10.2 *** age was associated with lymphatic metastasis (P = 0.0267).Other factors,including tumor recurrence,size,grade,cartilage invasion,and positive margins,were evaluated and not significantly associated with ***:In this study,the metastatic rate of auricular SCC was 6.35%,which is within the previously reported *** histological prognostic factors were identified in this study,which may be due to our limited sample *** the absence of established prognostic criteria,decisions regarding prophylactic treatment should be made on an individual basis with multidisciplinary support.

Accurate determination of DNA yield from individual mosquitoes for population genomic applications

Accurate estimates of DNA quantity are likely to become increasingly important for successful genomic screening of insect populations via recently developed, highly multiplexed genotyping assays and high-throughput sequencing methods. Here we show that genomic DNA extractions from single Anopheles gambiae Giles using a standard commercial kit-based methodology yield extracts with concentrations below the linear range of spectrophotometric absorbance at 260 nm. Concentrations determined by spectropho- tometry were not reproducible, and are therefore neither accurate nor reliable. However, DNA quantification using a fluorescent nucleic acid stain （PicoGreen） gave highly reproducible concentration estimates, and indicated that, on average, single mosquitoes yielded approximately 300 ng of DNA. Such a total yield is currently insufficient for many highthroughput genome screening applications, necessitating whole genome amplification of all or most individuals in a population prior to genotyping.

AIM:To investigate all patients referred to our center with non-responsive celiac disease (NrCD),to establish a cause for their continued ***:We assessed all patients referred to our center with non-responsive celiac disease over an 18-mo *** individuals were investigated to establish the eitiology of their continued *** patients were first seen in clinic where a thorough history and examination were performed with routine blood work including tissue transglutaminase antibody *** were also referred to a specialist gastroenterology dietician to try to identift any lapses in the diet and sources of hidden gluten ingestion.A repeat small intestinal biopsy was also performed and compared to biopsies from the referring hospital where ***,lactulose hydrogen breath testing,pancreolauryl testing and computed tomography scan of the abdomen were undertaken if the symptoms *** clinical progress was followed over a minimum of 2 ***:One hundred and twelve consecutive patients were referred with *** were found not to have celiac disease (CD).Of the remaining 100 patients,45% were not adequately adhering to a strict gluten-free diet,with 24 (53%) found to be inadvertently ingesting gluten,and 21 (47%) admitting *** colitis was diagnosed in 12% and small bowel bacterial overgrowth in 9%.refractory CD was diagnosed in 9%.Three of these were diagnosed with intestinal *** 2 years,78 patients remained well,eight had continuing symptoms,and four had ***:In individuals with NrCD,a remediable cause can be found in 90%:with continued gluten ingestion as the leading *** propose an algorithm for investigation.

We report two cases where a proximal humeral locking plate was used for the fixation of an extremely distal, type Ⅲ peri-prosthetic femoral fractures in relation to a total knee replacement(TKr). In each case there was concern regarding the fixation that could be achieved using the available anatomic distal femoral plates due to the size and bone quality of distal fragment. The design of the Proximal Humeral Internal Locking System(PHILOS) allows nine 3.5-mm locking screws to be placed over a small area in multiple directions. This allowed a greater number of fixation points to be achieved in the distal fragment. Clinical and radiological short-term follow-up(6-12 mo) has been satisfactory in both cases with no complications. We suggest the use of this implant for extremely distal femoral fractures arising in relation to the femoral component of a TKr.

Symptomatic neuromas are an all-too-common complication following limb amputation or extremity trauma,leading to chronic and debilitating pain for *** resection of symptomatic neuromas has proven to be the superior method of intervention,but traditional methods of neuroma resection do not address the underlying pathophysiology leading to the formation of a future symptomatic neuroma and lead to high reoperation *** approaches employ the physiology of peripheral nerve injury to harness the regeneration of nerves to their *** review explores the underlying pathophysiology of neuroma formation and centralization of pain *** compares the traditional surgical approach for symptomatic neuroma resection and describes three novel surgical strategies that harness this pathophysiology of neuroma formation to their *** traditional resection of symptomatic neuromas is currently the standard of care for amputation patients,but new techniques including the regenerative peripheral nerve interface,targeted muscle reinnervation,and intraosseous transposition have shown promise in improving patient pain outcomes for postamputation pain and residual limb *** neuromas are a chronic and debilitating complication following amputation procedures and trauma,and the current standard of care does not address the underlying pathophysiology leading to the formation of the *** techniques are under development that may provide improved patient pain outcomes and a higher level of care for symptomatic neuroma resection.

AIM:To investigate the role of psychological characteristics as risk factors for oesophageal adenocarcinoma(OAC),as well as the reflux-mediated precursor ***:An all-Ireland population-based case-control study recruited 230 reflux oesophagitis(rO),224 Barrett's oesophagus(BO) and 227 OAC patients and 260 *** case/control group completed measures of stress,depression,self-efficacy,self-esteem,repression and social support.A comparative analysis was undertaken using polytomous logistic regression adjusted for potential ***:Compared to controls,OAC patients were almost half as likely to report high stress levels over their lifetime(P = 0.010,Or 0.51;95%CI:0.29-0.90)and 36% less likely to report having experienced depression(Or 0.64;95%CI:0.42-0.98).rO patients reported significantly higher stress than controls particularly during middle-and senior-years(P for trends rO patients were 37% less likely to report having been highly emotionally repressed(Or 0.63;95%CI:0.41-0.95).All case groups(OAC,rO and BO) were more likely than controls to report having had substantial amounts of social support(Or 2.84;95%CI:1.63-4.97;Or 1.97;95%CI:1.13-3.44 and Or 1.83;95%CI:1.03-3.24,respectively).CONCLUSION:The improved psychological profile of OAC patients may be explained by response *** role of psychological factors in the development of OAC requires further investigation.
