Clinical significance of surgical resection for hepatocellular carcinoma with portal vein invasion:a nationwide cohort study

Background:Hepatocellular carcinoma(HCC)with portal vein invasion(PVI)is considered an advanced stage with a poor *** current guidelines recommend systemic treatment for HCC with PVI,surgical resection could produce acceptable outcomes in selected *** study aimed to identify the clinical significance of surgical resection for HCC with PVI patients using a large-scale nationwide ***:This retrospective,multicenter,observational cohort analyzed data from the Korean Primary Liver Cancer Registry.A total of 16,781 patients who were newly diagnosed with HCC between 2008 and 2018 were enrolled in this *** with worse Child-Turcotte-Pugh scores(≥7)or performance status(≥2)were *** them,998 patients who received treatment for HCC with PVI were included in the analysis and were divided into two groups:resection group of 151(15.1%)and palliative group of 847(84.9%)who received transarterial and systemic therapy according to the treatment *** matching the number and size of the tumors and model for end-stage liver disease(MELD)score between the groups,the final study cohort for analysis comprised 151(26.6%)patients in the resection group and 417(73.4%)in the palliative *** primary endpoints were overall survival(OS)and cancer-specific survival(CSS).Results:The number and maximum size of HCC did not differ between the resection and palliative groups after matching[1(range,1-5)vs.1(range,1-6),P=0.11 and 5.5(range,1.2-20.6)vs.6.0(range,1.0-20.5)cm,P=0.24,respectively].Tumor markers,including alpha-fetoprotein(AFP)and protein induced by vitamin K absence or antagonist-II(PIVKA-II),also did not differ between the groups(P=0.29 and P=0.36,respectively).The 5-year OS and CSS rates of the resection and palliative groups were 44.8%and 17.4%(P<0.001)and 47.7%and 18.6%(P<0.001),*** analysis showed that palliative treatment intent was the most significant risk factor for OS and CSS[odds ratio(OR

Integrated transcriptome analysis for the hepatic and jejunal mucosa tissues of broiler chickens raised under heat stress conditions

Background:Heat stress(HS)is one of the most important threats for the current poultry ***,many efforts have been made to ameliorate the adverse effect of HS on poultry production;however,physiological and molecular mechanisms pertaining to HS are still limited in ***,the objective of the current study was to investigate functional alterations based on individual and integrated transcriptomes in the liver and jejunal mucosa tissues of broiler chickens exposed to HS ***:Broiler chickens exposed to HS showed decreased growth performance and increased corticosterone concentrations in the *** the transcriptome analysis,the number of differentially expressed genes(DEGs)were identified in the liver and jejunal mucosa by HS *** the liver,genes related to amino acid oxidation,tryptophan metabolism,lipid metabolism,oxidative phosphorylation,and immune responses were altered by HS,which support the reason why heat-stressed poultry had decreased productive *** the jejunal mucosa,genes related to defense systems,glutathione metabolism,detoxification of xenobiotics,and immune responses were differently expressed by HS *** integrated transcriptome analysis with DEGs found in the liver and jejunal mucosa showed a considerable connectivity between the core nodes in the constructed networks,which includes glutathione metabolism,xenobiotic metabolism,carbon metabolism,and several amino acid ***:The core network analysis may indicate that increased requirement of energy and amino acids in the jejunal mucosa of broiler chickens exposed to HS conditions is likely compromised by increased oxidation and synthesis of amino acids in the ***,our results may provide comprehensive insights for molecular and metabolic alterations of broiler chickens raised under HS conditions,which can aid in the development of the novel strategies to ameliorate the negative effect of HS on poultry

Characteristics and Nonlinear Growth of the Singular Vector Related to a Heavy Rainfall Case over the Korean Peninsula

In this study, singular vectors related to a heavy rainfall case over the Korean Peninsula were calculated using the fifth-generation Pennsylvania State University-National Center for Atmospheric Research Mesoscale Model （MM5） adjoint modeling system. Tangent linear and adjoint models include moist physical processes, and a moist basic state and a moist total energy norm were used for the singular-vector calculations. The characteristics and nonlinear growth of the first singular vector were analyzed, focusing on the relationship between the basic state and the singular vector. The horizontal distribution of the initial singular vector was closely related to the baroclinicity index and the moisture availability of the basic state. The temperature-component energy at a lower level was dominant at the initial time, and the kinetic energy at upper levels became dominant at the final time in the energy profile of the singular vector. The nonlinear growth of the singular vector appropriately reflects the temporal variations in the basic state. The moisture-component energy at lower levels was dominant at earlier times, indicating continuous moisture transport in the basic state. There were a large amount of precipitation and corresponding latent heat release after that period because the continuous moisture transport created favorable conditions for both convective and nonconvective precipitation. The vertical propagation of the singular-vector energy was caused by precipitation and the corresponding latent heating in the basic state.

Transient electronics represent an emerging class of technology comprising materials that can vanish in a controlled manner in response to stimuli. In contrast to conventional electronic devices that are designed to operate over the longest possible period, transient electronics are defined by operation typically over a short and well-defined period; when no longer needed, transient electronics undergo self-deconstruction and disappear completely. In this work, we demonstrate the fabrication of thermally triggered transient electronic devices based on a paper substrate, specifically, a nitrocellulose paper. Nitrocellulose paper is frequently used in acts of magic because it consists of highly flammable components that are formed by nitratil^g cellulose by exposure to nitric acid. Therefore, a complete and rapid destruction of electronic devices fabricated on nitrocellulose paper is possible without producing any residue （i.e., ash）. The transience rates can be modified by controlling radio frequency signal-induced voltages that are applied to a silver （Ag） resistive heater, which is stamped on the backside of the nitrocellulose paper. The Ag resistive heater was prepared by a simple, low-cost stamping fabrication, which requires no harsh chemicals or complex thermal treatments. For the electronics on the nitrocellulose paper substrate, we employed semiconducting carbon nanotube （CNT） network channels in the transistor for superior electrical and mechanical properties.

Current therapeutic approaches for volumetric muscle loss(VML)face challenges due to limited graft availability and insufficient *** overcome these limitations,tissue-engineered scaffolds have emerged as a promising *** this study,we developed aligned ternary nanofibrous matrices comprised of poly(lactide-co-ε-caprolactone)integrated with collagen and Ti_(3)C_(2)T_(x)MXene nanoparticles(NPs)(PCM matrices),and explored their myogenic potential for skeletal muscle tissue *** PCM matrices demonstrated favorable physicochemical properties,including structural uniformity,alignment,microporosity,and *** vitro assays revealed that the PCM matrices promoted cellular behaviors and myogenic differentiation of C2C12 ***,in vivo experiments demonstrated enhanced muscle remodeling and recovery in mice treated with PCM matrices following VML *** insights from next-generation sequencing revealed that MXene NPs facilitated protein and ion availability within PCM matrices,leading to elevated intracellular Ca^(2+)levels in myoblasts through the activation of inducible nitric oxide synthase(i NOS)and serum/glucocorticoid regulated kinase 1(SGK1),ultimately promoting myogenic differentiation via the m TOR-AKT ***,upregulated i NOS and increased NO–contributed to myoblast proliferation and fiber fusion,thereby facilitating overall myoblast *** findings underscore the potential of MXene NPs loaded within highly aligned matrices as therapeutic agents to promote skeletal muscle tissue recovery.

Manifestation of attosecond XUV fields temporal structures in attosecond streaking spectrogram

The features of an attosecond extreme ultraviolet （XUV） field are encoded in the attosecond XUV spectrogram. We investigate the effect of the temporal structures of attosecond XUV fields on the attosecond streaking spectrogram. Factors such as the number of attosecond XUV pulses and the temporal chirp of attosecond XUV pulses are considered. Results indicate that unlike the attosecond streaking spectrogram for an attosecond XUV field with two pulses of a half-cycle separation of streaking field, the spectrogram for the attosecond XUV field with three pulses demonstrates fine spectral fringes in separated traces.

Effects of Heterogeneous Condensation on the Transonic Flow Fields on a Bump Model

In the present study, effect of the heterogeneous condensation on the characteristics of shock wave generated on the bump model in the transonic flow field was investigated numerically. As a result, it was found that, for the flow field with steady adiabatic shock wave on the bump model, the condensation with heterogeneous nucleation has a strong effect on the whole flow field, and it reduced the strength of the shock. Furthermore, the total pressure loss was dependent on the concentration of the solid particles per unit volume.

Numerical Analysis of Chevron Nozzle Effects on Performance of the Supersonic Ejector-Diffuser System

The supersonic nozzle is the most important device of an ejector-diffuser *** best operation condition and optimal structure of supersonic nozzle are hardly known due to the complicated turbulent mixing,compressibility effects and even flow unsteadiness which are generated around the nozzle *** the present study,the primary stream nozzle was redesigned using convergent nozzle to activate the shear actions between the primary and secondary streams,by means of longitudinal vortices generated between the Chevron *** same geometrical model of ejector-diffuser system was created to validate the results of experimental *** operation characteristics of the ejector system were compared between Chevron nozzle and conventional convergent nozzle for the primary stream.A CFD method has been applied to simulate the supersonic flows and shock waves inside the *** is observed that the flow structure and shock system were changed and primary numerical analysis results show that the Chevron nozzle achieve a positive effect on the supersonic ejector-diffuser system *** ejector with Chevron nozzle can entrain more secondary stream with less primary stream mass flow rate.

Optimization of post-warranty sequential inspection for second-hand products

This paper considers an optimal sequential inspection schedule for a second-hand product after that the free nonrenewable warranty is expired. The length of warranty is prespecified and during the warranty period, the product is minimally repaired by the dealer when it fails. Following the expiration of the non-renewing warranty, the product is inspected and upgraded sequentially a fixed number of times at the expenses of the *** each inspection, the failure rate of the product is reduced proportionally so that the product is upgraded. The product is assumed to deteriorate as it ages and the replacement of the product occurs when a fixed number of inspections are rendered. In addition,the intervals between two successive inspections are assumed to decrease monotonically. The main objective of this paper is to determine the optimal improvement level to upgrade the product at each inspection so that the expected maintenance cost during the life cycle of the product is minimized from the perspective of the customer. Under the given cost structures, we derive an explicit formula to obtain the expected maintenance cost incurred during the life cycle of the product and discuss the method to find the optimal level of the improvement analytically in case the failure times follow the Weibull distribution. Numerical results are analyzed to observe the impact of relevant parameters on the optimal solution.
