Chungtaejeon, a Korean fermented tea, prevents the risk of atherosclerosis in rats fed a high-fat atherogenic diet

OBJECTIVE： Hypercholesterolemia is one of the well-established risk factors for cardiovascular mortality and morbidity in coronary heart disease. The aim of this study was to investigate the anti-atherogenic effect of Chungtaejeon（CTJ, a Korean fermented tea） aqueous extract on proliferation and migration of human aortic smooth muscle cells（HASMCs） in vivo and in ***： The authors used high-fat atherogenic diet（HFAD） to induce hyperlipidemia in Wistar rats in in vivo animal experiments and used HASMCs for in vitro cell experiments. For the in vitro cell experiment, the proliferation of HASMCs was evaluated using the MTT assay. Similarly, the expression of matrix metalloproteinases（MMPs） in HASMCs was measured using gelatin zymography. Antimigratory activity of CTJ was revealed using the wound-healing model and Boyden＇s chamber assay. In the in vivo experiment, CTJ was administered in three different doses for 20 d from the initiation of the HFAD. After 20 d, the serum lipid profile and total lipid contents in liver were ***： Treatment with CTJ for 24 h dose-dependently inhibited the proliferation and migration of HASMCs and expression of MMP-2 in HASMCs. The oral administration of CTJ at concentrations of 200 and 400 mg/kg decreased the levels of low-density lipoprotein cholesterol, total serum cholesterol and hepatic cholesterol of HFAD-fed ***： CTJ possessed strong antiproliferative, antimigratory, as well as lipid-lowering activities. Thus, CTJ can be considered as a therapeutic option in the treatment of high-fat diet-induced atherosclerosis.

MRI of gastric carcinoma:Results of T and N-staging in an in vitro study

AIM： To determine the accuracy of 1.5-T magnetic resonance imaging （MRI） in the evaluation of gastric wall invasion and perigastric lymph node metastasis in gastric adenocarcinoma.METHODS： Twenty resected gastric specimens containing 20 tumors were studied with a 1.5-T MR system using a commercial head surface coil. MR scanning was performed with a T1 weighted image （TR/TE = 500/20）, and a T2 weighted image （TR/TE = 2500/90）. MR findings were compared with pathologic findings.RESULTS： A T1-weighted image demonstrated three layers in the normal gastric wall. All of the gastric tumors were well demonstrated by lesions and location. In a MRI findings of gastric wall invasion, there was 1 case of T1, 7 of T2, 11 of T3. Pathologic results of resected specimens included 3 cases of pT1, 4 of pT2, and 12 of pT3. The accuracy of T staging with MRI was 74% （14 of 19）. MRI findings of lymph node metastasis included 6 cases of NO, 13 cases of N1. The accuracy of the N staging with MRI was 47% （9 of 19）.CONCLUSION： MRI has a high diagnostic accuracy in the evaluation of the T staging of gastric cancerin vitro and thus potentially enables preoperative histopathologic staging.

Risk Analysis of Breakwater Caisson Under Wave Attack Using Load Surface Approximation

A new load surface based approach to the reliability analysis of caisson-type breakwater is proposed. Uncertainties of the horizontal and vertical wave loads acting on breakwater are considered by using the so-called load surfaces, which can be estimated as functions of wave height, water level, and so on. Then, the first-order reliability method（FORM） can be applied to determine the probability of failure under the wave action. In this way, the reliability analysis of breakwaters with uncertainties both in wave height and in water level is possible. Moreover, the uncertainty in wave breaking can be taken into account by considering a random variable for wave height ratio which relates the significant wave height to the maximum wave height. The proposed approach is applied numerically to the reliability analysis of caisson breakwater under wave attack that may undergo partial or full wave breaking.

Probabilistic Distribution of the Maximum Wave Heigh

A new method of treating maximum wave height as a random variable in reliability analysis of breakwater caissons isproposed. The maximum wave height is expressed as the significant wave height multiplied by the so-called wave height *** proposed wave height ratio is a type of transfer function from the significant wave height to the maximum wave *** the condition of a breaking wave, the ratio is intrinsically nonlinear. Therefore, the probability density function for thevariable cannot be easily defined. In this study, however, it can be derived from the relationship between the maximum andsignificant waves in a nonbreaking environment. Some examples are shown to validate the derived probability density functionfor the wave ratio parameter. By introducing the wave height ratio into reliability analysis of caisson breakwater, the maximumwave height can be used as an independent and primary random variable, which means that the risk of caisson failure during itslifetime can be evaluated realistically.

AIM:To evaluate diagnostic value ofα-fetoprotein (AFP)-L3 and prothrombin induced by vitamin K absence-Ⅱ(PIVKA-Ⅱ)in hepatocellular carcinoma(HCC). Endoscopic management of occluded metal biliary stents:Metal versus 10F plastic stents

AIM:To compare the efficacy of self-expandable metal stents(SEMSs) with 10F plastic stents(PSs) in the endoscopic management of occluded ***:We retrospectively reviewed the medical records of 56 patients who underwent SEMS insertion for palliation of unresectable malignant biliary obstruction between 2000 and 2007 and subsequent endoscopic retrograde biliary drainage(ERBD) with SEMS or PS for initial SEMS occlusion between 2000 and ***:Subsequent ERBD with SEMS was performed in 29 patients and with PS in *** median time to stent occlusion after subsequent ERBD was 186 d in the SEMS group and 101 d in the PS group(P= 0.118).Overall median stent patency was 79 d for the SEMS group and 66 d for the PS group(P = 0.379).The mean number of additional biliary drainage procedures after subsequent ERBD in patients that died(n = 50) during the study period was 2.54 ± 4.12 for the SEMS group and 1.85 ± 1.95 for the PS group(P = 0.457).The mean total cost of additional biliary drainage procedures after the occlusion of subsequent SEMS or PS was $410.04 ± 692.60 for the SEMS group and $630.16 ± 671.63 for the PS group(P = 0.260).Tumor ingrowth as the cause of initial SEMS occlusion was the only factor associated with a shorter time to subsequent stent occlusion(101 d for patients with tumor ingrowth vs 268 d for patients without tumor ingrowth,P = 0.008).CONCLUSION:Subsequent ERBD with PSs offered similar patency and number of additional biliary drainage procedures compared to SEMSs in the management of occluded SEMS.
