Anatomical features associated with venous congestion in DIEP flap using CT angiography with three-dimensional reconstruction

Aim: Computed tomography angiography (CTA) using three-dimensional (3D) virtual reconstruction has been increasingly used in planning deep inferior epigastric artery perforator (DIEP) breast reconstruction. Although the most common complication associated with this surgery is diffuse venous congestion, its origin remains unclear. The aim of this study was to assess the anatomical characteristics of the anterior abdominal wall vessels that could predict venous congestion, using CTA with 3D virtual reconstruction. Methods: A retrospective case-control study was conducted and a total of 169 DIEP flaps were reviewed. An abdominal CTA with 3D virtual reconstruction was analyzed with regard to anatomical features of the abdominal wall vessels. Seven venous congestive cases were identified. For each case, 3 controls that had not exhibited any vascular complications were randomly selected. Results: The global venous congestion rate was 4.14%. No statistically significant differences were found between the groups' superficial inferior epigastric vein (SIEV) diameter (P = 0.915), number of branches of SIEV (P = 0.371), number of perforators per flap (P = 0.255), flap subcutaneous tissue thickness (P = 0.652), direct communications between SIEV-perforators (P = 0.418), and communications of both SIEVs across the abdominal midline (P = 0.371). Conclusion: The present study provided new information concerning the identification of the controversial anatomical features associated with venous congestion in DIEP flaps. CTA and 3D virtual reconstruction were useful tools for evaluating the abdominal wall anatomy and for planning DIEP breast surgery, but neither for predicting nor preventing the diffuse congestive phenomenon.

Role of the combination of biologics and/or small molecules in the treatment of patients with inflammatory bowel disease

Inflammatory bowel disease(IBD)is a group of chronic diseases that includes ulcerative colitis,Crohn’s disease,and indeterminate *** with IBD require prolonged treatment and high utilization of healthcare resources for proper *** treatment of patients with IBD is focused on achieving therapeutic goals including clinical,biochemical,and endoscopic variables that result in improvement of the quality of life and prevention of *** IBD treatment includes tumor necrosis factor inhibitors,integrin antagonist,antagonist of the p40 subunit of interleukin 12/23,and small molecule ***,despite the multiple treatments available,about 40%of patients are refractory to therapy and present with persistent symptoms that have a great impact on their quality of life,with hospitalization and surgery being necessary in many *** therapy,a strategy sometimes applicable to refractory IBD patients,includes the combination of two biologics or a biologic in combination with a small molecule *** are two distinct scenarios in IBD patients in which this approach can be used:(1)Refractory active luminal disease without extraintestinal manifestations;and(2)patients with IBD in remission,but with active extraintestinal manifestations or immune-mediated inflammatory *** review provides a summary of the results(clinical response and remission)of different combinations of advanced drugs in patients with IBD,both in adults and in the pediatric *** addition,the safety profile of different combinations of dual therapy is *** use of newer combinations,including recently approved treatments,the application of new biomarkers and artificial intelligence,and clinical trials to establish effectiveness during long-term followup,are needed to establish new strategies for the use of advanced treatments in patients with refractory IBD.

Convergence of optimal control problems governed by second kind parabolic variational inequalities

We consider a family of optimal control problems where the control variable is given by a boundary condition of Neumann type. This family is governed by parabolic variational inequalities of the second kind. We prove the strong convergence of the optimal control and state systems associated to this family to a similar optimal control problem. This work solves the open problem left by the authors in IFIP TC7 CSMO2011.

A graph-based approach for the structural analysis of road and building layouts

A better understanding of the relationship between the structure and functions of urban and suburban spaces is one of the avenues of research still open for geographical information *** research presented in this paper develops several graph-based metrics whose objective is to characterize some local and global structural properties that reflect the way the overall building layout can be cross-related to the one of the road *** structural properties are modeled as an aggregation of parcels,buildings,and road *** introduce several computational measures(Ratio Minimum Distance,Minimum Ratio Minimum Distance,and Metric Compactness)that respectively evaluate the capability for a given road to be connected with the whole road *** measures reveal emerging sub-network structures and point out differences between less-connective and moreconnective parts of the *** on these local and global properties derived from the topological and graph-based representation,and on building density metrics,this paper proposes an analysis of road and building layouts at different levels of *** metrics developed are applied to a case study in which the derived properties reveal coherent as well as incoherent neighborhoods that illustrate the potential of the approach and the way buildings and roads can be relatively connected in a given urban ***,and by integrating the parcels and buildings layouts,this approach complements other previous and related works that mainly retain the configurational structure of the urban network as well as morphological studies whose focus is generally limited to the analysis of the building layout.

On a Classical Theorem on the Diameter and Minimum Degree of a Graph

In this work, we obtain good upper bounds for the diameter of any graph in terms of its minimum degree and its order, improving a classical theorem due to Erdos, Pach, Pollack and Tuza. We use these bounds in order to study hyperbolic graphs （in the Gromov sense）. To compute the hyperbolicity constant is an almost intractable problem, thus it is natural to try to bound it in terms of some parameters of the graph. Let H（n, δ0） be the set of graphs G with n vertices and minimum degree 50, and J（n, Δ） be the set of graphs G with n vertices and maximum degree A. We study the four following extremal problems on graphs： a（n,δ0） = min{δ（G）｜ G ∈H（n, δ0）}, b（n, δ0） =- max{δ（G）｜ e ∈H（n, δ0）}, α（n, Δ） = min{δ（G） [ G ∈ J（n, Δ）} and β（n,Δ） = max{δ（G） ] G∈Π（n,Δ）}. In particular, we obtain bounds for b（n, δ0） and we compute the precise value of a（n, δ0）, α（n, Δ） and w（n, Δ） for all values of n, r0 and A, respectively.

The architectural modernity that prevailed in Germany during the first half of the twentieth century cannot be understood without an appreciation of the work of Lilly Reich,a brilliant designer of interiors,furniture,and ephemeral architecture who exerted significant influence from her active position as a member of the Deutscher *** the critical recognition she acquired in the media of the time during her solo career,her association with the architect Mies van der Rohe,far from positioning her in the field of contributions,relegated her to the role of a mere *** this reason,this article aims to shed light on the extensive legacy of Lilly Reich,a woman whose figure is being acknowledged as a result of recent research but who was nevertheless omitted from the leading books on the history of *** methodology employed in the research analyses comparatively“what she did”and“what was said about her”,highlighting the convergences and divergences between“work”and“criticism”.The study and its results yield interesting conclusions regarding both the recognition that Lilly Reich received during her lifetime and the criticism she received after her death and up to the present day.

Effect of Implementing Ecosystem Functional Type Data in a Mesoscale Climate Model

In this paper, we introduce a new concept of land-surface state representation for southern South America, which is based on ＂functional＂ attributes of vegetation, and implement a new land-cover （Ecosystem Functional Type, hereafter EFT） dataset in the Weather and Research Forecasting （WRF） model. We found that the EFT data enabled us to deal with functional attributes of vegetation and time-variant features more easily than the default land-cover data in the WRF. In order to explore the usefulness of the EFT data in simulations of surface and atmospheric variables, numerical simulations of the WRF model, using both the US Geological Survey （USGS） and the EFT data, were conducted over the La Plata Basin in South America for the austral spring of 1998 and compared with observations. Results showed that the model simulations were sensitive to the lower boundary conditions and that the use of the EFT data improved the climate simulation of 2-m temperature and precipitation, implying the need for this type of information to be included in numerical climate models.

The Impact of Ecosystem Functional Type Changes on the La Plata Basin Climate

In this paper, the effects of land cover changes on the climate of the La Plata Basin in southern South America are investigated using the Weather and Research Forecasting （WRF） Model configured on a 30/10- km two-way interactive nested grid. To assess the regional climate changes resulting from land surface changes, the standard land cover types are replaced by time-varying Ecosystem Functional Types （EFTs）, which is a newly devised land-cover classification that characterizes the spatial and interannual variability of surface vegetation dynamics. These variations indicate that natural and anthropogenic activities have caused changes in the surface physical parameters of the basin, such as albedo and roughness length, that contributed to regional climate changes. EFTs are obtained from functional attributes of vegetation computed from properties of the Normalized Difference Vegetation Index （NDVI） to represent patches of the land surface with homogeneous energy and gas exchanges with the atmosphere. Four simulations are conducted, each experimental period ranging from September to November in two contrasting years, 1988 and 1998. The influence of an identical EFT change on the surface heat fluxes, 2-m temperature and humidity, 10-m winds, convective instabilities and large-scale moisture fluxes and precipitation are explored for 1988 （a dry year） and 1998 （a wet year）. Results show that the surface and atmospheric climate has a larger response to the same EFT changes in a dry year for 2-m temperature and 10-m wind; the response is larger in a wet year for 2-m water vapor mixing ratio, convective available potential energy, vertically integrated moisture fluxes and surface precipitation. For EFTs with high productivity and a weak seasonal cycle, the near- surface temperature during the spring of 1988 and 1998 increased by as much as I^C in the central and western portions of La Plata Basin.
