Prevention of de novo HBV infection by the presence of anti-HBs in transplanted patients receiving core antibody-positive livers

AIM： To analyze whether the presence of anti-HBs in liver transplant recipients is effective in preventing HBV infection. METHODS： Twenty-three patients receiving anti-HBc positive liver were studied. nine recipients were anti-HBc positive as a result of previous HBV infection. Of them, one also received HBV vaccine during the pre-liver transplantation period. Fourteen recipients were anti-HBs positive due to HBV vaccine administered during the pretransplant period. Liver biopsy was obtained in 10/14 anti-HBc negative/anti-HBs positive recipients and in 4/9 anti-HBc positive recipients. RESULTS： After a mean foUow-up period of 46 months, 1 recipient with protective serum anti-HBs levels developed de novo HBV infection as a consequence of immune escape HBV mutants. Among the 14 vaccinated anti-HBc negative/anti-HBs positive recipients, 1/10 patients with available liver biopsy （10%） had liver HBV-DnA at 13 mo post-liver transplantation without serum viral markers and did not develop de novo HBV *** vaccinated anti-HBc positive recipient without HBV vaccine response was HBV-DnA positive in serum and liver, viral DnA was continuously negative in the following tests, so a spontaneous seroconversion was diagnosed. COnCLUSIOn： The presence of anti-HBs as a result of HBV vaccine or past HBV infection seems to be effective at protecting patients receiving livers from anti-HBc positive donors. However, the emergence of immune escape HBV mutants, which can evade the anti-HBs protection, should be considered as a risk of HBV infection.

Influence of the Fitted Straight Line for Confidence Bands Algorithm in Q-Q Plots

Confidence bands in a normal Q-Q Plot allow us to detect non-normality of a data set rigorously, and in such a way that the conclusion does not depend on the subjectivity of the observer of the graph. In the construction of the graph, it is usual to fit a straight line to the plotted points, which serves both to check the hypothesis of normality (linear configuration of the plotted points) and to produce estimates of the parameters of the distribution. We can opt for dif-ferent types of lines. In this paper, we study the influence of five types of fitted straight lines in a normal Q-Q Plot used for construction the confidence bands based on the exact distribution of the order statistics.

Low-Dose Misoprostol versus Vaginal-Insert Dinoprostone for At-Term Labor Induction: A Prospective Cohort Study

Introduction: Given the pharmacological change taking place in Spanish delivery rooms, we designed this study to compare the effectiveness and safety of misoprostol (25 mcg/4h) to 10 mg dinoprostone in its slow-releasing vaginalinsert form to induce at-term labor. Methods: In a prospective cohort study (2013-2015), 401 at-term pregnant women referred for induction (Bishop score ≤ 6), who gave birth at the Rey Juan Carlos Hospital in Móstoles (Madrid, Spain), of whom 241 participants were induced with misoprostol and 160 with dinoprostone. Patients were recruited by non-probabilistic sampling of consecutive cases as the inductions in the Obstetrics Block during the study period were indicated. Results: The cesarean section rate was 14% for the misoprostol group and 19% for the dinoprostone group [RR = 1.38 (95%CI: 0.89 - 2.13), p = 0.145]. no differences were found in the vaginal birth rate in ng induction [44% vs. 53%, RR = 1.08 (0.90 - 1.29), p = 0.401], nor in the vaginal birth rate in ng oxytocin or meconium fluid, or for maternal-neonatal complications deriving from the method employed. The percentage of cesarean sections due to failed induction was lower in the misoprostol group (15% vs. 39%, p = 0.028). The time between induction and giving birth was shorter for the dinoprostone group (20 h vs. 23.4 h, p = 0.018). Conclusion: Inducing delivery by low-dose misoprostol offers similar effectiveness and safety to induction by dinoprostone in its slow-releasing vaginal-insert form. However, our sample is limited, and few studies are available on the subject. Further analyses to draw solid conclusions are required.

目的:分析普通用户使用电子设备/电脑的工作距离(WD)及移动电话使用距离(MPUD)的特征。方法:横向描述性研究。经过筛选后,纳入每天使用电子设备/电脑超过2h的专业人员100例,年龄20-62(平均41.2)岁,测量每位受试者工作环境中使用电子设备/电脑的工作距离(WD)、手臂长度和MPUD。结果:纳入受检者WD明显长于MPUD(61.22±12.35 vs 32.22±6.35 cm;P<0.001),MPUD明显短于臂长(74.44±4.65 cm;P<0.001)。4%和91%的受检者WD和MPUD分别小于或等于40 cm。WD与MPUD、手臂长度均呈弱相关性(r=0.387,P<0.001;r=0.260,P<0.001)。不同职业受试者WD有显著差异(P=0.021),军队人员相比大学研究人员和讲师的WD更长(P=0.025)。结论:普通用户使用电子设备/电脑的工作距离与移动电话使用距离差别较大,移动电话使用距离较近,这对于矫正老视具有一定的参考意义。

Canine Leptospirosis Serology in Southern Mexico City

Background: Leptospirosis is an important zooanthroponotic disease spread worldwide which infection is recognized as a re-emergent disease. Leptospirosis is a systemic disease of humans and domestic animals, mainly dogs, cattle and swine, characterized by fever, renal and hepatic insufficiency, pulmonary manifestations and reproductive failure. Objective: To study the seroprevalence of anti-Leptospira antibodies in an urban canine population. Materials and Methods: The study was performed in March 2014 in stray dogs that had been captured in the districts of Coyoacan, Iztapalapa, Iztacalco and Benito Juarez in Mexico City;117 dogs were analyzed, from each of which 5 ml of blood were obtained. Serology was performed using a MAT test (Microscopic Agglutination Test), finding that the antigens corresponded to 13 serotypes of Leptospirainterrogans. Three age groups were formed: Group 1) younger than a year (n = 28), Group 2) 1 to 6 years (n = 75), and Group 3) older than 6 years (n = 14). Results: Of the analyzed sera, 28.2% were seropositive to one or more Leptospira serotypes, 74% of the positive seracoagglutinated with two or more serotypes. The most frequent serotypes were: Canicola, Icterohaemorrhagiae and Portland-vere. Serotypes Canicola, Pyrogenes, and Bratislava resulted statistically significant (p 0.001). The presence of anti-Leptospira antibodies in the blood of animals of 1 to 6 years resulted with a p 0.001. The sera from Iztapalapa reacted to nine serotypes. Conclusion: The 28.2% of seropositivity indicates Leptospira transmission in the canine population that was studied could exhibit a potential public health risk.

Determining the specific status of the Iberian sturgeons by means genetic analyses of old specimens

To clarify the species status of sturgeon from rivers of the Iberian Peninsula, eight molecular markers (4 nuclear and 4 mitochondrial) have been analysed in different specimens from historical museum samples and prehistoric samples from archaeological sites. These analyses indicate that one of these specimens (UGP captured in the Guadalquivir River in the 19th century) is A. sturio, based on all the eight molecular markers, four of them used from the first time in this study. In previous analyses based on 5 genetic markers, our group assigned two specimens captured in this river in the 1970-80s (EBD8173 and EBD8401) to the species A. naccarii, suggesting the presence of this species in the Iberian Peninsula. In this work, this conclusion is drawn after successfully obtaining a mitochondrial marker in a very old scute from a prehistoric site (Acinipo, about 1500 BC, from the Guadalquivir River basin). On the other hand, in the specimen EBD8174 captured in the Guadalquivir in 1975, we have obtained two new mitochondrial markers confirming that it can be considered A. sturio for all the mitochondrial markers, but nuclear ones identify it as A. naccarii. Finally, two very old samples (nerja E-VI and nerja n/62-63) were not successfully characterized by any molecular markers. Some aspects and consequences of our results are discussed, such as the origin of the “mosaic” specimen EBD8174 and, above all, the native status of A. naccarii in historic and prehistoric times in the southern Iberian Peninsula.
