Student self-assessment of strengths and needed improvements during a family medicine clerkship

Objective:There are few reports on how students self-assess their performance on a family medicine *** studied what students perceived as their strengths and areas of needed improvement at the mid-point in our family medicine ***:We introduced a form for family medicine clerkship students to self-assess their strengths and areas of needed improvements using the clerkship objectives as a *** cal-culated the frequency in which each clerkship objective was reported as a strength or an area of needed *** students’open-ended comments,two reviewers independently organ-ized students’comments into themes,then negotiated any initial differences into a set of themes that incorporated both the reviewers’*** performed c2 tests to determine any significant differences in the frequency of responses between male and female ***:During the study period(July 2012 to June 2014),372 students submitted com-pleted self-assessment *** most frequently reported strengths were professional objectives(48.9%)and interpersonal communication objectives(43.0%)The most frequently reported areas of needed improvement were the ability to explain key characteristics of commonly used medica-tions(29.3%)and the ability to develop a management plan(28.5%).There were no significant differences in the frequency of responses between male and female ***:We now have a better understanding of students’perceived strengths and areas of needed improvement in our family medicine *** have shared this information with our community faculty preceptors so that they will be better prepared to work with our *** medicine clerkship preceptors at other institutions may also find these results useful.

Barriers and Facilitators in Interorganizational Disaster Response: Identifying Examples Across Europe

Disaster response actors are facing new challenges, which encompass not only new and ever more complex threats but also the need to collaborate across organizational boundaries and even state *** on scale, these interactions have to work across governance setups, political and legal conditions, organizational cultures, as well as personal preferences and experiences that vary among actors, organizations, and countries. But which concrete measures are taken by crisis management actors at different scales to bridge these challenges and which of these could serve others as example to address comparable challenges in their contexts? This study made attempts to analyze whether certain solutions across organizations and states exist that facilitate effective interorganizational crisis management in the member states of the European Union(EU). It is based on selected expert interviews with representatives of different types of disaster response organizations(health services,police services, fire services, and other crisis management organizations) from seven EU member states(Germany,Netherlands, United Kingdom, Ireland, Italy, Austria, and Greece).

Cholangiocarcinoma:A 7-year experience at a single center in Greece

AIM： To evaluate survival rate and clinical outcome of cholangiocarcinoma. METHODS： The medical records of 34 patients with cholangiocarcinoma, seen at a single hospital between the years 1999-2006, were retrospectively reviewed. RESULTS： Thirty-four patients with a median age of 75 years were included. Seventeen （50%） had painless jaundice at presentation. Sixteen （47.1%） were perihilar, 15 （44.1%） extrahepatic and three （8.8%） intrahepatic. Endoscopic retrograde cholangiography （ERCP） and/or magnetic resonance cholangiography （MRCP） were used for the diagnosis. Pathologic confirmation was obtained in seven and positive cytological examination in three. Thirteen patients had co-morbidities （38.2%）. Four cases were managed with complete surgical resection. All the rest of the cases （30） were characterized as non-resectable due to advanced stage of the disease. Palliative biliary drainage was performed in 26/30 （86.6%）. The mean follow-up was 32 mo （95% CI, 20-43 too）. Overall median survival was 8.7 mo （95% CI, 2-16 mo）. The probability of 1-year, 2-year and 3-year survival was 46%, 20% and 7%, respectively. The survival was slightly longer in patients who underwent resection compared to those who did not, but this difference failed to reach statistical significance. Patients who underwent biliary drainage had an advantage in survival compared to those who did not （probability of survival 53% vs 0% at 1 year, respectively, P = 0.038）. CONCLUSION： Patients with cholangiocarcinoma were usually elderly with co-morbidities and/oradvanced disease at presentation. Even though a slight amelioration in survival with palliative biliary drainage was observed, patients had dismal outcome without resection of the tumor.

Biochar Increases Diuron Sorption and Reduces the Potential Contamination of Subsurface Water with Diuron in a Sandy Soil

The herbicide diuron is widely used in agricultural areas in Brazil, whereas it has high potential for subsurface water contamination due to its physicochemical characteristics. Recent studies have demonstrated the potential of biochar as a sorbent and possible pesticide leaching mitigation. The objective of this study was to investigate the long-term effect of biochar application on the kinetics of sorption and desorption of diuron in a Cerrado Haplic Plinthosol. Samples were collected in an experiment conducted in the field in a randomized block design consisting of the combination of two levels of fertilizer application (0 and 300 kg ha −1 of 5-25-15 formula of NPK fertilizers) and three doses of biochar (0, 16, and 32 Mg ha −1 ). The Freundlich isotherm accurately described the sorption of diuron in all treatments. Biochar application increased the sorption and reduced the desorption of diuron. This effect was attributed to the contribution of biochar to total organic carbon (C) and C in the humin fraction and to the increase in the reactivity of the humic acid and humin fractions, which was significantly highly correlated with the sorption coefficient ( K f ). A positive correlation between the partition coefficient of organic C and K f confirmed the importance of the soil organic compartment for the sorption of diuron. The higher diuron sorption and lower diuron desorption capacities of sandy soils after biochar application could reduce the potential risk of diuron leaching and contamination of subsurface water.

Predicting utility of a model for end stage liver disease in alcoholic liver disease

AIM： To validate the statistic utility of both the Maddrey Discriminant Function score and the Model for End-Stage Liver Disease as predictors of short term （30 d and 90 d） mortality in patients with alcoholic hepatitis and to assess prognostic factors among clinical characteristics and laboratory variables of patients with alcoholic hepatitis. METHODS： Thirty-four patients with the diagnosis of alcoholic hepatitis admitted to Hippokration University Hospital of Athens from 2000 to 2005 were assessed in the current retrospective study and a statistical analysis was conducted. RESULTS： 30- and 90-d mortality rates were reported at 5.9% （2/34） and 14.7% （5/34）, respectively. Significant correlation was demonstrated for the Model for End- Stage Liver Disease （P30 = 0.094, P90 = 0.046） and the Maddrey Discriminant Function score （P30 = 0.033, P90 = 0.038） with 30- and 90-d mortality whereas a significant association was also established for alanine aminotrans- ferase （P = 0.057）, fibrin degradation products （P = 0.048） and C-reactive protein （P = 0.067） with 90-d mortality. For 30-d mortality the Area Under the Curve was 0.969 （95%CI： 0.902-1.036, P = 0.028） for the Model for End-Stage Liver Disease score and 0.984 （95%CI： 0.942-1.027, P = 0.023） for the Maddrey Discriminant Function score with the optimal cut off point of 30.5 （sensitivity 1, specificity 0.937） and 108.68 （sensitivity 1, specificity 0.969）, respectively. Accordingly, for 90-d mortality the Area Under the Curve was 0.762 （95%CI： 0.559-0.965, P = 0.065） for the Model for End-Stage Liver Disease score and 0.752 （95%CI： 0.465-1.038, P = 0.076） for the Maddrey Discriminant Function score with the optimal cut off point of 19 （sensitivity 0.6, specificity 0.6） and 92 （sensitivity 0.6, specificity 0.946）, respectively. Spontaneous elimination of hepatitis C virus infection: A retrospective study on demographic, clinical, and serological correlates

To find correlates to spontaneous clearance of hepatitis C virus （HCV） infection, this study compared individuals with self-limited and chronic infection with regard to clinical, demographic, and serological pa- rameters. METHODS： Sixty-seven anti-HCV positive and repeatedly HCV RNA negative individuals were considered to have resolved HCV infection spontaneously. To determine the viral genotype these patients had been infected with HCV serotyping was performed. For comparison reasons, 62 consecutive patients with chronic hepatitis C were enrolled. Cases and controls were compared stratified for age and sex. RESULTS： Retrospective analysis showed （1） a lower humoral reactivity to HCV in patients with self-limited compared to chronic HCV-infection and （2） that younger age, history of iv drug use, and acute/post-acute hepatitis A or B co-infections, but not viral genotypes, are independent correlates for spontaneous HCV clearance. CONCLUSION： The stronger humoral reactivity to HCV in patients with persistent infections and in those with a history of iv drug use is supposed to be due to continuous or repeated contact（s） to the antigen. Metachronous hepatitis A or hepatitis B infections might favor HCV clearance.

Conceptual Framework for Calculation of Climate Performance with Pre-verified LCA-Tools

This article proposes the use of current European standards and verification rules for EPDs （environmental product declarations） as a basis for evaluating the climate impact of contractors＇ alternative designs of infrastructural projects. The proposed conceptual framework for verified climate calculations is based on the Swedish-based, the International EPD System. Several Swedish and Norwegian stakeholders were engaged in this joint project to understand driving forces and obstacles and provide key insights for the successful implementation of the framework for a transparent and comparable assessment of climate impact from alternative design options. In order to perform a sharp test of some main hypothesizes, the project participants tested the proposed framework in bridge and roads projects.
