Air in the main pancreatic duct:A case of innocent air

Air in the main pancreatic duct has been reported only rarely and might be associated with either a spontane- ous or a surgically induced alteration of the anatomy of the biliary tract. We report a case of ＂innocent＂ air found incidentally in the main pancreatic duct. To our knowledge, this is only the third such case reported. A 54-year-old woman presented with hemoptysis that had lasted for 3 d. She underwent a chest computed tomography scan, which revealed not only focal bron- chiectasis in the left lower lobe, but also air in the main pancreatic duct and dilatation of the common bile duct. She was managed conservatively for the hemop- blsis and no further problems developed. She had no specific gastrointestinal symptoms and had no history of surgery or medication. Her laboratory parameters were normal. Magnetic resonance cholangiopancrea- tography also demonstrated air in the main pancreatic duct and a dilated common bile duct （CBD）. Duode- noscopy revealed separate biliary and pancreatic ori-rices with patulous openings and some air bubbles ap- pearing in the pancreatic orifice. Endoscopic retrograde cholangiopancreatography （ERCP） showed the dilated CBD and pancreatic duct with some air bubbles, but no other abnormal lesions. She was discharged with no further problems. Most patients with air in the main pancreatic duct have had a pancreatobiliary disease, or a history of pancreatobiliary disease, pancreatobiliary surgery or sphincterotomy. If the air is innocent, as in our case, ERCP should be performed to evaluate any altered sphincteric function or anatomy such as patu- Ious openings.

Orostachys japonicus ethyl acetate fraction suppresses MRSA biofilm formation

Objective:To investigate the effect of Orostachys(O.)japonicus,a perennial herbaceous plant of the Family Crassulaceae,on biofilm formed by methicillin-resistant Staphylococcus aureus(MRSA).Methods:Powdered *** was extracted by 95%methanol,concentrated,and then,systematically fractionated with n-hexane,dichloromethane(DCM),ethyl acetate(EtOAc),n-butanol,and H2 O according to *** them,the flavonoid-rich EtOAc fraction demonstrated the highest antibacterial activity and was used in this *** the biofilm inhibition assay,cell-surface attachment assay,confocal laser scanning microscopy,latex agglutination assay,and real time qRT-PCR,we examined whether the EtOAc fraction inhibited the formation of MRSA ***:The EtOAc fraction exhibited distinct activity against biofilm formation and cell-surface attachment of MRSA up to 1 mg/m L through down-regulating the expression of mec A gene and the production and agglutination of penicillin-binding protein 2 a as solidly observed in biofilm inhibition assay,cell-suface attachment assay,confocal laser scanning microscopy,latex agglutination assay,and real time qRT-PCR ***:These results suggest that *** could be utilized as a potential resource for the development of new antibiofilm formation of MRSA and antibacterial agents in the future.

BACKGROUND The trigeminocardiac reflex(TCR)is usually caused by an increased parasympathetic tone when pressure or traction is applied to the surrounding tissue of the trigeminal ***,the inexperienced anesthesiologists may have challenges on the management of TCR *** SUMMARY This is the case of an 18-year-old woman diagnosed with hemangioma of the upper *** the operation,about 1 h after surgery started,a constant 1:1 premature ventricular complex was detected,and blood pressure was decreased when approaching the deeper part with more strong traction for exposure of the *** the management of arrhythmias,such as lidocaine and atropine,was injected,arrhythmia induced by surgical stimulation could not be eliminated *** the traction repeated,bradycardia was also repeated,despite injecting additional ***,the anesthesiologist and the surgeon decided to perform the operation only to the extent that the vascular tissue was selectively removed only at the site without the *** With TCR,anesthesiologists should perform appropriate *** addition to proper drug administration,surgeons should be consulted to cope with stopping the surgery and setting the scope of the surgery even if the site is superficial.

Efficient photovoltaic effect in graphene/h-BN/silicon heterostructure self-powered photodetector

Graphene(Gr)/Si-based optoelectronic devices have attracted a lot of academic attention due to the simpler fabrication processes,low costs,and higher performance of their two-dimensional(2D)/three-dimensional(3D)hybrid interfaces in Schottky junction that promotes electron-hole ***,due to the built-in potential of Gr/Si as a photodetector,the Iph/Idark ratio is often hindered near zero-bias at relatively low illumination *** is a major drawback in self-powered *** this study,we have demonstrated a self-powered van der Waals heterostructure photodetector in the visible range using a Gr/hexagonal boron nitride(h-BN)/Si structure and clarified that the thin h-BN insertion can engineer asymmetric carrier transport and avoid interlayer coupling at the *** dark current was able to be suppressed by inserting an h-BN insulator layer,while maintaining the photocurrent with minimal decrease at near *** a result,the normalized photocurrent-to-dark ratio(NPDR)is improved more than 104 ***,both Iph/Idark ratio and detectivity,increase by more than 104 times at−0.03 V drain *** proposed Gr/h-BN/Si heterostructure is able to contribute to the introduction of next-generation photodetectors and photovoltaic devices based on graphene or silicon.

Comparative outcomes of the pathogen in cultured Jones tubes used in lacrimal bypass surgery according to follow up periods

AIM:To evaluate the pathogens in cultured Jones tubes used in lacrimal bypass surgery according to the postoperative periods and to obtain data for the prevention of infection of functional lacrimal stent ***:Totally 71 patients(81 eyes)who underwent the removal of Jones tubes were enrolled in *** the removed Jones tubes were cultured for bacterial and fungal identification and tested for bacterial antibiotic *** results were analyzed according to the duration of the inserted Jones tube after lacrimal bypass ***:Of the 81 eyes,bacteria were isolated from 69 eyes(85.2%)and fungi from 6 eyes(7.4%).Among 69 eyes,40.6% showed Staphylococcus aureus(***),11.6% were Pseudomonas aeruginosa(***).Gram-positive bacteria were isolated more than Gramnegative bacteria,but Gram-negative bacteria showed a higher incidence in the Jones tube implanted for over 10y(P=0.035).The antibiotic sensitivity test showed that 46.4% of *** were resistant to *** terms of antibiotics commonly used in ocular clinical practice,vancomycin was sensitive to *** and Streptococcus pneumoniae(***),amikacin responded to *** and Proteus mirabilis(***).Trimethoprim/sulfamethoxazole(TMP/SMX)was all sensitive to ***,*** and *** except ***:*** is the most commonly found organism in the Jones tube after lacrimal bypass surgery,and 46.4% of them are methicillin-resistant ***(MRSA),sensitive to ***,*** responded with amikacin is dominantly detected in the Jones tubes implanted for more than 10y.

Evaluation of Pre-Emergence and Post-Emergence Herbicides for Weed Management in Miscanthus sacchariflorus and Miscanthus sinensis

Miscanthus,is a promising bioenergy crop,considered superior to other bioenergy crops because of its higher water and nutrient use efficiency,cold tolerance,and higher production of *** weeds and grass weeds,cause major problems in the Miscanthus field.A field experiment was conducted in 2018 and 2019,to assess the effects of pre-emergence(alachlor and napropamide)and post-emergence herbicides(nicosulfuron,dicamba,bentazon,and glufosinate ammonium)on broadleaf and grass weeds in *** and *** *** weed control efficiency and phytotoxicity of pre-and post-emergence herbicides were evaluated at 30 days after treatment(DAT)and compared to those of the control *** results showed wide variations in the susceptibility of the weed species to the treated *** with nicosulfuron 40 ***^(-1) provided the most effective overall weed control(with 10%visual injury),without affecting the height and biomass of neither Miscanthus species in the ***-emergence herbicides such as glufosinate ammonium 400 ***^(-1) and dicamba 482 ***^(-1) were effective and inhibited the growth and density of the majority of weeds to a 100%;however,they showed significant phytotoxicity(toxicity scale of 1-10)to both species of *** application of glufosinate ammonium caused severe injuries to the foliar region(90%visual injury)of both Miscanthus ***,*** showed a slightly higher tolerance to the herbicides nicosulfuron,bentazon and napropamide with 10%visual injury at the recommended dose than *** present study clearly showed that infestation of broadleaf and grass weeds in Miscanthus fields can cause significant damage to the growth and biomass of Miscanthus and applying pre-emergence and post-emergence herbicides effectively controls the high infestation of these weeds.

Clinical characteristics of asymptomatic Terson syndrome in the patients with aneurysmal subarachnoid hemorrhage

●AIM:To investigate clinical characteristics of asymptomatic Terson syndrome and its clinical impact in patients with aneurysmal subarachnoid hemorrhage(SAH).●METHODS:This retrospective,interventional study included 31 patients with aneurysmal SAH,and the medical records were *** addition to baseline characteristics of the study population such as age,sex,and underlying medical history,multi-modal imaging analysis,including fluorescein angiography(FA),spectral domain optical coherence tomography(SD-OCT),were also *** Coma Scale(GCS),Hunt-Hess(HH)grade,and Fisher scale at the time of admission,and functional outcome by using modified Rankin Scale(mRS)at 6 mo were compared.●RESULTS:Of the 31 patients,10 patients(32.3%)were diagnosed with Terson *** the patients with Terson syndrome did not report visual symptoms at the time of ophthalmologic *** showed microvascular changes of retinal capillaries and varying degrees of disc ***-OCT allowed intuitive anatomical localization of multi-layered retinal hemorrhages and assessment of ellipsoid zone *** patients with Terson syndrome showed significantly worse GCS(P=0.047)and HH grade(P=0.025)than those without,except Ficher scale(P=0.385).There was no significant difference in the mRS(P=0.250)at 6 *** baseline factors,the HH grade was the only significant factor associated with Terson syndrome(B=1.079,P=0.016).●CONCLUSION:In our study,32.3%of the patients have Terson syndrome without visual *** baseline HH grade is significantly correlated with Terson syndrome,and there is no significant difference in the functional outcome between the patients with and without Terson *** syndrome may develop without any visual symptoms as shown in our study,and ophthalmologic screening may be recommended to prevent further visual deterioration especially in the patients with poor HH grade at the time of aneurysmal SAH.

Comparative studies on the binding site of anesthetics to GABA a receptors using in silico docking methods

Although the GABAA receptor(GABAAR)has been proposed as the main action site for sevoflurane,isoflurane,halothane,enflurane,propofol,and benzodiazepines(BZDs),binding of these anesthetics with high-resolution structures of the GABAAR have been rarely examined by comparative docking ***,various combinations of ligands on more GABAARs with various subtypes need to be analyzed to understand the elaborate action mechanism of GABAARs better because some GABAA ligands showed specificity toward the distinct subtypes of the ***:We performed in silico docking analysis to compare the binding modes of sevoflurane,isoflurane,halothane,enflurane,propofol,and BZDs to the GABAAR based on one of the most recently provided 3D *** performed the docking analysis and the affinity-based ranking of the binding ***:Our docking studies revealed that isoflurane,halothane,and enflurane docked in an extracellular domain(ECD)on GABAARs,in contrast to ***:Our results supported a multi-site mechanism for the allosteric modulation of *** was bound to the pore or favored various subsites in the transmembrane domain(TMD).Our result confirmed that different chemically related BZD ligands interact via distinct binding modes rather than by using a common binding mode,as previously suggested.

The prevalence of nonalcoholic fatty liver disease(NAFLD)and its associated metabolic risk factors,including obesityand metabolic syndrome,have increased over the lastdecade in both developed and developing countries(1).NAFLD is considered to be the hepatic manifestation ofmetabolic syndrome and insulin resistance which is oneof multiple hits leading to NAFLD(2).
