Partial Nephrectomy as Treatment of an Atypical Metastasis from Prostate Cancer—A Case Report and Review of Literature

Background: Prostate cancer is the second most common type of cancer in man and the second in cancer-specific deaths in this population in the world. Most of the causes of death related to prostate cancer are due to its distant metastases, with the most common sites being: skeleton, distant lymph nodes, liver and lung. Renal metastasis is rare, and studies suggest infiltration due to arterial microembolization of the tumor. A key point in this scenario is the clinical suspicion of differential diagnoses, to offer the patient an effective therapy in such a specific case. Aim: To report a case of a patient with prostate cancer undergoing partial nephrectomy whose histopathological report revealed a metastatic lesion of that primary site. Case Presentation: 74 years old man, referred in May 2015 due to high PsA level and lumbago. PsA 323.11 ng/dl, rectal examination cT3a;biopsy was performed and histopathological study reported bilateral prostate adenocarcinoma, Gleason’s score 8 (4 + 4). Patient’s staging showed multiple secondary implants on skeletal scintigraphy. Tomography revealed solid exophytic lesion in the lower pole of the right kidney (4.7 × 3.6 cm);prostate without cleavage planes with seminal vesicles and pelvic node enlargement. Hormone therapy was initiated, PsA levels dropped to 9.51 ng/dl and total testosterone Conclusion: Given the rarity of these cases, it is not possible to presume that nephrectomy enhances the survival rates. However, we observed that partial nephrectomy was a good choice for our patient, being the first case described in the literature. More reports should be available and studies with higher levels of evidence should be conducted to assist us in patient orientation and decision making.

BACKGROUND Endoscopic drainage remains the treatment of choice for unresectable or inoperable malignant distal biliary obstruction(MDBO).AIM To compare the safety and efficacy of plastic stent(Ps)vs self-expanding metal stent(sEMs)placement for treatment of *** This meta-analysis was developed according to Preferred Reporting Items for systematic Reviews and Meta-Analysis guidelines.A comprehensive search was performed in MEDLINE,Cochrane,Embase,Latin American and Caribbean Health sciences Literature,and grey literature to identify randomized clinical trials(RCTs)comparing clinical success,adverse events,stent dysfunction rate,reintervention rate,duration of stent patency,and mean *** difference(RD)and mean difference(MD)were calculated and heterogeneity was assessed with I2 *** analyses were performed by sEMs *** Twelve RCTs were included in this study,totaling 1005 *** was no difference in clinical success(RD=-0.03,95%confidence interval[CI]:-0.01,0.07;I 2=0%),rate of adverse events(RD=-0.03,95%CI:-0.10,0.03;I2=57%),and mean patient survival(MD=-0.63,95%CI:-18.07,19.33;I2=54%)between sEMs vs Ps ***,sEMs placement was associated with a lower rate of reintervention(RD=-0.34,95%CI:-0.46,-0.22;I2=57%)and longer duration of stent patency(MD=125.77 d,95%CI:77.5,174.01).subgroup analyses revealed both covered and uncovered sEMs improved stent patency compared to Ps(RD=152.25,95%CI:37.42,267.07;I2=98%and RD=101.5,95%CI:38.91,164.09;I2=98%;respectively).stent dysfunction was higher in the covered sEMs group(RD=-0.21,95%CI:-0.32,-0.1;I²=205%),with no difference in the uncovered sEMs group(RD=-0.08,95%CI:-0.56,0.39;I²=87%).CONCLUsION While both stent types possessed a similar clinical success rate,complication rate,and patient-associated mean survival for treatment of MDBO,sEMs were associated with a longer duration of stent patency compared to Ps.

Byrsonima intermedia A Juss. is a species with pharmacological properties from the Brazilian Cerrado that shows difficulties related to sexual propagation. Research on cell viability may provide useful information for the selection of cells with embryogenic potential during the callus culture. Within this context, our research is aimed at establishing the cell viability of calli from Byrsonima intermedia leaf segments. The calli went through three subculture phases, of 60 days each, in Ms medium with 0.09 M sucrose, 0.6% agar, pH 5.8 and 4.52 μM 2,4-D. The calli were stored in dark conditions and samples were collected every 10 days from each subculture for viability tests with fluorescein 3,6-diacetate (FDA) and 2,3,5-triphenyltetrazolium chloride (TTC). The staining methods allowed quantifying cell viability in each subculture. The best results from the FDA tests were obtained at 21, 25 and 29 days for the first, second and third subcultures respectively, with 53.86%, 61.88% and 53.73% viable cells. Regarding the TTC test, the largest absorbance values were obtained at 21, 27 and 28 days for the first, second and third subcultures respectively. Fluorescence and spectrophotometry analyses were efficient for determination of cell viability during callus cultivation period.

Determining the levels of heavy metals in honey is a measure to control its quality. The objective of this study was to determine the contents of Cd, Cu, Pb and Zn in honey of stingless bee. The differential pulse anodic stripping voltammetry (DPAsV) was used to determine the trace elements in honey samples. Pb was detected in 100% of the samples, Cu and Zn in 98.15%, and Cd only in 33.33%. The trace elements analyzed detected in the samples remained within tolerable levels in foods for human consumption.
