Background:Neonatal sepsis is a leading cause of neonatal deaths in developing *** current recommended in-hospital treatment is parenteral ampicillin(or penicillin)and gentamicin in young infants for 10-14 days;however,very few could access and *** current review is to evaluate the feasibility of gentamicin in community based ***:Both observational and randomized controlled trials were ***,Embase,Cochrane Central Register of Controlled Trials and Central Trial Register of India were searched until September *** assessed the risk of bias by Cochrane Collaboration’s"risk of bias"***:Two observational studies indicated feasibility ensuring coverage of population,decrease in case fatality rate in the group treated by community health *** an RCT,no significant difference was observed in the treatment failure rates[odds ratio(OR)=0.88],and the mortality in the first and second week(OR=1.53;OR=2.24)between gentamicin and ceftriaxone *** the gentamicin group,the combination of penicillin and gentamicin showed a lower rate of treatment failure(OR=0.44)and mortality at second week of life(OR=0.17)as compared to the combination of gentamicin and oral ***:Gentamicin for the treatment of neonatal sepsis is both feasible and effective in community-based settings and can be used as an alternative to the hospitalbased care in resource compromised *** there was less evidence in the management of neonatal sepsis in hospitals as was seen in this review in which we included only one RCT and three observational studies.

Purpose: To evaluate diagnostic accuracy of IS6110 insertion genes, hsp65, and Xpert MTB/RIF for rapid diagnosis of pulmonary tuberculosis. Methods: Sixty patients, medically reported HIV negative, clinically suspected of having pulmonary tuberculosis, were included in this study, and consented before enrolment. Sputum samples were gathered once, and tested by smear for Acid Fast Bacilli (AFB). Cultured in the Loewenstein-Jensen (LJ) medium for M. tuberculosis growth, M. tuberculosis DNA was detected by conventional PCR targeting IS6110, and hsp65 genes using specific primers, and automated nested real-time PCR targeting rpoB gene. Sensitivity, specificity and diagnostic accuracy were calculated for each method compared to culture. Results: Compared with culture as reference method, smear, IS6110, hsp65, and Xpert MTB/RIF had sensitivity 77.14%, 100%, 100%, and 100%, specificity 92%, 96%, 96%, and 96.97%, and diagnostic accuracy 83.33%, 98.33%, 98.33% and 98.21% respectively. Molecular diagnostic methods had the highest diagnostic accuracy, whereas smear had the lowest. No statistical significance, (p value > 0.05) was detected between the patients’ demographic data and the presence or absence of TB infection. Conclusion: The diagnostic accuracy that we got from the molecular methods, confirmed the diagnostic value of molecular detection of M. tuberculosis in pulmonary cases, supporting the application of automated and conventional PCR in rapid analysis. Smear could be more efficient when used for treatment monitoring. Combination between one-molecular techniques with smear as a routine method could be valid for rapid diagnosis of TB.

Neurofibroma arising from intrathoracic vagus nerve is a rare entity. We present a 6-year-old child with stigmata of neurofibromatosis along with a lump in the back. She also had history of on and off cough. On investigation there was a large mediastinal mass abutting the trachea. She underwent a posterolateral thoracotomy with partial resection of the lump. Intraoperatively the mass appeared to arise from the vagus nerve. Post operative stay was uneventful. Biopsy of the mass revealed a benign neurofibroma with no evidence of invasion. She is presently asymptomatic after 1 year of follow-up.

Chronic kidney disease （CKD） patients have high cardiovascular mortality and morbidity. The presence of traditional and CKD related risk factors results in exaggerated vascular calcification in these patients. Vascular calcification is associated with reduced large arterial compliance and thus impaired barorefex sensi-tivity （BRS） resulting in augmented blood pressure （BP） variability and hampered BP regulation. Barorefex plays a vital role in short term regulation of BP. This review discusses the normal barorefex physiology, methods to assess baroreflex function, its determinants along with the prognostic significance of assessing BRS in CKD patients, available literature on BRS in CKD patients and the probable patho-physiology of barorefex dysfunction in CKD.

Hernia is a common problem of the modern world with its incidence more in developing countries. Inguinal hernia is the most common groin hernia repaired worldwide. With advancement in technology operative techniques of repair have also evolved. A Pub Med and COCHRANE database search was accomplished in this regard to establish the current status of laparoscopic inguinal hernia repair in view of recent published literature. Published literature support that laparoscopic hernia repair is best suited for recurrent and bilateral inguinal hernia although it may be offered for primary inguinal hernia if expertise is available.

Many existing real time commit protocols try to improve system performance by allowing a committing cohort to lend its data to an executing cohort, thus reducing data inaccessibility. They block the borrower from sending WORKDONE/PREPARED message and restrict them from lending data so that transaction abort chain is limited to one. Thus, transaction execution time increases. This paper proposes a modified real time commit protocol for distributed real time database systems (DRTDBS), Allow Commit Dependent and in Time borrowers for Incredible Value added data lending without extended abort chain (ACTIVE), where borrower cohorts are categorized as commit and abort dependent. Further, the commit dependent borrowers can lend data to executing cohorts with still limiting the transaction abort chain to one only and reducing the data inaccessibility. Also, an incoming executing cohort having borrowing factor greater than one can only borrow the dirty data items from lender. This minimizes the fruitless borrowing by the cohort. The performance of ACTIVE is compared with PROMPT, 2SC and SWIFT protocols for both main memory resident and disk resident databases with and without communication delay. Simulation results show that the proposed protocol improves the system performance up to 4% as transaction miss percentage.

Disease and sufferings lead religious persons nearer to almighty in search of mercy. Not much is known about how to design and disseminate programs of spiritual exercises and spiritual indicators for persons suffering with some disease (sufferers). Research is required to advance the understanding of prioritized internal and external indicators responsible for spiritual awakening among these sufferers. In this paper, an attempt is made to compute OFDP (overall futuristic decision priority) weights to generate spiritual scenarios for spiritual awakening by using MCFDM (multi-criteria futuristic decision-making) methodology. The generated prioritized spiritual scenarios responsible for spiritual awakening among sufferers were then used to assess the effect of some kind of disease (physical problem, suffering, disorder, etc.) on strengthening their religious faith and defining their optimal spiritual trajectories towards ultimate reality. In this research work, a two-phased DRFI (disease-religious faith-interaction) experiment is also designed and developed. The results derived from Phase-I pilot research study based on prioritized spiritual scenarios on 20 male and female sufferers are also presented.
