BACKGROUND:Many Chinese herbs,especially herbal injections,have been shown to have anti-tumor effects in recent ***,since most reports focus on the clinical effectiveness of these herbs,their mechanisms of action are not well *** this study,we assessed apoptosis in the hepatocellular carcinoma (HCC) cell line HepG2 induced by an injectable extract from the seed of Coix lacryma-jobi (Semen coicis,SC),and monitored the expression of Bcl-2 and ***:Injectable SC was applied to HepG2 cells at different concentrations and the cells were collected 12,24 and 48 hours later.5-fluorouracil was used as a positive control group,and fluorescence-activated cell-sorting cytometry was used to measure the apoptosis rate of HepG2 cells and the expression of Bcl-2 and caspase-8 ***:SC induced apoptosis in HepG2 cells in a concentration and time-dependent manner,and the expression of caspase-8 was elevated and ***,it did not significantly influence the expression of ***:Injectable SC may induce apoptosis in HCC cells by regulating the expression of caspase-8.

Transoesophageal emergency ventricular pacing with esophageal balloon electrode catheter

Objective To evaluate the safety and possibility of transesophageal emergency ventricular pacing with esophageal balloonelectrode *** Sixty four patients with critical bradyarrhythmias were divided randomly to receive either transesophagealventricular pacing with esophageal balloon electrode catheter(*** group,n=32),or transesophageai ventricular pacing withnormal esophageal electrode catheter(normal catheter group,n=32).Pulse width(ms),current output(mA),and the success rate werecompared between the 2 *** Success rate in the balloon catheter group was 87.5%,which was significantly higher thanthat in the nodal catheter group(18.7%,X2=27.7,P＜0.01).The success rate oftransesophageal ventricular pacing with esophagealballoon electrode catheter was 87.5%.The average current output(mA)was significantly lower in the balloon catheter group than thatin the normal catheter group (24.5+2.5 mA VS 53±3.4mA,P＜0.05).Conclusions Esophageal balloon electrode catheter pacing couldbecome an available teclmiqBe for transesophageal emergency ventricular pacing.

Relationship between expressions of E-cadherin and alpha-catenin and biological behaviors of human pancreatic cancer

OBJECTIVES: To investigate the expressions of E-eadherin and alpha-catenin in pancreatic carcinoma and their relationship with biological behaviors, and clarify the mechanism of invasion and metastasis of pancreatic cancer. METHODS: The expressions of E-eadherin and alpha-catenin was examined in 47 patients with infiltrative ductal adenocarcinoma of the pancreas and 12 specimens of normal pancreatic tissues by immunohistochemical technique (PicTure^(TM) two-step method). Proliferation cell nuclear antigen (PCNA) was tested as an index of the proliferation degree of pancreatic cancer cells. RESULTS: The immunoreactivity of E-cadherin and alpha-catenin was expressed by normal ductal and acinar cells with strong membranous staining at the intercellular border in 12 specimens of normal pancreatic tissues. The abnormal rate of E-cadherin expression in pancreatic cancer was 53.2% (25/47), and it was significantly related to differentiation, high proliferation degree and lymph node and liver metastases (P0.05, all). BACKGROUND: Spinal cord injury (SCI) is found to be related to increased prevalence of gallstones and acute acal- culous cholecystitis. In this study we assessed the prevalence of cholelithiasis in male patients with SCI and the correla- tion of cholelithiasis with age and weight of patients, level of injury, as well as severity and duration of SCI. METHODS: One hundred male SCI patients (58 patients rated ASIA A or B and 42 rated ASIA C or D) aged more than 20 years (average 46.5 years) suffered from a spinal cord injury for more than one year. One hundred male vo- lunteers served as controls without SCI and biliary diseases (age range 20-68 years; average 42.6 years). The two groups were subjected to ultrasonography of the gallbladder and biliary tract. RESULTS: The prevalence of cholelithiasis in the group of SCI patients and the control group was 26% and 10% re- spectively. Significant differences in the prevalence of cho- lelithiasis were found between the normal controls and SCI patients and between high- and low-level injury (P 0.05). CONCLUSIONS: SCI represents a major risk factor for the development of cholelithiasis, especially in patients with high-level injury. Cholelithiasis in SCI patients is not relat- ed to their age and weight, the severity of spinal lesion, and the duration of spinal cord injury.

Hepatic blood inflow occlusion without hemihepatic artery control in treatment of hepatocellular carcinoma

AIM:To investigate the clinical significance of hepatic blood inflow occlusion without hemihepatic artery control (biowHAC) in the treatment of hepatocellular carcinoma (HCC).METHODS:Fifty-nine patients with HCC were divided into 3 groups based on the technique used for achieving hepatic vascular occlusion:group 1,vascular occlusion was achieved by the Pringle maneuver (n=20);group 2,by hemihepatic vascular occlusion (HVO) (n=20);and group 3,by biowHAC (n=19).We compared the procedures among the three groups in term of operation time,intraoperative bleeding,postoperative liver function,postoperative complications,and length of hospital ***:There were no statistically significant differences (P > 0.05) in age,sex,pathological diagnosis,preoperative Child's disease grade,hepatic function,and tumor size among the three *** intraoperative complications or deaths occurrred,and there were no significant intergroup differences (P > 0.05) in intraoperative bleeding,hepatic function change 3 and 7 d after operation,the incidence of complications,and length of hospital *** and Pringle maneuver required a significantly shorter operation time than HVO;the difference in the serum alanine aminotransferase or aspartate aminotransferase levels before and 1 d after operation was more significant in the biowHAC and HVO groups than in the Pringle maneuver group (P biowHAC is convenient and safe;this technique causes slight hepatic ischemia-reperfusion injury similar to HVO.

Pre-ligation of afflicted hepatic inflow and outflow blood vessels of lesioned liver lobe during hepatectomy

OBJECTIVE: To compare the merits of hepatectomy after pre-ligation of the hepatic inflow and outflow blood vessels of the lesioned liver lobe with those of Pringle's maneuver. METHODS: A total of 68 patients were divided into two groups A and B. In the group A (n=38), Pringle's maneuver was employed, whereas in the group B (n=30), hepatectomy after pre-ligation of the hepatic inflow and outflow blood vessels of the lesioned side of the liver was used. Perioperative blood loss, postoperative bleeding and drainage, time of liver function recovery as well as incidence of postoperative complications were compared between the 2 groups. RESULTS: The mean perioperative blood loss, the mean amount of postoperative bleeding and drainage, the time of liver function recovery as well as incidence of postoperative complications were significantly higher in the group A than in the group B (P<0.01). CONCLUSION: Hepatectomy after pre-ligation of the hepatic inflow and outflow blood vessels of the lesioned side of the liver is superior to Pringle's maneuver.

An analysis of 680 cases of cholangiocarcinoma from 8 hospitals

BACKGROUND: The outcome of patients with cholangio-carcinoma is poor. To evaluate the experience in the diagnosis and surgical treatment of cholangiocarcinoma, we investigated the status quo of diagnosis and treatment of cholangiocarcinoma in china. METHOD: The clinical data of 680 patients with cholangiocarcinoma treated at 8 hospitals from 1995 to 2001 were retrospectively analyzed with SPSS software package. RESULTS: The incidence of the tumor was the highest in the age group of 60-65 years. Meanwhile,the incidence was higher in aged men than in aged women, with a male to female ratio of 1.39:1. Proximal cholangiocarcinoma was the commonest ( 41.6% ) and distant cholangiocarcinoma the second(28.7% ) in the 680 patients. B-mode ultrasonogra-phy for cholangiocarcinoma was performed in 80.3% of the patients. Non-traumatic examinations such as computed tomography (CT), magnetic resonance image (MRI) and magnetic resonance cholangiopancreatography ( MRCP ) were more widely used than that of traumatic examinations such as percutaneous transhepatic cholangiography (PTC) and endoscopic retrograde cholangiopancreatography ( ER-CP). The low- and middle-differentiation cancer of the proximal bile duct accounted for about 50%. Most of the patients suffered from late-stage cholangiocarcinoma. The resection rate of the tumor was low, and the rate of radical operation was only 21.6% (147/680). CONCLUSIONS: Cholangiocarcinoma is common in the aged men. Its diagnosis and treatment have been improved, but little. Most patients are diagnosed as having late-stage cholangiocarcinoma at the time of outpatient clinic, and the rate of radical operation is low. Thereforce, it is necessary to reinforce the early diagnosis and treatment of cholangiocarcinoma to improve the outcome after operation.

Abnormalexpression of E-cadherin and alpha-, beta-catenin in 47pancreatic carcinoma tissues was demonstrated in 53.2 %,61.7 % and 68.1%, respectively. Both abnormal expressionof E-cadherin and alpha-catenin significantly correlated withdifferentiation, lymph node and liver metastases (P＜0.05,respectively), whereas aberrant beta-catenin expression onlycorrelated with lymph node and liver metastases (P＜0.001).Abnormal E-cadherin and alpha-, beta-catenin expressionwas not associated with tumor size, invasion and survivaltime of patients (P＞0.05, all).CONCLUSION: Pancreatic cancer likely occurs in case ofE-cadherin-catenin complex genes mutations or deletionsand abnormal expression of proteins, which significantlycorrelate with the biologic character of the tumor and lymphnode and liver metastases. It is suggested that the abnormalE-cadherin-catenin complex expression plays an importantrole in the development and progression of tumor, and thusmay become a new marker in pancreatic cancer metastasis.

The visual spatial attention mechanism in the brain was studied in 16 young subjects through the visual search paradigm of precue-target by the event-related potential (ERP) technique, with the attentive ranges cued by different scales of Chinese character and region cues. The results showed that the response time for Chinese character cues was much longer than that for region cues especially for small region cues. With the exterior interferences, the target stimuli recognition under region cues was much quicker than that under Chinese character cues. Compared with that under region cues, targets under Chinese character cues could lead to increase of the posterior P1, decrease of the N1 and increase of the P2. It should also be noted that the differences between region cues and Chinese character cues were affected by the interference types. Under exterior interferences, no significant difference was found between region cues and Chinese character cues; however, it was not the case under the interior interferences. Considering the difference between the exterior interferences and the interior interferences, we could conclude that with the increase of difficulty in target recognition there was obvious difference in the consumption of anterior frontal resources by target stimuli under the two kinds of cues.
