Larynx Cancer: Review of 11 Years of Activity in the ENT and Head and Neck Surgery Department of University Hospital Gabriel Toure, Bamako

Objectives: To study the epidemio-clinical, therapeutic and evolutionary aspects of laryngeal cancers in the ENT department of the University Hospital Gabriel Toure. Materials and Methods: This was a descriptive, retrospective study over a period of 11 years from January 2008 to December 2019. We made an exhaustive sampling of all cases of malignant tumors of the larynx in the ENT department of the University Hospital Gabriel TOURE. Variables studied: frequency, sex, age, clinic, imaging, histology, TNM classification, treatment received and follow-up time was 1 to 11 years. Result: we noted a male predominance (44 men/13 women). The average age was 52.23 years, with extremes of 32 and 74 years. Smoking was reported by 40 patients (72% of cases). The average in pack-years is 29.32 with extremes of 4 to 60 packs/ year. Alcoholism was reported by 6 patients (9% of cases). Squamous cell carcinoma was the predominant histological type (100%);extension to 2 or 3 floors was objectified in 69% of cases. Complementary radiotherapy in 26 patients after total laryngectomy. Chemotherapy was done in a neoadjuvant situation in 36 cases. Overall survival was: 52.6% at 1 year, 33.3% at 3 years;26.3% at 5 years and 12.28% at 11 years. Conclusion: Most often, late diagnosis is due to the absence of a national strategy and the underestimation of the first symptoms. Surgery coupled with radiotherapy is the treatment of choice.

Objective: To study the epidemiological, clinical and therapeutic aspects of hearing loss in 15-year-olds and over at the CHU GT of Bamako. Materials and Method: This is a longitudinal prospective study carried out in the ENT department of the University hospital Gabriel Toure in Bamako over 13 months (from September 2018 to September 2019). We carried out an exhaustive sampling of all the patients who consulted in the department for hearing loss and whose deafness was confirmed at pure tone audiometry with age greater than or equal to 15 years. The exclusion criteria were all patients under the age of 15 as well as a hearing loss related to earwax or foreign bodies and refusals to participate in the study. Results: Two hundred and thirty (230) patients were collected during the study. Dominated by the male sex with a sex ratio of 1.5 or 3 men for 2 women, with an average age of 38.16 ± 19 years with extremes ranging from 15 to 86 years. Among our patients, 37.40% had unilateral deafness, progressive installation evolving more than 12 weeks with a rate of cases had headaches 41% and had a history of chronic otitis media (CMO) 27%. There were a few cases of mixed type deafness, 30% had mild deafness, 22% moderate deafness and 40% severe deafness, according to the BIAP classification. Among our patients, 22.16% had conductive hearing loss, and those with sensorineural hearing loss accounted for 15.24%. Conclusion: Pure tone audiometry remains essential in the diagnosis of deafness. It not only makes it possible to confirm or invalidate the deafness, but also to define the type of deafness and especially to classify the deafness according to the degree of the average hearing loss established by the BIAP.

Thrombosis of the Transverse Sinus: About a Clinical Observation and Review of the Literature

Cerebral venous thrombosis has an unfavorable prognosis. It is a rather rare pathology concerning 3 to 5 cases per million inhabitants. The clinical symptomatology also varies according to the topography of the venous thrombosis and, in some cases, the CVT can have an unusual presentation. Progress and accessibility of non-invasive imaging currently allow early diagnosis of CVT. Brain MRI is the reference method for the diagnosis of CVT. We report a case of transverse sinus thrombosis in a 32-year-old male patient who consulted for headaches through which we want to study the etiological, clinical, paraclinical, therapeutic aspects as well as the evolutionary profile. The clinical history dates back to 2 weeks ago with frontal headaches radiating to the occipital region, throbbing of severe intensity, progressive onset and permanent evolution associated with right unilateral anterior purulent rhinorrhea. He had no nasal obstruction, epistaxis, hearing loss or other otological symptoms;no neurological deficit or notion of head trauma. Cerebral and maxillofacial computed tomography showed right maxillary sinusitis and right transverse sinus thrombosis. We carried out medical treatment based on antibiotics and analgesics without the use of anticoagulants. The evolution was favorable after four weeks of treatment. Conclusion: Transverse sinus thrombosis has a non-specific and heterogeneous clinical presentation. Headaches are the first sign. MRI and CT can help establish the diagnosis. The treatment is both etiological and symptomatic.

In Mali, cholesteatoma surgery remains a challenge due to the limited number of otological practitioners, and the low socio-economic level of patients reflecting the difficulties in performing CT or MRI for post-operative follow-up. We therefore initiated this work with the objective of analyzing, through a broad bibliographic review, the clinical, paraclinical, topographical, therapeutic and progressive aspects of a series of ear cholesteatoma. Materials and Method: This was a retrospective and prospective, descriptive study that took place in the ENT and head and neck surgery department of the Gabriel Touré University Hospital over 36 months from November 2020 to October 2023. These were patients admitted to the department for chronic cholestematous otitis media. Result: In total, we collected 34 files of patients admitted for cholesteatoma. This represented 9% of cases compared to all otological surgeries carried out during the same period, i.e. 362 cases. The average age of our patients was 35.31 years, with extremes ranging from 7 years to 80 years, there were 24 men and 10 women, i.e. a M/F ratio of 2.4. The average time to diagnosis was 7 years. The main functional signs were dominated by chronic fetid purulent otorrhea associated with hypoacusis in 94.6% of cases. One case of meningeal complication and three cases of cerebral empyema were reported. Clinically, a postero-superior and marginal tympanic perforation was observed in 53%, a retraction pocket in 5.9%, the sentinel polyp in 9%, and a non-marginal tympanic perforation in 32%. Pure-tone audiometry performed preoperatively for all patients showed conductive hearing loss in 83%. In 82% of cases we created a recess with the creation of a mini box. An ossiculoplasty was performed immediately with fragments of cartilage. The postoperative course found the disappearance of otorrhea (88.23%), the persistence of otorrhea in one case, two cases of facial paralysis, and one case of tinnitus. We did not identify

Differential Geochemical Features of Clinopyroxene from the Floodplain Sediments in Yangtze and Hanjiang Rivers and Its Implication for Detrital Source Identification

The evolution history of the Yangtze River attracted increasing attention in recent years. Comparison of the properties of the floodplain sediments between the upper and lower reaches of the Three Gorges is of significance to identify the material detrital sources for the Yangtze River Basin. Clinopyroxene （Cpx） is the typical mineral widely distributed in the sediments of the Yangtze River Ba- sin. Cpx was extracted from the fine sediments with the grain size ranging from 0.063 to 0.125 mm in the upper and lower reaches of the Three Gorges. The sediments, from which the Cpx was extracted, were taken from the floodplains of the catchments of both the Yangtze River and the Hanjiang River, as well as from the surface of the Zhoulao Core drilled in the center of the Jianghan Basin where the two rivers flow through. Geochemistry of these Cpx was investigated, concerning the contents of major elements measured by electron probe microanalysis （EPMA） and the contents of trace elements analyzed by laser ablation inductively coupled plasma-mass spectrometry （LA-ICP-MS）. The Cpx in the Yangtze River contains titaniferous augite which is sourced from the Panzhihua-Xichang region in South Sichuan Province, whereas the Cpx in the Hanjiang River is mainly from the surroundings without any titanif- erous augite. The REE analysis infers that the titaniferous augite is the diagnostic mineral of the upper Yangtze River, and thus could be used as a tracer mineral to identify the detrital sources for the sedi- ments formed during the evolution of the Yangtze River. The Cpx from the surface sediments of the Jianghan Plain （Zhoulao Core） shows comparable geochemical nature with that of the Yangtze River, but is different in geochemistry from that of the Hanjiang River, implying that the modern sediments in the Jianghan Plain are mainly sourced from the upper Yangtze River.

Carbon and Oxygen Isotopic Compositions: How Lacustrine Environmental Factors Respond in Northwestern and Northeastern China

Surface lake sediments, 28 from Hoh XU, 24 from northeastern China, 99 from Lake Bosten, 31 from Ulungur and 26 from Heihai were collected to determine δ13C and δ18O values. Considering the impact factors, conductivity, alkalinity, pH, TOC, C/N and carbonate-content in the sediments, CI, P, S, and metal element ratios of Mg/Ca, Sr/Ca, Fe/Mn of bulk sediments as environmental variables enable evaluation of their influences on δ13C and δ18O using principal component analysis （PCA） method. The closure and residence time of lakes can influence the correlation between δ13C and δ18O. Lake water will change from fresh to brackish with increasing reduction and eutrophication effects. Mg/Ca in the bulk sediment indicates the characteristic of residence time, Sr/Ca and Fe/Mn infer the salinity of lakes. Carbonate formation processes and types can influence the 6~3C--~1So correlation. ~180 will be heavier from Mg-calcite and aragonite formed in a high-salinity water body than calcite formed in freshwater conditions. When carbonate content is less than 30%, there is no relationship with either 613C or 6180, and also none between δ13C and δ18O. More than 30%, carbonate content, however, co-varies highly to δ13C and δ18O, and there is also a high correlation between δ13C and δ18O. Vegetation conditions and primary productivity of lakes can influence the characteristics of δ13C and δ18O, and their co-variance. Total organic matter content （TOC） in the sediments is higher with more terrestrial and submerged plants infilling. In northeastern and northwestern China, when organic matter in the lake sediments comes from endogenous floating organisms and algae, the δ13C value is high. δ13C is in the range of-4‰ to 0‰ when organic matter comes mainly from floating organisms （C/N〈6）; in the range of-4‰ to 8‰ when organic matter comes from diatoms （C/N=6 to 8）; and -8‰ to -4‰ when organic matter comes from aquatic and terrestrial plants （C/N〉8）.

Demand response(DR) is gaining more and more importance in the architecture of power systems in a context of flexible loads and high share of intermittent generation. Changes in electricity markets regulation in several countries have recently enabled an effective integration of DR mechanisms in power systems. Through its flexible components(pumps, tanks), drinking water systems are suitable candidates for energy-efficient DR mechanisms. However, these systems are often managed independently of power system operation for both economic and operational reasons. Indeed, a sufficient level of economic viability and water demands risk management are necessary for water utilities to integrate their flexibilities to power system operation. In this paper,we proposed a mathematical model for optimizing pump schedules in water systems while trading DR blocs in a spot power market during peak times. Uncertainties about water demands were considered in the mathematical model allowing to propose power reductions covering the potential risk of real-time water demand forecasting *** results were discussed on a real water system in France, demonstrating both economic and ecological benefits.
