Is autoimmune hepatitis a frequent finding among HCV patients with intense interface hepatitis?

AIM:To evaluate the overlap of autoimmune hepatitis in hepatitis C virus(HCV)-infected patients with intense interface hepatitis.METHODS:Among 1759 patients with hepatitis C submitted to liver biopsy,92(5.2%) presented intense interface hepatitis.These patients were evaluated regarding the presence of antinuclear antibody(ANA),anti-smooth muscle antibody(SMA) and anti-liver/kidney microsomal antibody(LKM-1),levels of γ-globulin and histological findings related to autoimmune hepatitis(plasma cell infiltrate and presence of rosettes).RESULTS:Among patients with hepatitis C and intense interface hepatitis there was a low prevalence of autoantibodies(ANA=12%,SMA=5%,LKM-1=0%) and the median γ-globulin level was within the normal range.Typical histological findings of autoimmune disease were observed in only two cases(2%).After applying the score for diagnosis of autoimmune hepatitis,only one patient was classified with a definitive diagnosis of autoimmune hepatitis.Since overlap with autoimmune hepatitis was not the explanation for the intense necroinflammatory activity in patients with chronic hepatitis C we sought to identify the variables associated with this finding.The presence of intense interface hepatitis was associated with more advanced age,both at the time of infection and at the time of the biopsy,and higher prevalence of blood transfusion and alcohol abuse.CONCLUSION:Although possible,overlap with autoimmune hepatitis is a very rare association in HCV-infected patients with intense interface hepatitis,an unusual presentation which seems to be related to other host variables.

Sensory or motor peripheral neuropathy may be observed in a significant proportion of hepatitis C virus (HCV)-infected patients.However,central nervous system (CNS) involvement is uncommon,especially in cryoglobulin-negative subjects.We describe a case of peripheral neuropathy combined with an ischemic CNS event as primary manifestations of chronic HCV infection without cryoglobulinemia.Significant improvement was observed after antiviral therapy.We discuss the spectrum of neurological manifestations of HCV infection and review the literature.

Objectives:To determine whether air pollution or changes in SARS-CoV-2 lineages lead to an increase in mortality.Methods:Descriptive statistics were used to calculate rates of infection(2020-2021).RT ePCR was used to compare viral loads from October 2020 to February 2021.Nextgeneration sequencing(NGS)(n=92)was used to examine and phylogenetically map SARS-CoV-2 lineages.A correlative“air pollution/temperature”index(I)was developed using regression analysis.PM_(2.5),PM_(10),O_(3),NO_(2),SO_(2),and CO concentrations were analyzed and compared to the mortality.Results:The mortality rate during the last year was~32%.Relative SARS-CoV-2 viral loads increased in December 2020 and January 2021.NGS revealed that approximately 80%of SARS-CoV-2 linages were b.1.243(33.7%),b1.1.222(11.2%),b.1.1(9%),b.1(7%),b.1.1.159(7%),and b.1.2(7%).Two periods were analyzed,the prehigh-and high-mortality periods and no significant lineage differences or new lineages were found.Positive correlations of air pollution/temperature index values with mortality were found for IPM_(2.5) and IPM_(10).INO_(2).ISO_(2),and ICO but not for O_(3).Using ICO,we developed a model to predict mortality with an estimated variation of~±5 deaths per day.

Dengue is a global health problem without current specific treatment nor safe vaccines available.While severe dengue is related to pre-existing non-neutralizing dengue virus(DENV)antibodies,the role of T cells in protection or pathology is unclear.Using cutaneous DENV infection in immunocompetent mice we previously showed the generation of PNA+germinal centers(GCs),now we assessed the activation and proliferation of b and T cells in draining lymph nodes(DLNs).We found a drastic remodelling of DLN compartments from 7 to 14 days post-infection(dpi)with greatly enlarged b cell follicles,occupying almost half of the DLN area compared to*24%in na?ve conditions.Enormous clusters of proliferating(Ki-67+)cells inside b follicles were found 14 dpi,representing*33%of b cells in DLNs but only*2%in noninfected mice.Inside GCs,we noticed an important recruitment of tingle body macrophages removing apoptotic cells.In contrast,the percentage of paracortex area and total T cells decreased by 14–16 dpi,compared to controls.Scattered randomly distributed Ki-67+T cells were found,similar to non-infected mice.CD69 expression by CD4+and CD8+T cells was minor,while it was remarkable in b cells,representing 1764.7%of change from basal levels 3 dpi.The apparent lack of T cell responses cannot be attributed to apoptosis since no significant differences were observed compared to noninfected mice.This study shows massive b cell activation and proliferation in DLNs upon DENV infection.In contrast,we found very poor,almost absent CD4+and CD8+T cell responses.

Ni-based catalysts obtained from perovskites oxides for ethanol steam reforming

Perovskites as host structures of cations were used in order to generate in situ active and stable catalysts for ethanol steam reforming. For this purpose,La_(1-x)Mg_xAl_(1-y)Ni_yO_3(x = 0.1; y = 0,0.1,0.2,0.3) perovskites were synthetized by the citrate method.Ni segregation is evident for a substitution level higher than 0. 2. The segregation of Ni as NiO generated species interacts with different metal-support after the reduction step. The y = 0.1 catalyst presents the highest H_2 yield value about 85% during reaction time,with low mean values of CH_4 and CO selectivities of 3.4% and 11%,respectively and a low carbon formation. The better performance of y = 0.1 catalyst could be attributed to the minor proportion of segregated phases,thus a controlled expulsion of Ni is successfully reached.

Idiosyncratic drug-induced liver injury(iDILI) encompasses the unexpected harms that prescription and non-prescription drugs,herbal and dietary supplements can cause to the liver.iDILI remains a major public health problem and a major cause of drug attrition.Given the lack of biomarkers for iDILI prediction,diagnosis and prognosis,searching new models to predict and study mechanisms of iDILI is necessary.One of the major limitations of iDILI preclinical assessment has been the lack of correlation between the markers of hepatotoxicity in animal toxicological studies and clinically significant iDILI.Thus,major advances in the understanding of iDILI susceptibility and pathogenesis have come from the study of well-phenotyped iDILI patients.However,there are many gaps for explaining all the complexity of iDILI susceptibility and mechanisms.Therefore,there is a need to optimize preclinical human in vitro models to reduce the risk of iDILI during drug development.Here,the current experimental models and the future directions in iDILI modelling are thoroughly discussed,focusing on the human cellular models available to study the pathophysiological mechanisms of the disease and the most used in vivo animal iDILI models.We also comment about in silico approaches and the increasing relevance of patient-derived cellular models.

Management of hepatitis C in patients with chronic kidney disease

Hepatitis C virus(HCV) infection is highly prevalent among chronic kidney disease(CKD) subjects under hemodialysis and in kidney transplantation(KT) recipients, being an important cause of morbidity and mortality in these patients. The vast majority of HCV chronic infections in the hemodialysis setting are currently attributable to nosocomial transmission. Acute and chronic hepatitis C exhibits distinct clinical and laboratorial features, which can impact on managementand treatment decisions. In hemodialysis subjects, acute infections are usually asymptomatic and anicteric; since spontaneous viral clearance is very uncommon in this context, acute infections should be treated as soon as possible. In KT recipients, the occurrence of acute hepatitis C can have a more severe course, with a rapid progression of liver fibrosis. In these patients, it is recommended to use pegylated interferon(PEG-IFN) in combination with ribavirin, with doses adjusted according to estimated glomerular filtration rate. There is no evidence suggesting that chronic hepatitis C exhibits a more aggressive course in CKD subjects under conservative management. In these subjects, indication of treatment with PEG-IFN plus ribavirin relies on the CKD stage, rate of progression of renal dysfunction and the possibility of a preemptive transplant. HCV infection has been associated with both liver disease-related deaths and cardiovascular mortality in hemodialysis patients. Among those individuals, low HCV viral loads and the phenomenon of intermittent HCV viremia are often observed, and sequential HCV RNA monitoring is needed. Despite the poor tolerability and suboptimal efficacy of antiviral therapy in CKD patients, many patients can achieve sustained virological response, which improve patient and graft outcomes. Hepatitis C eradication before KT theoretically improves survival and reduces the occurrence of chronic graft nephropathy, de novo glomerulonephritis and post-transplant diabetes mellitus.

In nature,plants acquire nutrients from soils to sustain growth,and at the same time,they need to avoid the uptake of toxic compounds and/or possess tolerance systems to cope with them.This is particularly challenging when the toxic compound and the nutrient are chemically similar,as in the case of phosphate and arsenate.In this study,we demonstrated that regulatory elements of the phosphate starvation response(PSR)coordinate the arsenate detoxification machinery in the cell.We showed that arsenate repression of the phosphate transporter PHT1;1 is associated with the degradation of the PSR master regulator PHR1.Once arsenic is sequestered into the vacuole,PHR1 stability is restored and PHT1;1 expression is recovered.Furthermore,we identified an arsenite responsive SKP1-like protein and a PHR1 interactor F-box(PHIF1)as constituents of the SCF complex responsible for PHR1 degradation.We found that arsenite,the form to which arsenate is reduced for compartmentalization in vacuoles,represses PHT1;1 expression,providing a highly selective signal versus phosphate to control PHT1;1 expression in response to arsenate.Collectively,our results provide molecular insights into a sensing mechanism that regulates arsenate/phosphate uptake depending on the plant’s detoxification capacity.

Damsin (1) is a natural pseudoguaianolide sesquiterpene that inhibits NF-κb, a protein complex that controls the transcription of DNA in mammalian cells, and has a potential for standing model for the development of new anti-cancer lead structures. In order to do a preliminary structure-activity study and improve the anti-cancer activity, fourteen derivatives and analogs were prepared and evaluated. These were chosen to represent both structural diversity and structural novelty. The importance of α methylene-γ-lactone moiety for the anti-cancer activity was confirmed, even though other features in the scaffold were shown to be important for the activity. In some cases a new substitution negatively affected the initial activity, however, two analogues, indolo [3,2-c]-4-desoxydamsin (5) and ambrosin (6), were found to be more potent.

In this work,we propose a new Greedy Forward algorithm to be used in Wireless Sensor Networks （WSNs） routing protocols.Differently from current Greedy Forward algorithms,which depend on the position information of nodes,our approach uses only the Received Signal Strength Indicator （RSSI） of exchanged packets.Since nodes’ position are usually computed based on three or more distance estimations （commonly by means of RSSI measurements） and the positions of beacon nodes （which have inaccuracies by themselves）,by using only RSSI measurements,we should achieve better results.based on this observation,we propose the RSSR （Received Signal Strength Routing） algorithm with two variants:RSSR Election and RSSR Selection.In the RSSR Election,the next hop is dynamically elected at each step and no packets are required for the routing task.In the RSSR Selection,neighbors exchange packets with RSSI information and the next hop of the packet is then selected from a routing table at each step.Our results indicate clearly that the proposed algorithms have all the benefits of a Greedy Forward algorithm but with better performance and without requiring positions information.
