Treatment and survival in a population-based sample of patients diagnosed with gastroesophageal adenocarcinoma

AIM： TO examine the extent of use of specific therapies in clinical practice, and their relationship to therapies validated in clinical trials. METHODS： The US National cancer Institutes＇ Patterns of care study was used to examine therapies and survival of patients diagnosed in 2001 with histologically-confirmed gastroesophageal adenocarcinoma （/7 = 1356）. The study re-abstracted data and verified therapy with treating physicians for a population-based stratified random sample. RESULTS： Approximately 62% of patients had stomach adenocarcinoma （SAc）, while 22% had gastric-cardia adenocarcinoma （GcA）, and 16% lower esophageal adenocarcinoma （EAc）. Stage 1V/ unstaged esophageal cancer patients were most likely and stage Ⅰ -Ⅲ stomach cancer patients least likely to receive chemotherapy as all or part of their therapy; gastric-cardia patients received chemotherapy at a rate between these two. In multivariable analysis by anatomic site, patients 70 years and older were significantly less likely than younger patients to receive chemotherapy alone or chemoradiation for all three anatomic sites. Among esophageal and stomach cancer patients, receipt of chemotherapy was associated with lower mortality; but no association was found among gastric-cardia patients. cONcLUSION： This study highlights the relatively low use of clinical trials-validated anti-cancer therapies in community practice. Use of chemotherapy-based treatment was associated with lower mortality, dependent on anatomic site. Findings suggest that physicians treat lower esophageal and SAc as two distinct entities, while gastric-cardia patients receive a mix of the treatment strategies employed for the two other sites.

An Innovative Method to Accommodate chinese Medicine Pattern Diagnosis within the Framework of Evidence-Based Medical Research

Pattern diagnosis is an integral aspect of chinese medicine （cM）. cM differentiates biomedical diseases into patterns, based upon the patient＇s symptoms and signs. Pattern identification （PI） is used to diagnose, direct the treatment principle and determine the treatment protocol. Most cM research has used fixed formula treatments for Western-defined diseases with outcomes measured using objective biomedical markers. This article presents an innovative method used in a randomised controlled pilot study using acupuncture for participants with hepatitis c virus. Each participant＇s cM patterns were identified and quantified at baseline which directed the treatment protocol for the treatment group. Data identified that while each participant expressed different patterns at baseline all participants displayed multiple patterns. Six patterns showed some expression by all 16 participants; Liver （Gan） yin vacuity expressing a group aggregate mean percentage of 47.2, binding depression of Liver qi 46.9, and Liver Kidney （Shen） yin vacuity 45.1. Further sub category gender grouping revealed that pattern ranking changed with gender; Liver yin vacuity （male 53.4%, female 51.93%）, binding depression of Liver qi （male 50.0%, female 42.86%） and Liver Kidney yin vacuity （male 42.9%, female 47.96%）. The quantification of cM patterns described in this article permitted statistical evaluation of presenting cM patterns. Although this methodology is in its infancy it may have potential use in the integration of PI with rigorous evidence based clinical research. Biomedical markers often do not relate to symptom/signs and therefore this innovative measure may offer an additional cM evaluation methodology and further cM PI understanding.

We report synthesis windows for growth of millimeter-long ZnTe nanoribbons and ZnSe nanowires using vapor *** tuning the local conditions at the growth substrate,high aspect ratio nanostructures can be synthesized.A cu-ion immersion doping method was applied,producing strongly p-type conduction in ZnTe and ionic conduction in *** extreme aspect ratio wide-bandgap semiconductors have great potential for high density nanostructured optoelectronic circuits.

Vaginal Birth after One caesarean Section—Ten Years Experience in a South Eastern Nigerian Hospital

Background: The rising caesarean section rate worldwide is an increasing source of concern to all. A significant contributor to this is a previous caesarean delivery. One strategy in the armamentarium of obstetricians for reducing this is vaginal birth after one caesarean section (VBAc). The safety, outcome and complications of this procedure in our center which is in a developing country had never been studied hence the need for this work. Aim: To find out the outcome and complications of vaginal births after one caesarean delivery in our center. Method: A retrospective study involving a ten-year review of all cases of trials of labour after one caesarean delivery in our center was done. Result: Of the 305 trials of labour after one caesarean delivery, 221 women had vaginal birth giving a vaginal birth after one caesarean section rate of 72.5%. Majority of the babies 303 (99.2%) were alive and had good Apgar scores while 2 (0.7%) died giving a perinatal mortality rate of 7 per 1000 babies. There were complications in 21 (6.7%) of the women with retained placenta, in 7 (2.3%) of the women as the commonest, followed by postpartum haemorrhage in 6 (1.96%). Five (1.62%) women had scar dehiscence while 3 (0.98%) had uterine rupture. conclusion: Vaginal birth after one caesarean section is safe in well selected cases even in our resource poor settings. It offers our women a culturally-acceptable option of fulfilling long-held dreams of vaginal delivery even after one caesarean section and should be offered to our women whenever possible.

Direct measurement of oxygen consumption rates from attached and unattached cells in a reversibly sealed, diffusionally isolated sample chamber

Oxygen consumption is a fundamental component of metabolic networks, mitochondrial function, and global carbon cycling. To date there is no method available that allows for replicate measurements on attached and unattached biological samples without compensation for extraneous oxygen leaking into the system. Here we present the Respiratory Detection System, which is compatible with virtually any biological sample. The RDS can be used to measure oxygen uptake in microliter-scale volumes with a reversibly sealed sample chamber, which contains a porphyrin-based oxygen sensor. With the RDS, one can maintain a diffusional seal for up to three hours, allowing for the direct measurement of respiratory function of samples with fast or slow metabolic rates. The ability to easily measure oxygen uptake in small volumes with small populations or dilute samples has implications in cell biology, environmental biology, and clinical diagnostics.

目的：评价联合应用沙利度胺与脂质体阿霉素治疗复发性卵巢上皮性癌和腹膜癌的疗效和毒副作用。方法：将9例经手术和术后泰素＋卡铂化疗后复发患者纳入本研究。每4周予以脂质体阿霉素30～50mg／m^2化疗，共3～8个疗程；同时每日予以沙利度胺100～300mg，其剂量从100mg／d开始，此后每2周增加50～100mg。通过体检、影像学检查和测定cA125水平评价治疗效果。结果：9例患者的中位年龄为56岁（35～81岁），FIGO分期为ⅢB～Ⅳ期。本次化疗的中位疗程数是6个疗程。中位随访时间为18．5个月。最常见的不良反应为疲劳（55．6％）和神经系统症状（55．6％）。3例患者出现Ⅲ度副反应，1例为无力、1例过敏性皮疹和1例末梢神经感觉异常。根据REcIST标准评价有病灶的6例患者疗效，2例部分缓解，2例治疗中疾病无进展，1例病情进展，1例失访。另有2例cA125升高的患者，在联合化疗3～4个疗程后cA125水平降低50％以上。1例二次肿瘤细胞减灭术后化疗的患者治疗后随访36个月无肿瘤复发迹象。在随访的8例患者中，中位疾病无进展时间为6．5个月（0～36个月）；中位总的生存时间为20．5个月（8～36个月）。结论：联合应用沙利度胺与脂质体阿霉素治疗复发性卵巢上皮性癌具有可接受的毒副作用，同时具有一定的治疗效果。
