Regarding p.ognosis, p.tients with a non-ST elevation acute coronary syndrome(ACS) are a very heterogeneous p.p.lation, with varying risks of early and long-term adverse events. Early risk stratification at admission seems to be essential for a tailored therap.utic strategy. We sought to comp.re the p.ognostic value of three ACS risk scores(RSs) and their ability to p.edict benefit from myocardial revascularization p.rformed during initial hosp.talization. Methods and results: We studied 460 consecutive p.tients admitted to our coronary care unit with an ACS[age: 63± 11 years, 21.5% female, 55% with myocardial infarction(MI)]. For each p.tient, the Thrombolysis In Myocardial Infarction(TIMI), p.atelet glycop.otein IIb/IIIa in Unstable agina: Recep.or Sup.ression Using Integrilin(p.RSUIT), and Global Registry of Acute Coronary Events(GRACE) RSs were calculated using sp.cific variables collected at admission. Their p.ognostic value was evaluated by the combined endp.int of death or Ml at 1 year. The best cut-off value for each RS, calculated with receiver op.rating characteristic curves, was used to assess the imp.ct of myocardial revascularization on the combined incidence of death or MI. death or MI at 1 year was 15.4% (32 deaths/49 MIs). The best p.edictive accuracy for death or MI at 1 year was obtained by the GRACE RS(AUC)[area under the curve: 0.715; confidence interval(CI: 0.672- 0.756)] but the p.rformance of the p.RSUIT RS(AUC: 0.630; CI: 0.584- 0.674), and TIMI RS(AUC: 0.585; CI: 0.539- 0.631) was also good. We found a statistically significant interaction between the risk stratified by the best cut-off value for the GRACE and p.RSUIT RSs and myocardial revascularization, with a better p.ognosis for the high-risk p.tients. The high-risk p.tients rep.esented 36.7, 28.7, and 57.8% of the p.p.lation, for the GRACE, p.RSUIT, and TIMI RSs, resp.ctively. Orchiectomy is the recommended treatment for many testicular ***,testicular p.osthesis p.acement is not always p.rformed for different *** this study,we aimed to evaluate p.tients'op.nions and the imp.ct on sexual function and quality of *** retrosp.ctive observational single-center study included p.tients who underwent orchiectomy between January 2014 and december 2020 at the dep.rtment of Urology,Braga's Hosp.tal(Braga,p.rtugal),where testicular imp.ants were always available and the decision to undergo the p.ocedure was made fully indep.ndent of *** comp.eted four questionnaires that assessed demograp.ic data,satisfaction,self-esteem,and sexual *** the 96 p.tients who underwent orchidectomy,59 rep.ied to the questionnaires,and of these p.tients,86.4%decided to undergo silicone-based testicular p.ostheses *** remaining 13.6%refused the imp.ant based on concerns about comp.ications(37.5%),because they felt that it was unnecessary(37.5%),or because it was not offered by the doctor(25.0%).Overall,96.1%of these p.tients were satisfied with the imp.ant;however,25.5%classified it as"too firm".No statistically significant differences were found in sexual function(all p.0.05).However,it can be observed that there are more p.tients with p.ostheses p.esenting normal sexual activity comp.red to p.tients without p.ostheses(74.0%vs 50.0%),and none of them rep.rted severe erectile dysfunction(0 vs 16.7%).Regarding self-esteem,both p.tients with and without p.ostheses p.esent very similar average scores with no statistically significant *** p.esent study highlights the highest level of satisfaction among p.tients who received testicular *** p.ostheses imp.antation is a safe p.ocedure that does not hamp.r sexual function after orchiectomy.

Introduction: At the beginning of the COVID-19 p.ndemic, a drop.in the number of p.tients treated for cardiac emergencies raised concern about cardiovascular mortality in that p.riod. An increase in care delay for p.tients with ST-segment elevation myocardial infarction (STEMI) may have affected clinical outcomes. Objectives: To analyze delay times and clinical outcomes of p.tients with STEMI undergoing p.imary p.rcutaneous coronary intervention (p.CI), before and during the COVID-19 p.ndemic. Methods: Retrosp.ctive observational study that included p.tients with STEMI undergoing p.CI from december 2018 to July 2021. The COVID-19 p.ndemic cases were divided into two group.: p.ndemic I—from March to August 2020;and p.ndemic II—from Sep.ember 2020 to July 2021. p.tients were comp.red according to the p.riod of hosp.talization. p.imary outcomes were delay times in assistance and clinical outcomes (acute kidney injury [AKI], p.st-p.ocedural vascular comp.ications and in-hosp.tal mortality). Results: 108 p.tients were included, 39 (36.1%) in the p.e-p.ndemic p.riod, 13 (12.1%) in p.ndemic I and 56 (51.8%) in p.ndemic II. Time from onset of symp.oms to arrival at the service and door-to-balloon time did not differ significantly among group.. Vascular comp.ications were more frequent during the p.ndemic (I and II) than in the p.e-p.ndemic p.riod (2.5% p.e-p.ndemic vs 15.4% p.ndemic vs 12.5% p.ndemic II;p.= 0.03). AKI incidence was similar in all three p.riods. There was a non-significant increase in in-hosp.tal mortality during the COVID-19 p.ndemic. Conclusion: In p.tients with STEMI, there was an increase in vascular comp.ications and a trend toward increased mortality during the COVID-19 p.ndemic. delay times to admission and rep.rfusion did not differ significantly between before and during the p.ndemic.

BACKGROUND In p.tients with metastatic colorectal cancer(mCRC),the treatment op.ions are limited and have been p.oved to be affected by rat sarcoma virus(RAS)mutational *** RAS wild-typ.(wt)p.tients,the combination of antiep.dermal growth factor recep.or(EGFR)monoclonal antibodies with chemotherap.(CT)is more effective than CT *** the other hand,RAS-mutated p.tients are not eligible for treatment with anti-EGFR *** SUMMARY Eleven p.tients with initially RAS-mutated mCRC were followed from diagnosis to May *** the time of cell-free DNA determination,five p.tients had undergone one CT line,five p.tients had undergone two CT lines,and one p.tient had undergone three CT lines(all in combination with bevacizumab).At the second and third treatment lines[second line(2L),third line(3L)],p.tients with neo-RAS wt received a combination of CT and *** neo-RAS wt p.tients treated with anti-EGFR,our findings indicated an increase in p.ogression-free survival for both 2L and 3L(14.5 mo,p.0.119 and 3.9 mo,p.0.882,resp.ctively).Regarding 2L overall survival,we registered a slight increase in neo-RAS wt p.tients treated with anti-EGFR(33.6 mo vs 32.4 mo,p.0.385).At data cut-off,two p.tients were still alive:A RAS-mutated p.tient undergoing 3L treatment and a neo-RAS wt p.tient who received 2L treatment with anti-EGFR(ongoing).CONCLUSION Our case series demonstrated that monitoring RAS mutations in mCRC by liquid biop.y may p.ovide an additional treatment line for neo-RAS wt p.tients.
