Retrospective chart review of skin cancer presence in the wide excisions

AIM: To investigate cancer cell absence or presence in wide excision after biopsy of squamous cell carcinoma(SCC) and basal cell carcinoma(BCC) ***: 200 patients(100 BCC and 100 SCC) from the same dermatology clinic, who had positive margin upon biopsy, were selected from a computer generated randomized report. All selected patients had wide excision following biopsy. To determine the correlation of gender, age distribution and cancer absence, BCC and SCC cases were separated based on excision-cancer absent or present after wide excision. χ2 tests, Fisher's exact tests were used to analyze the ratio of male to female between excision-cancer absent and excision-cancer present patients, while Mann-Whitney U test were used to compare the age distribution in the two groups. Statistical analyses were performed using SPSS version 16.0 for ***: Our retrospective chart review of the patients showed that cancer cells were absent in 49% of BCC patients(n = 100) and 64% of SCC patients(n = 100) who had previously had positive margins upon biopsy. Gender analysis showed the ratio of male to female(M/F) in the BCC arm was significantly higher compared with the SCC arm in those with excision-cancer absent(2.06 vs 0.66; P = 0.004; χ2 test). But M/F of excision-cancer absent and excision-cancer present in neither BCC nor SCC patients was statistically significant. Age adjustment showed no significant difference between excision-cancer absent and excision-cancer present in BCC and SCC patients. Nevertheless, in excision-cancer absent cases, the age distribution showed that the BCC patients were younger than SCC patients(average age 67 vs 74; P lso indicated that in the patient group of 71-80 years old, there were more SCC patients who showed excision-cancer absence(67.6% vs 39.4%; P = 0.02; χ2 test).CONClUSION: Our study indicates that approximately 50% or more of BCC and SCC patients with positive margin

Evaluation of Stocking All Female largemouth Bass (Micropterus salmoides) in Alabama (USA) Ponds

Excessive largemouth bass ( Micropterus salmoides ) recruitment in small impoundments leads to density-dependent growth depression, causing populations to have length distributions skewed towards smaller fish. To address this issue, we conducted three trials that evaluated stocking all female largemouth bass (F-lMB) in two small Alabama ponds (0.5 and 2.0 ha) where the drainage basin was isolated. Age-1 fish were individually identified for sex using a micro pipette inserted in the urogenital pore, tagged, and stocked at 38-40 F-lMB/ha into ponds that contained pre-established sunfish ( lepomis spp.) populations. Female largemouth bass were collected with electrofishing, angling, and rotenone over a 2.5- to 5-year period among the three trials. Sex was correctly identified for 179 of the 180 F-lMB stocked. One male was detected in the first trial which resulted in successful largemouth bass reproduction and this trial was terminated after two years. Growth was rapid the first 2 years after stocking as 3-year-old fish reached an average of 429 to 459 mm and 1.37 to 1.66 kg in all 3 trials. However, 2 years after stocking, growth was nil in the second trial even though relative weights of F-lMB were generally greater than 100. In this second trial, removal of about 30% of the F-lMB inhabiting the pond and stocking small Nile tilapia Oreochromis niloticus (25 - 75 mm) resulted in average weights increasing from 1.30 to 2.34 kg in 1 year. Growth of F-lMB continued in the third trail, and average size approached 500 mm and 2.2 kg three years after stocking. Annual survival rates of F-lMB were high and ranged from 0.78 to 0.93 among the 3 trials. Stocking F-lMB offers an attractive alternative in ponds to create a low density largemouth bass population that displays fast growth and high survival when catch-and-release fishing is primarily practiced.

用三个红皮软粒冬小麦(Triticum aestivum l)品种研究了去穗对干物质分配的影响。田间小区按随机区组裂区试验设计(品种为主区,穗处理为副区)。穗处理为两种,一是不去穗(对照),二是在开花后4天去25％的穗(SR)。从去穗之日起刮生理成熟,每隔七天测一次分蘖各器官(穗、穗梗,茎和蘖)的干物重和非结构性炭水化合物总量(TNC)。同对照相比,SR的籽粒产量下降25％,但处理间粒重无显著差异。在SR处理后的一周或二周后,SR副区中蘖,穗梗和茎杆样本的干重和TNC均比对照高。就几个品种平均而言,在整个灌浆期间,对照样本中的净干重增加了1.5g。相比之下,SR副区中的无穗蘖在开花至成熟期间,干物质损失了1.89g。这些发现说明,由无穗蘖生产的干物质并未向同株去穗蘖大量运送以致影响产量。

目的研究环丙沙星在儿科患者中使用与关节病、其他不良事件以及药物相互作用相关的安全性数据。方法在MEDlINE、EMBASE、CINAHl、CENTRAl数据库以及其他相关文献中对相关文章进行系统搜索。对所有研究中使用了环丙沙星且患者年龄≤17a的所有类型研究进行收集。对研究中有报道了药物安全性数据的研究进行进一步分析。结果共105篇文章符合纳入标准,共涉及16184例儿科患者。共发现有1065例不良事件的报告(发生率7%,95%CI:3.2%～14.0%)。最常见的不良事件为肌肉骨骼不良事件、肝功能检查异常、恶心、白细胞计数变化和呕吐。共发现6例患者出现了药物之间相互作用[与氨茶碱(4例),与甲氨蝶呤(2例)]。仅在1例新生儿中由于发生过敏反应导致死亡。在232例患者中发生了258例的肌肉骨骼不良事件(发生率1.6%,95%CI:0.9%～2.6%)。其中关节痛占50%。发生关节病的年龄范围为7个月～17a(中位年龄为10a)。所有患者的关节病在治疗后都有所缓解或者被治愈。1项前瞻性对照研究中估计关节病的发生风险为9.3(OR95%CI:1.26～1.97)。结论使用环丙沙星会引发肌肉骨骼不良事件的发生。但是,这些肌肉骨骼不良事件经过治疗后都是可逆的。关于环丙沙星诱发关节病的风险问题还需要进一步的研究。

新发晚期卵巢癌患者使用拓扑替康/卡铂后使用紫杉醇/卡铂治疗的Ⅱ期研究

Objective. Incorporating topotecan into standard platinum/taxane chem otherapy for advanced ovarian cancer has been complicated by myelosuppression. This study evaluated sequential doublets of topotecan and carboplatin, followed by paclita xel and carboplatin, in newly diagnosed advanced ovarian cancer patients. Method s. Forty-five patients (median age, 56 years; range, 38-77 years) with stage I II/IV disease and GOG performance status lled and received four cyc les of topotecan (1.0 mg/m2/day on days 1 to 3) and carboplatin (AUC 4 on day 1) , followed by four cycles of paclitaxel (175 mg/m2 via 3-h IV infusion on day 1 ) and carboplatin (AUC 5 on day 1). All cycles were 21 days. Antitumor response was assessed after four and eight cycles; patients with clinical complete respon se (CR) underwent second-look laparotomy for determination of pathologic CR (PC R). Dose reductions were instituted for grade 4 neutropenia and thrombocytopenia , and for grade 3/4 nonhematologic toxicity. Results. Among 41 CA-125 evaluable patients, complete and partial responses were observed in 29 (70.7%) and 11 (26.8%) patients, r espectively. Of the 12 clinical CRs (43%) in 28 evaluable patients, 10 patients underwent second-look laparotomy, with 3 PCRs (30%). Median time to progressi on was 14 months and actuarial survival was 23 months. Neutropenia was the prima ry toxicity and cause of dose adjustments and delays, including two deaths. Conc lusion. The antitumor activity observed is comparable with other series, althoug h neutropenic complications were increased. Progression-free and actuarial surv ivals were slightly inferior. A Phase Ⅲtrial (GOG182) of sequential doublets in the reverse sequence is ongoing.

The CO2laser has been used in large tumors about the entire body, but, primarily,our experience has been with tumors of the head and neck. We find that in cutting fla-ps, either from the chest, neck or from the forehead, the laser works excellently. It isparticularly helpful when patients have nay sort of blood dyscrasia in which the plateletsare lowered. Its use in the face of infection is excellent and tends to stcrilize the base asone excises the lesion. We have used it for many different purposes-excision of tumorsof the face with either primary closure or skin graft and tumors of the scalp with much thesame sort of treatment.
