Neuropathic pain is chronic pain generated by disorders of the peripheral and central nervous system, including skull base tumours. A skull base tumour can be any type of tumour that forms in the skull base, and this includes vestibular schwannomas which arise from the sheath of the inner ear vestibulocochlear nerve(eighth cranial nerve). Growth of the tumour, surgical resection, and/or stereotactic radiotherapy may result incompression and/or irritation of the fifth cranial nerve(trigeminal nerve) resulting in facial pain and/or numbness. Non-trigeminal afferent input may contribute to the wide constellation of symptoms seen in orofacial pain patients. The purpose of this report was to develop a decision tool to guide the recognition and treatment of neuropathic pain in this specialized population. Recommendations for treatment are based on evidence presented in Canadian and international neuropathic treatment guidelines. Algorithms are included for assessment and treatment of adult patients with agents that are recognized to have analgesic efficacy within the broad context of neuropathic pain.

Neutropenia in colorectal cancer treated with oxaliplatin-based hyperthermic intraperitoneal chemotherapy: An observational cohort study

BACKGROUND The implications of neutropenia after cytoreductive surgery and hyperthermic intraperitoneal chemotherapy(HIpEC)treatment have never been *** To evaluate the occurrence of neutropenia and its effect on the risk of increased Clavien-Dindo morbidity as well as its effect on overall or disease-free *** All patients with colorectal peritoneal metastases(1996-2015)completing cytoreductive surgery and oxaliplatin-based HIpEC treatment from a biinstitutional database(Uppsala and Sydney)were included in the ***-Dindo grade 3-4 morbidity differences between the neutropenia group vs nonneutropenia group were calculated and Kaplan-Meier curves with log rank test were *** and multivariable Cox regression models for diseasefree survival were *** Two hundred and forty-six patients were identified–32 postoperative any-grade neutropenia patients and 214 non-neutropenia *** neutropenia group had more combination oxaliplatin+irinotecan treatment than the nonneutropenia group(66%vs 13%,p=0.0001).The neutropenia group was not associated with increased Clavien-Dindo grade 3-4 *** overall survival was 53 mo vs 37 mo for the neutropenia and non-neutropenia group,p=*** disease-free survival was 16 mo vs 11 mo,respectively,p=*** was an independent prognostic factor for disease-free survival with hazard ratio:0.58,95%confidence interval:0.36-0.95,p=*** 13%of patients developed neutropenia which was not associated with increased Clavien-Dindo grade 3-4 *** was an independent positive prognostic factor for disease-free survival and was associated with more intense HIpEC *** is in direct contrast to the current paradigm of decreasing the treatment intensity.

Localized CO_2 laser bonding process for MEMS packaging

The packaging poses a critical challenge for commercialization of MEMS products. Major problems with the packaging process include degraded reliability caused by the excess stress due to thermal mismatch and altered performance of the MEMS device after packaging caused by thermal exposure. The localized laser bonding technique for ceramic MEMS packaging to address above-mentioned challenges was investigated. A continuous wave CO2 laser was used to locally heat sealing material for ceramic MEMS package lid to substrate bonding. To determine the laser power density and scanning speed, finite element analysis thermal models were constructed to simulate the localized laser bonding process. Further, the effect of external pressure at sealing ring on the bonding formation was studied. pull testing results show that the scanning speed and external pressure have significant influence on the pull strength at the bonding interface. Cross-sectional microscopy of the bonding interface indicates that the packages bonded with relatively low scanning speed and external pressure conditions have higher bonding quality. This research demonstrates the potential of localized laser bonding process for ceramic MEMS packaging.

Contemporary coronary artery bypass grafting

Current evidence clearly demonstrates that coronary artery bypass grafting （CABG） remains the ＂gold standard＂ treatment for most patients with multivessel and left main stem disease. This article summarizes the relevant evidence basis demonstrating that CABG, in comparison to stenting, reduces mortality and subsequent myocardial infarction and the need for repeat revascularization. The article also describes the evidence basis to support the use of more arterial grafts during CABG and the current role of off-pump CABG.

Direct-to-indirect mapping for optimal low-thrust trajectories

Optimal,many-revolution spacecraft trajectories are challenging to solve.A connection is made for a class of models between optimal direct and indirect *** transfers that minimize thrust-acceleration-squared,primer vector theory maps direct,many-impulsive-maneuver trajectories to the indirect,continuous-thrust-acceleration *** mapping algorithm is independent of how the direct solution is obtained and requires only a solver for a boundary value problem and its partial derivatives.A Lambert solver is used for the two-body problem in this *** mapping is simple because the impulsive maneuvers and co-states share the same linear space around an optimal *** numerical results,the direct coast-impulse solutions are demonstrated to converge to the indirect continuous solutions as the number of impulses and segments *** two-body design space is explored with a set of three many-revolution,many-segment examples changing semimajor axis,eccentricity,and *** first two examples involve a small change to either semimajor axis or eccentricity,and the third example is a transfer to geosynchronous *** a single processor,the optimization runtime is seconds to minutes for revolution counts of 10 to 100,and on the order of one hour for examples with up to 500 *** of these thrust-acceleration-squared solutions are good candidates to start a homotopy to a higher-fidelity minimization problem with practical constraints.
