Background Predictive scales have been used to prognosticate long-term outcomes of traumatic brain inj.ry(TBI),but gaps remain in predicting mortality using initial trauma resuscitation *** sought to evaluate the association of clinical variables collected during the initial resuscitation of intubated pediatric severe patients with TBI with in-hospital *** Intubated pediatric trauma patientsj.nuary 2011 to December 2020 were *** between initial trauma resuscitation variables(temperature,pulse,mean arterial blood pressure,GCS score,hemoglobin,international normalized ratio(INR),platelet count,oxygen saturation,end tidal carbon dioxide,blood glucose and pupillary response)and mortality were evaluated with multivariable logistic *** Among 314 patients,median age was 5.5 years(interquartile range(IQR):2.2-12.8),GCS score was 3(IQR:3-6),Head Abbreviated Inj.ry Score(hAIS)was 4(IQR:3-5),and most had a severe(25-49)Inj.ry Severity Score(ISS)(48.7%,153/314).Overall mortality was 26.8%.GCS score,hAIS,ISS,INR,platelet count,and blood glucose were associated with in-hospital mortality(all p<0.05).As age and GCS score increased,the odds of mortality *** 1-point increase in GCS score was associated with a 35%decrease in odds of *** hAIS,INR,and blood glucose increased,the odds of mortality *** each 1.0 unit increase in INR,the odds of mortality increased by 1427%.Conclusions Pediatric patients with severe TBI are at substantial risk for in-hospital *** are needed to examine whether earlier interventions targeting specific parameters of INR and blood glucose impact mortality.

Increased expression levels of the RNA splicing regulator Transformer2fl （abbreviated Tra2fl） have been reported in several types of cancer. Recent work has revealed an intimate cross-regulation between Tra2fl and the highly similar Tra2a protein in human breast cancer cells, though these two proteins are encoded by separate genes created by a gene duplication that occurred over 500 million years ago. This cross-regulation involves splicing control of a special class of exons, called poison exons. Down-regulation of Tra2fl reduces splicing inclu- sion of a poison exon in the mRNA encoding Tra2a, thereby up-regulating Tra2a protein expression. This buffers any splicing changes that might be caused by individual depletion of Tra2fl alone. Discovery of this cross-regulation pathway, and its by-pass by j.int deple- tion of both human Tra2 proteins, revealed Tra2 proteins are essential for breast cancer cell viability, and led to the identification of important targets for splicing control. These exons include a critical exon within the checkpoint kinase 1 （CHK1） gene that plays a crucial function in the protection of cancer cells from replication stress. Breast cancer cells depleted for Tra2 proteins have reduced CHK1 protein levels and accumulate DNA damage. These data suggest Tra2 proteins and/or their splicing targets as possible cancer drug targets.

Obj.ctives: Ultrasound guidance increases the success of internal j.gular central venous catheter (CVC) placement. The common femoral vein (CFV) is considered an easier site and physicians frequently place CFV CVCs using anatomical landmark guidance. No research has demonstrated the effect of patient weight on placement of a femoral CVC. The authors hypothesized that resident physicians could determine by anatomical landmarks the insertion site of a femoral CVC in normal and overweight adults. Methods: Investigators consented adult patients in the Emergency Department to serve as study models. Patients were grouped by BMI: Normal (BMI (BMI > 25). PGYI-III Emergency Medicine residents who volunteered to participate were included. Residents inspected and palpated the inguinal regions on a patient and marked the patient’s skin directly over the CFV. A faculty member assessed success using a 10 MHz linear probe (Sonosite Micro Maxx) centered over the mark. The attempt was successful if the mark was directly over the CFV and unsuccessful if distal, lateral, or medial to the CFV. Results: Nineteen residents (6 PGYI, 7 PGY II, and 6 PGYIII) assessed 17 patients. There were 43 attempts on Normal patients and 57 attempts on Overweight patients. The success rate on Normal patients was 74% and on Overweight patients was 42% (p = 0.0021). Conclusion: Residents were more successful in correctly identifying the CFV by anatomical landmarks in Normal patients. Training year had no effect on success. Ultrasound guidance may increase the success rate of placing a femoral CVC in patients with BMI > 25.

We provide a review of our recent 100-Gb/s, high spectral efficiency （SE） experiment targeting transoceanic and regional undersea transmission distances. We demonstrated that simple pre-filtering at the transmitter together with a maximum a posteriori probability （MAP） detection algorithm can significantly improve SE. We transmitted 96×100-Gb/s pre-filtered polarization division multiplexed return-to-zero quadrature phase shift keyed （PDM-RZ-QPSK） channels with 300% SE over 10608 km using 52-km spans of 150-μm 2 fiber and simple single-stage erbium-doped fiber amplifiers （EDFAs）. We also achieved 400% SE over 4368 km using similar techniques.

Ginger (Zingiber officinale) is rich in antioxidants and phytochemicals with anti-inflammatory, antimicrobial and anticancer properties. Ginger has been used as a condiment and for the treatment of ailments for many years. However, there are limited studies on the antioxidant and scavenging power of processed ginger. The obj.ctives of this study were to determine the effects that processing has on the total phenolic and flavanoid content of ginger and its antioxidant potential using 1,1-Diphenyl-2-picrylhydrazyl (DPPH), Radical-Scavenging Activity and Ferric Reducing Antioxidant Power (FRAP). Fresh ginger (FG) was used as a control with oven drying (OD) sun drying (SD) and freeze drying (FD) as processing methods. Freeze-dried ginger (77.87%) showed a significantly higher (p < 0.05) free radical scavenging ability as compared to other treatment groups (18.40% - 72.90%). However, sundried ginger had the highest ferric-reducing antioxidant power at 35.28 ± 0.69 mmol Fe (II)/mg ginger. Total phenolic content content (mg GAE/100g) for fresh, oven died, sun-dried, and freeze-dried were 514.02 ± 3.92, 796.46 ± 8.16, 878.76 ± 13.17, and 1021.15 ± 12.95, respectively. Flavanoid content of processed ginger was highest in freeze-dried samples (458.82 CE/100g ginger).

Excited-state singlet axygen(^(1)O_(2)),generated during photodynamic therapy(PDT),is believed to be the primary cytotoxic agent with a number of clinically approved *** relative concentration in cells or tissues can be measured directly through its near-infrared(NIR)luminescence emission,which has correlated well with in vitro cell and in triro normal skin treatment ***,its correlation with the response of tumor tiss1e in vito is examined,using the photosensitizer meso-tetralydroxyphenylchlorin(mTHPC)in an animal model comprising luciferase-and green fluorescent protein(GFP)-transduced gliosarcoma grown in a dorsal window *** change in the bioluminescence signal,imaged pre-treatment and at 2,5 and 9 d post treatment,was used as a quantitative measure of the tumor response,which was classified in individual tumors as"non","moderate"and"strong"in order to reduce the variance in the *** the bioluminescence-based response vs the^(1)O_(2)counts demonstrated clear correlation,indicating tha^(1)O_(2)luminescence provides a valid do-simetric technique for PDT in tumor tissue.
