Simple liver cysts are congenital with a prevalence of 2.5%-4.25%. Imaging, whether by US, cT or MRI, is accurate in distinguishing simple cysts from other etiologies, including parasitic, neoplastic, duct-related, and traumatic cysts. Symptomatic simple liver cysts are rare, and the true frequency of symptoms is not known. Symptomatic simple liver cysts are predominantly large （〉 4 cm）, right-sided, and more common in women and older patients. The vast majority of simple hepatic cysts require no treatment or follow-up, though large cysts （〉 4 cm） may be followed initially with serial imaging to ensure stability. Attribution of symptoms to a large simple cyst should be undertaken with caution, after alternative diagnoses have been excluded. Aspiration may be performed to test whether symptoms are due to the cyst; however, cyst recurrence should be expected. Limited experience with both laparoscopic deroofing and aspiration, followed by instillation of a sclerosing agent has demonstrated promising results for the treatment of symptomatic cysts. Here, we describe a patient with a large, symptomatic, simple liver cyst who experienced complete resolution of symptoms following cyst drainage and alcohol ablation, and we present a comprehensive review of the literature.

Physical inactivity is a well-established risk factor for chronic diseases,such as cardiovascular disease,cancer,and diabetes *** is a growing awareness that physical inactivity should also be regarded as a risk factor for acute respiratory infections(ARIs).ARIs,such as the common cold,influenza,pneumonia,and coronavirus disease 2019(cOVID-19),are among the most pervasive diseases on earth and cause widespread morbidity and *** in support of the linkage between ARIs and physical inactivity has been strengthened during the cOVID-19 pandemic because of increased scientific ***-scale studies have consistently reported that the risk for severe cOVID-19 outcomes is elevated in cohorts with low physical activity and/or physical fitness,even after adjusting for other risk *** lowered risk for severe cOVID-19 and other ARIs in physically active groups is attributed to exercise-induced immunoprotective effects,including enhanced surveillance of key immune cells and reduced chronic *** consensus groups,including those who submitted the Physical Activity Guidelines for Americans,have not yet given this area of research the respect that is *** is time to add“reduced risk for ARIs”to the“Exercise is Medicine”list of physical activity-related health benefits.

The literature suggests there is about a 1 % risk per year of a 10 % global agricultural shortfall due to catastrophes such as a large volcanic eruption, a medium asteroid or comet impact, regional nuclear war, abrupt climate change, and extreme weather causing multiple breadbasket failures. This shortfall has an expected mortality of about 500 million people. To prevent such mass starvation, alternate foods can be deployed that utilize stored biomass. This study developed a model with literature values for variables and, where no values existed,used large error bounds to recognize uncertainty. Then Monte carlo analysis was performed on three interventions: planning, research, and development. The results show that even the upper bound of USD 400 per life saved by these interventions is far lower than what is typically paid to save a life in a less-developed country. Furthermore, every day of delay on the implementation of these interventions costs 100–40,000 expected lives(number of lives saved multiplied by the probability that alternate foods would be required). These interventions plus training would save 1–300 million expected lives. In general, these solutions would reduce the possibility of civilization collapse, could assist in providing food outside of catastrophic situations, and would result in billions of dollars per year of return.

This paper reviews the historical evolution of the language and organization surrounding the health of personnel in the British Army from‘hygiene’through to‘wellbeing’.It starts by considering the health of the army in the midnineteenth century and the emergence of military hygiene as a professional *** continues by looking at advances in military hygiene in the two world *** was replaced by the term‘health’in the 1950 s as the collective noun used by professionals working in this *** unity split when the professions of occupational medicine and public health established separate faculties and training ***,the health issues for the armed forces remain fundamentally *** forward,the term‘wellbeing’is helping to refresh the close relationships between executives,their medical advisers and those within the population of health professions charged with keeping the British Army *** core theme is the collaborations between civil society,executive leadership and medical services in maximizing the health of the military population from recruitment through to life as a veteran.

Oesophageal junctional adenocarcinoma is a challeng-ing and increasingly common disease. Optimisation ofpre-operative staging and consolidation of surgery inlarge volume centres have improved outcomes, howev-er the preferred adjunctive treatment approach remainsa matter of debate. This review examines the benefitsof neoadjuvant, peri-operative, and post-operative che-motherapy and chemoradiotherapy in this setting in anattempt to reach an evidence based conclusion. Recentfindings relating to the molecular characterisation ofoesophagogastric cancer and their impact on therapeu-tics are explored, in addition to the potential benefitsof fluoro-deoxyglucose positron emission tomography(FDG-PET) directed therapy. Finally, efforts to decreasethe incidence of junctional adenocarcinoma using earlyintervention in Barrett's oesophagus are discussed,including the roles of screening, endoscopic mucosalresection, ablative therapies and chemoprevention.

The best known cases of cell autotomy are the formation of erythrocytes and thrombocytes(platelets) from progenitor cells that reside in special niches. Recently, autotomy of stem cells and its enigmatic interaction with the niche has been reported from male germline stem cells(GScs) in several insect species. First described in lepidopterans, the silkmoth, followed by the gipsy moth and consecutively in hemipterans, foremost the milkweed bug. In both, moths and the milkweed bug, GScs form finger-like projections toward the niche, the apical cells(homologs of the hub cells in Drosophila). Whereas in the milkweed bug the projection terminals remain at the surface of the niche cells, in the gipsy moth they protrude deeply into the singular niche cell. In both cases, the projections undergo serial retrograde fragmentation with progressing signs of autophagy. In the gipsy moth, the autotomized vesicles are phagocytized and digested by the niche cell. In the milkweed bug the autotomized vesicles accumulate at the niche surface and disintegrate. Autotomy and sprouting of new projections appears to occur continuously. The significance of the GSc-niche interactions, however, remains enigmatic. Our concept on the signaling relationship between stem cell-niche in general and GSc and niche(hub cells and cyst stem cells) in particular has been greatly shaped by Drosophila melanogaster. In comparing the interactions of GScs with their niche in Drosophila with those in species exhibiting GSc autotomy it is obvious that additional or alternative modes of stem cell-niche communication exist. Thus, essential signaling pathways, including niche-stem cell adhesion(E-cadherin) and the direction of asymmetrical GSc division- as they were found in Drosophila- can hardly be translated into the systems where GSc autotomywas reported. Background: Prosopis species have been introduced to many areas outside their native range to provide benefits to local communities. Several Prosopis species and their hybrids(hereafter "mesquite") have, however, become naturalised and invasive and now generate substantial costs. Management options are limited because of the complex conflicts of interest regarding benefits and costs. Management policies and strategies must take account of such conflicts, but further insights are needed on the dimensions of uses and impacts before such information can be usefully applied. current policy in South Africa allows for the growth and use of mesquite in one province,but not in others where its control is mandatory. We report on a study to quantify the direct use and perceptions of non-timber forest products(NTFPs) from mesquite and native trees in South ***: Semi-structures household interviews were conducted with various stakeholder groups to identify what tree products are used, to ascertain amounts used as well as to gauge perceptions of natural resource use between different tree species and use over ***: The direct household use value of native trees was higher than that of mesquite, and local stakeholders attached greater value to products from native trees than from mesquite. Therefore, native trees are and will still be preferentially harvested, and mesquite is unlikely to offer protection to native species by providing an alternative source of products. Mesquite pods do, however, provide valuable additional resources(fodder and medicinal products). The use of both native trees and mesquite is decreasing as the incomes of poorer households rise and as alternative energy sources become available. Forty-four different animal biles obtained from both invertebrates and vertebrates (including human bile) have been used for centuries for a host of maladies in traditional chinese medicine (TcM) beginning with dog, ox and common carp biles approximately in the Zhou dynasty (c. 1046-256 BcE). Overall, different animal biles were prescribed principally for the treatment of liver, biliary, skin (including burns), gynecological and heart diseases, as well as diseases of the eyes, ears, nose, mouth and throat. We present an informed opinion of the clinical efficacy of the medicinal uses of the different animal biles based on their presently known principal chemical components which are mostly steroidal detergent-like molecules and the membrane lipids such as unesterified cholesterol and mixed phosphatidylcholines and sometimes sphingomyelin, as well as containing lipopigments derived from heme principally bilirubin glucuronides. All of the available information on the ethnopharmacological uses of biles in TcM were collated from the rich collection of ancient chinese books on materia medica held in libraries in china and United States and the composition of various animal biles was based on rigorous separatory and advanced chemical identification techniques published since the mid-20th century collected via library (Harvard’s countway Library) and electronic searches (PubMed and Google Scholar). Our analysis of ethnomedical data and information on biliary chemistry shows that specific bile salts, as well as the common bile pigment bilirubin and its glucuronides plus the minor components of bile such as vitamins A, D, E, K, as well as melatonin (N-acetyl-5-methoxytryptamine) are salutary in improving liver function, dissolving gallstones, inhibiting bacterial and viral multiplication, promoting cardiac chronotropsim, as well as exhibiting anti-inflammatory, anti-pyretic, anti-oxidant, sedative, anti-convulsive, anti-allergic, anti-congestive, anti-diabe
