Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tNGcs). However, the use of tNGcs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tNGc would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tNGcs and compared the various features of these improved silk^(+) tNGcs with the tubes without holes(silk^(–) tNGcs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tNGcs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tNGcs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tNGcs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tNGcs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. Objectives:To provide concise information to clinicians on how to better interpret multiparametric magnetic resonance imaging for prostate cancer risk *** and methods:We analyzed 2 separate *** patients receiving a Prostate Imaging Reporting and Data System version 2(PI-RADSv2)score of 1 or 2,we reviewed the charts of 226 patients who underwent multiparametric magnetic resonance imaging of the prostate ordered from 2015 to 2017 to determine who developed clinically significant prostate cancer(csPca)by August 27,*** patients receiving PI-RADSv2 a score of 3,4,or 5,we reviewed the results of 733 fusion biopsies on solitary *** analysis was used to further determine risk factors for ***:Ten percent of men with PI-RADSv2 a score of 1 eventually developed *** percent with a score of 2 were eventually diagnosed with *** 1 of 226 with a score of 1 or 2 developed *** PI-RADSv2 scores of 3,4,and 5,csPca was detected in 16%,45%,and 67%of fusion *** zone(PZ)PI-RADSv2 score of 4 or 5 and prostate-specific antigen density(PSA-D)were significant predictors of csPca on multivariable *** a PSA-D×PI-RADSv2 score of≤0.39,we identified 38%of men with a PI-RADSv2 score of 3 in the PZ or 3,4,or 5 in the transition zone who could have avoided a benign ***:The vast majority of patients with PI-RADSv2 scores 1 and 2 can be safely monitored with close *** with Pl-RADSv2 scores of 4 and 5 in the PZ should be *** zone lesions with a Pl-RADSv2 score of 3 and transition zone lesions with 3,4,or 5 can be risk-stratified using the PSA-D x PI-RADSv2 score to determine who may safely avoid a biopsy and who should proceed to fusion biopsy.

Acute and chronic respiratory illnesses cause widespread morbidity and mortality,and this class of illness now includes the novel coronavirus severe acute respiratory syndrome that is causing coronavirus disease-2019(cOVID-19).The world is experiencing a major demographic shift toward an older,obese,and physically inactive *** factor assessments based on pandemic data indicate that those at higher risk for severe illness from cOVID-19 include older males,and people of all ages with obesity and related comorbidities such as hypertension and type 2 *** in and of itself leads to negative changes in innate and adaptive immunity,a process termed *** causes systemic inflammation and adversely impacts immune function and host defense in a way that patterns *** primary prevention strategies to reduce the risk for cOVID-19 at both the community and individual levels include mitigation activities and the adoption of lifestyle practices consistent with good immune *** and human studies support the idea that,in contrast to high exercise workloads,regular moderate-intensity physical activity improves immunosurveillance against pathogens and reduces morbidity and mortality from viral infection and respiratory illnesses including the common cold,pneumonia,and *** odds are high that infectious disease pandemics spawned by novel pathogens will continue to inflict morbidity and mortality as the world’s population becomes older and more ***-19 is indeed a wake-up call,a tocsin,to the world that primary prevention countermeasures focused on health behaviors and hygiene demand our full attention and support.

A quasi two-dimensional two-phase flow cylinder model of slug motion in a voided line developed in this paper can predict reasonably the change of flow pattern of the slug, air entrainment, holdup and the distribution of axial velocity. However, if using the theory of transfer momentum to estimate the pressure-time history of slug at the elbow, the result of calculation is not good in agreement with that of experiment. Analysis of the experiment results and the estimations of using the theory of transient two-phase flow indicate that an evident water hammer response occurs in the slug, when the high-speed slug impacts on the elbow, and the peaks and waveform of pressure-time history are dependent mainly on the propagation of transient two-phase flow in the slug.

The widely promoted $1000 genome will be a reality with attendant changes for healthcare, including personalized medicine. What is less appreciated is the explosive demands on computation, both for cPU cycles and storage as well as the need for new computational methods. In this article we will survey some of these developments and demands.

I was disappointed to read about a device that was not successful in producing reliable vas occlusion in Amory et al. ＇s report in this issue of the journal [1]. For those who are unfamiliar with vasectomy terminology, it is important to understand that a vasectomy procedure has three major aspects：（1） anesthesia; （2） accessing or approaching the vas （that is, the method used to isolate a loop of the vas）; and （3） occluding the vas.
