Diabetic eye disease is strongly associated with the development of diabetic foot ulcers(DFUs).DFUs are a common and significant complication of diabetes mellitus(DM)that arise from a combination of micro-and macrovascular *** and associated metabolic dysfunction in DM lead to impaired wound healing,immune dysregulation,peripheral vascular disease,and diabetic neuropathy that predisposes the lower extremities to repetitive injury and progressive tissue damage that may ultimately necessitate *** retinopathy(DR)is caused by cumulative damage to the retinal microvasculature from hyperglycemia and other diabetes-associated *** severity of DR is closely associated with the development of DFUs and the need for lower extremity revascularization procedures and/or *** the lower extremity,the eye may also suffer end-organ damage from macrovascular compromise in the form of cranial neuropathies that impair its motility,cause optic neuropathy,or result in partial or complete ***,poor perfusion of the eye can cause ischemic retinopathy leading to the development of proliferative diabetic retinopathy or neovascular glaucoma,both serious,visionthreatening ***,diabetic corneal ulcers and DFUs share many aspects of impaired wound healing resulting from neurovascular,sensory,and immunologic ***,alterations in serum biomarkers,such as hemoglobin A1c,ceruloplasmin,creatinine,low-density lipoprotein,and highdensity lipoprotein,are associated with both DR and *** these parameters can aid in prognosticating long-term outcomes and shed light on shared pathogenic mechanisms that lead to end-organ *** frequent cooccurrence of diabetic eye and foot problems mandate that patients affected by either condition undergo reciprocal comprehensive eye and foot evaluations in addition to optimizing diabetes management.

Acute upper respiratory tract infections, i.e. common colds cause multiple symptoms and affect adults on average 2 - 4 times a year and children 6 - 8 times per year. Thousands of non-prescription, over-the-counter products are used by sufferers, aimed at relieving the various both er some symptoms such as cough, nasal congestion, sore throat, etc. This study evaluated real-world effectiveness of Wick MediNait or Wick DayMed medicines alone and together in a combination as a day and night treatment regimen. Adult cold sufferers were recruited in pharmacy by pharmacy staff following self-purchase of either Wick MediNait, Wick DayMed capsules, or the combination of DayMed and MediNait. Participants completed online questionnaires before and after product use (evening of the first day for DayMed and the morning after using MediNait at bedtime). For the primary endpoint “Wick MediNait was effective at relieving my symptoms”, there was a statistically significant difference (p < 0.0001) in the users who Strongly Agreed or Agreed vs. Neither Agreed or Disagreed, Disagreed or Strongly Disagreed. Further, all symptoms evaluated (cough, runny nose, nasal congestion, fever, headache, sore throat, muscle aches and pains, sneezing) were statistically improved after MediNait and DayMed alone and when both were used, and users were satisfied with both treatments. All treatments were well tolerated with minimal adverse event reporting. The diurnal use of DayMed capsules combined with the nocturnal use of MediNait allows cold sufferers to efficiently self-manage their symptom relief throughout the day and night.

Vicks VapoRub® (VVR) is a pharmaceutical preparation containing a combination of levomenthol, eucalyptus oil, turpentine oil and camphor as active ingredients, and thymol, cedarwood oil, and white soft paraffin as excipients. VVR is a petrolatum-based ointment to be either applied topically to the chest, throat, and back or added to hot water and the aromatic vapours inhaled. When used topically, the actives are evaporated by body temperature and inspired. The main therapeutic effects are the feeling of relief from nasal congestion and relief from cough. These were primarily experienced by patients as the trigeminal and olfactory impact of the aromatics and were hypothesized to be experienced within minutes. This was a randomized, single-(Investigator) blind, controlled, 2-arm (VVR vs. petrolatum), parallel design pilot study in 50 otherwise healthy adult patients suffering from common cold and experiencing nasal congestion. Speed to detection of a sensation of nasal cooling and nasal decongestion was assessed following application of the recommended amount of product. The time to first experience of a sensation of nasal cooling was significantly (p ® as directed experienced significant differences from control for sensation of nasal cooling in 12 seconds and the sensation of nasal decongestion in 62 seconds.

Common cold sufferers frequently report sleep disruption during the symptomatic period of infections. We examined the effects of treatment with a topical aromatic pharmaceutical ointment (Vicks VapoRub?), on associated sleep disturbances. The effects of Vicks VapoRub? versus placebo (petrolatum ointment) on subjective and objective measured sleep parameters were assessed in an exploratory study of 100 common cold patients, in a randomized, single blind, controlled, two-arm, parallel design study. The primary efficacy variable was subjective sleep quality measured with the SQSQ (Subjective Quality of Sleep Questionnaire). Additional measures included, ease of falling asleep and depth of sleep (measured with a post-sleep Visual Analog Scale), total sleep time, sleep onset latency, activity score, percentage of sleep, sleep efficiency (measured with actigraphy and SQSQ) and sleep quality index measured with a modified Karolinska Sleep Diary (KSD). The primary endpoint, “How was the quality of your sleep last night?” showed a statistically significant difference in change from baseline in favour of VapoRub treatment (p = 0.0392) versus placebo. Positive effects of VapoRub versus placebo were also observed for “How refreshed did you feel upon waking up?” (p = 0.0122) (SQSQ), “Did you get enough sleep?” (p = 0.0036) (KSD), “How was it to get up?” (p = 0.0120) (KSD) and “Do you feel well-rested?” (p = 0.0125) (KSD). No statistically significant changes from baseline versus placebo were detected in the Actiwatch endpoints. Vicks VapoRub? when applied before retiring to bed can reduce subjective sleep disturbances during a common cold. The results of this exploratory study support the belief among patients that the use of VapoRub improves subjective sleep quality during common cold which was associated with more refreshing sleep.

AIM To determine the radiation dose and image quality in coronary computed tomography angiography(CCTA)using state-of-the-art dose reduction methods in unselected"real world"*** In this single-centre study,consecutive patients in sinus rhythm underwent CCTA for suspected coronary artery disease(CAD)using a 320-row detector CT *** patients underwent the standard CT acquisition protocol at our institute(Morriston Hospital)a combination of dose saving advances including prospective electrocardiogram-gating,automated tube current modulation,tube voltage reduction,heart rate reduction,and the most recent novel adaptive iterative dose reconstruction 3D(AIDR3D)*** cohort comprised real-world patients for routine CCTA who were not selected on age,body mass index,or heart *** image quality was graded on a 4-point scale(4=excellent,1=non-diagnostic).RESULTS A total of 543 patients were included in the study with a mean body weight of 81±18 kg and a pre-scan mean heart rate of 70±11 beats per minute(bpm).When indicated,patients received rate-limiting medication with an oral beta-blocker followed by additional intravenous beta-blocker to achieve a heart rate below 65 *** median effective radiation dose was 0.88 mSv(IQR,0.6-1.4 mSv)derived from a Dose Length Product of61.45 ***(IQR,42.86-100.00 ***).This also includes what we believe to be the lowest ever-reported radiation dose for a routine clinical CCTA(0.18 mSv).The mean image quality(SD)was 3.65±0.61,with a subjective image quality score of 3("good")or above for 93%of patient *** Combining a low-dose scan protocol and AIDR3D with a 320-detector row CT scanner can provide high quality images at exceptionally low radiation dose in unselected patients being investigated for CAD.

Agriculture is essential for providing food and maintaining food security while concurrently delivering multiple other ecosystem services. However,agricultural systems are generally a net source of greenhouse gases and ammonia. They, therefore, need to substantively contribute to climate change mitigation and net zero ambitions. It is widely acknowledged that there is a need to further reduce and mitigate emissions across sectors, including agriculture to address the climate emergency and emissions gap. This discussion paper outlines a collation of opinions from a range of experts within agricultural research and advisory roles following a greenhouse gas and ammonia emission mitigation workshop held in the UK in March 2022. The meeting identified the top mitigation priorities within the UK's agricultural sector to achieve reductions in greenhouse gases and ammonia that are compatible with policy targets. In addition, experts provided an overview of what they believe are the key knowledge gaps, future opportunities and cobenefits to mitigation practices as well as indicating the potential barriers to uptake for mitigation scenarios discussed.

背景：对于非ST段抬高急性冠脉综合征（acute coronary syndromes，ACS）患者而言，与单独应用普通肝素相比，依诺肝素或者血小板糖蛋白Ⅱb／Ⅲa受体拮抗剂替罗非班与普通肝素联合应用都显示出较好的疗效。目前，尚不清楚依诺肝素和替罗非班联合应用是否像普通肝素和替罗非班标准联合方案一样安全有效。目的：在非ST段抬高的ACS患者中评价依诺肝素和替罗非班联合疗法与普通肝素和替罗非班联合方案的疗效及安全性。设计、地点及参试者：国际前瞻性、开标（open—label）、随机、非劣势（noninferiority）试验。在接受替罗非班和阿斯匹林治疗的非ST段抬高ACS患者中分别给予依诺肝素1mg／kg（n=2026）每12小时一次或经体重校正的静脉普通肝素（n=1961），然后进行比较。这项A to Z试验的A阶段试验是于1999年12月至2002年5月进行的。主要观察指标：意向治疗人群7天时死亡、再发心肌梗死和难治性缺血的发生情况，该意向治疗人群是根据优效性和非劣势原则确定的。应用“心肌梗死溶栓治疗试验”（Thrombolysis in Myocardial Infarction，TIMI）中之出血分级系统，通过监测出血发生率判定用药安全性。结果：在治疗第7天时，随机分配至依诺肝素组的2018例患者中有169例（8．4％）发生死亡、心肌梗死或者难治性缺血，而普通肝素组的1952例患者中有184例（9．4％）发生上述事件（风险比[hazard ratio，HR]为0．88，95％可信区间[confidence interval，CI]为0．71～1．08）。该结果符合预先设定的非劣势标准。除死亡外，所有一级和二级复合终点事件分析结果均提示，依诺肝素更为有益。死亡仅见于1％的患者（依诺肝素组23例，普通肝素组17例）。任何TIMI分级的出血发生率都很低（依诺肝素组为3．0％，普通肝素组为2．2％；P=0．13）。最差情况分析（合并了两种独立的出血判定策略）表明，采用依诺肝素每治疗200例患者就会发生一次TIMI严重出血。结论：对于接受替罗非班和阿斯匹林治疗的非ST段抬高ACS患者而言，依诺肝素是除普通普通肝素之外的另一种适当选择。依诺肝素可使一级终点相对减少12％，绝对减少1％，符合非劣势标准。该结果与以往未联用糖蛋白Ⅱb／Ⅲa受体拮抗剂的试验一致。
