Successful Abdominal organ Donation after Brain Death in a Patient with a Biventricular Assist Device: Extending Extended Criteria

Few studies address the potential for donation after brain death (DBD) in the limited population of patients with ongoing mechanical circulatory support (MCS). A case study was conducted reviewing available records of both donor and recipient, and available literature. The donor was a young female with an acute myocardial infarction precipitating emergent off-pump 2-vessel bypass graft complicated by profound cardiogenic shock refractory to inotropes and intra-aortic balloon pump. A heparin drip was started following percutaneous placement of a left ventricular-assist device (TandemHeart?) which improved her hemodynamics to stabilize for transfer. She ultimately required surgical placement of biventricular assist device (CentraMag?) to normalize hemodynamics. Two days post-operatively, she developed a cerebellar hemorrhage and was declared brain dead. Pre-donation blood chemistry showed adequate end-organ function. Both kidneys were placed locally. The liver was rejected for two regional status 1 patients and by all other local centers. We accepted the liver for a patient with polycystic liver disease with a MELD exception score of 20. The recipient is now 4 years post-transplant with excellent graft function. Extending donor criteria to include MCS patients can result in successful transplantation and should be considered in selected circumstances once satisfactory donor end-organ function is established.

Detecting the origins of Moisture over Southeast China:Seasonal Variation and Heavy Rainfall

To examine the ability of the Hybrid Single-Particle Lagrangian Integrated Trajectory （HYSPLIT） model to detect the origins and paths of moisture supplied to Southeast China, trajectories of air particles released over Southeast China were traced backward during 1 April 2012 to 31 March 2013 and three typical regional persistent heavy rainfall events. The HYSPLIT model provides more insightful information than water vapor flux. Analysis of the specific humidity along the trajectories revealed the origins of moisture and their contributions to the moisture supply in Southeast China. In the boreal summer half year, four key moisture transport paths from the eastern Indian ocean, central Indian ocean, South China Sea （SCS）, and western North Pacific （WNP） contribute 10%, 20%, 31%, and 16% of the moisture to Southeast China, respectively. In the winter half year, the contributions of the paths from the WNP and North China double. Examination of heavy rainfall events showed that under tropical storm conditions, all moisture transport routines are rotated cyclonically before reaching Southeast China. The invasion of cold air can trigger heavy rainfall in both the summer and winter half years but plays different roles： it does not contribute to the moisture supply but plays a key role in converging and uplifting the moisture in the summer half year, while it supplies a great amount of moisture in the winter half year as it absorbs abundant moisture in crossing the WNE

Barrett’s esophagus following treatment of achalasia with botulinum toxin

Achalasia is an uncommon primary motor disorder of the esophagus with an annual incidence of 0.8/100,000. Very few cases of coexistent Barrett’s esophagus (BE) and achalasia in patients without prior surgical myotomy or pneumatic dilation have been reported. We report the case of a 65 year old female who was diagnosed with achalasia in June 2002. Endoscopy at that time revealed biopsy-confirmed normal esophageal mucosa. The patient subsequently underwent two trials of botox injection with progressively worsening dysphagia. A repeat endoscopy two years later showed a short segment of salmon-colored mucosa in the distal esophagus which was biopsy-confirmed Barrett’s epithelium with no dysplasia. The patient eventually underwent laparoscopic Heller myotomy and Toupet fundoplication. Postoperatively, she recovered well and with significant alleviation of her dysphagia. This study reviews reported cases of coexistent achalasia and BE, and discusses possible etiologies of concurrent BE and achalasia, and implications for treatment.

Epidemiology and transmission of hepatitis B and C viruses in Kazakhstan

AIM: To investigate the epidemiology of hepatitis B virus (HBV) and hepatitis C virus (HCV) infection in the two major ethnic groups in Kazakhstan. METHoDS: A cross-sectional prospective study of HBV and HCV seroprevalence was performed among individuals born in Kazakhstan with no history of chronic hepatitis or liver disease. RESULTS: There were 290 volunteers (140 Russians and 150 Kazakhs) aged 10 to 64 years, males accounted for 46%. Active HBV infection (HBsAg positive) was present in 3.8%, anti-HBc in 30%. The prevalence was similar in females and males (33% vs 25%) (P = 0.18). The prevalence of anti-HBc increased from 19% in 10-29 years old volunteers to 53% in 50-years and older volunteers. The prevalence of HBV infection was higher in married than in single adults (38% vs 26%, respectively) (P = 0.2) and more common in Kazakhs (35%) than in Russians (24%) (P = 0.07). HCV infection was present in 9 subjects (3.2%), 5 of them also were positive for anti-HBc in the absence of HBsAg. CoNCLUSIoN: The frequency of active HBV infection (3.8%) coupled with a high prevalence of HBV exposure in those > 50 years of age increases with age, which suggests that horizontal transmission likely relates tothe use of contaminated needles. The low prevalence of HCV infection suggests that HBV and HCV are acquired differently in this group of subjects.

人类行为引起的空前的全球变化挑战着社会支撑我们这颗行星保持自身优点的能力。这就要求对变化进行主动的管理以培养地球的恢复力——维持在变化的情况下对社会很重要的一些属性,和适应性——发展新的社会生态构造以便在新的条件下有效地发挥作用。北极的广大地区仍然完整地保留着原型的生态及社会过程及反馈,这为我们为变化预先做出计划提供了可能是最后一个实验的机会。如果该策略的可行性在北极可以被证实,我们对于变化的动态所得到的进一步的理解就可以实施到那些被人类较大地改变了的地区中。现在执行管理北极变化的政策的条件可能很理想,因为近来的研究提供了极其重要的科学知识,合适的国际机制也已各就各位,而且,如果北极地区的国家选择这样做的话,他们具备实行必要改变的财力。
