Quantitative and qualitative subgenomic RNA profiles of SARS-CoV-2 in respiratory samples: A comparison between Omicron BA.2 and non-VOC-D614G

The SARS-CoV-2 Omicron variants are notorious for their transmissibility,but little is known about their subgenomic RNA(sgRNA)*** study applied RNA-seq to delineate the quantitative and qualitative profiles of canonical sgRNA of 118 respiratory samples collected from patients infected with Omicron BA.2 and compared with 338 patients infected with non-variant of concern(non-VOC)-D614G.A unique characteristic profile depicted by the relative abundance of 9 canonical sgRNAs was reproduced by both BA.2 and non-VOCD614G regardless of host gender,age and presence of ***,such profile was lost in samples with low viral load,suggesting a potential application of sgRNA pattern to indicate viral activity of individual patient at a specific time point.A characteristic qualitative profile of canonical sgRNAs was also reproduced by both BA.2 and *** presence of a full set of canonical sgRNAs carried a coherent correlation with crude viral load(AUC¼0.91,95%CI 0.88–0.94),and sgRNA ORF7b was identified to be the best surrogate marker allowing feasible routine application in characterizing the infection status of individual *** potentials in using sgRNA as a target for vaccine and antiviral development are worth pursuing.

Iron deficiency anemia(IDA) is associated with a number of pathological gastrointestinal conditions other than inflammatory bowel disease, and also with liver disorders. Different factors such as chronic bleeding, malabsorption and inflammation may contribute to IDA. Although patients with symptoms of anemia are frequently referred to gastroenterologists, the approach to diagnosis and selection of treatment as well as follow-up measures is not standardized and suboptimal. Iron deficiency, even without anemia, can substantially impact physical and cognitive function and reduce quality of life. Therefore, regular iron status assessment and awareness of the clinical consequences of impaired iron status are critical. While the range of options for treatment of IDA is increasing due to the availability of effective and well-tolerated parenteral iron preparations, a comprehensive overview of IDA and its therapy in patients with gastrointestinal conditions is currently lacking. Furthermore, definitions and assessment of iron status lack harmonization and there is a paucity of expert guidelines on this topic. This review summarizes current thinking concerning IDA as a common co-morbidity in specific gastrointestinal and liver disorders, and thus encourages a more unified treatment approach to anemia and iron deficiency, while offering gastroenterologists guidance on treatment options for IDA in everyday clinical practice.

Background:To evaluate the effects on near and intermediate visual performance after bilateral Laser Anterior Ciliary Excision(LaserACE)***:LaserACE surgery was performed using the VisioLite 2.94μm erbium:yttrium-aluminum-garnet(Er:YAG)ophthalmic laser system in 4 oblique quadrants on the sclera over the ciliary muscle in 3 critical zones of physiological importance(over the ciliary muscles and posterior zonules)with the aim to improve natural dynamic accommodative *** was performed on 26 patients(52 eyes).Outcomes were analyzed using visual acuity testing,Randot stereopsis,and the CatQuest 9SF patient ***:Binocular uncorrected near visual acuity(UNVA)improved from+0.20±0.16 logMAR preoperatively,to+0.12±0.14 logMAR at 24 months postoperatively(p=0.0014).There was no statistically significant loss in distance corrected near visual acuity(DCNVA).Binocular DCNVA improved from+0.21±0.17 logMAR preoperatively,to+0.11±0.12 logMAR at 24 months postoperatively(p=0.00026).Stereoacuity improved from 74.8±30.3 s of arc preoperatively,to 58.8±22.9 s of arc at 24 months postoperatively(p=0.012).There were no complications such as persistent hypotony,cystoid macular edema,or loss of best-corrected visual acuity(BCVA).Patients surveyed indicated reduced difficulty in areas of near vision,and were overall satisfied with the ***:Preliminary results of the LaserACE procedure show promising results for restoring visual performance for near and intermediate visual tasks without compromising distance vision and without touching the visual *** visual function and visual acuity improvements had clinical *** satisfaction was high postoperatively and sustained over 24 months.

Canadian Health Policies and Their Implications for Current Healthcare Reforms in China

Health policies have played significant roles in attaining good achievements in population health improvement. This paper chronologically reviews and analyzes health-rehted parliamentary actions and law enactments in Canada. It indicates that the salient feature of the Canadian health system is that it is pubhcly financed with a single-payer, but privately run. The Canadian health system is based upon five founding principles： care must be universal （covering everyone）, portable （ among all provinces）, comprehensive （ covering all medically necessary services ）, accessible （without barriers to access hke user fees ）, and pubhdy administered（ by the government）. This paper pays special attention to what and how China can learn from Canada and what should be avoided against failures of Canadian health system, and how it might provide a new health policy base for China＇s current health reform.

Suppression of Tumorigenicity 2 （ST2） is a member of the interleukin （IL）-1 receptor family The ST2 receptor exists in two isoforms - ST2 ligand （ST2L） and soluble ST2 （sST2）.ST2L is a membrane receptor and sST2 is a trun- cated receptor which is soluble in the blood, allowing it to be detected in serum. IL-33 is a member of the IL-1 family of ligand and is the fimctional ligand of ST2L receptor. It binds to the ST2L, thereby mediating its immune function.

ackground:Uncorrected refractive error is a major cause of vision impairment worldwide and its increasing prevalent necessitates effective screening and management ***,deep learning,a subset of Artificial Intelligence,has significantly advanced ophthalmological diagnostics by automating tasks that required extensive clinical *** recent studies have investigated the use of deep learning models for refractive power detection through various imaging techniques,a comprehensive systematic review on this topic is has yet be *** review aims to summarise and evaluate the performance of ocular image-based deep learning models in predicting refractive *** text:We search on three databases(PubMed,Scopus,Web of Science)up till June 2023,focusing on deep learning applications in detecting refractive error from ocular *** included studies that had reported refractive error outcomes,regardless of publication *** systematically extracted and evaluated the continuous outcomes(sphere,SE,cylinder)and categorical outcomes(myopia),ground truth measurements,ocular imaging modalities,deep learning models,and performance metrics,adhering to PRISMA *** studies were identified and categorised into three groups:retinal photo-based(n=5),OCT-based(n=1),and external ocular photo-based(n=3).For high myopia prediction,retinal photo-based models achieved AUC between 0.91 and 0.98,sensitivity levels between 85.10%and 97.80%,and specificity levels between 76.40%and 94.50%.For continuous prediction,retinal photo-based models reported MAE ranging from 0.31D to 2.19D,and R^(2) between 0.05 and *** OCT-based model achieved an AUC of 0.79–0.81,sensitivity of 82.30%and 87.20%and specificity of 61.70%–68.90%.For external ocular photo-based models,the AUC ranged from 0.91 to 0.99,sensitivity of 81.13%–84.00%and specificity of 74.00%–86.42%,MAE ranges from 0.07D to 0.18D and accuracy ranges from 81.60%to 96.70%.The reported papers colle

目的从医院领导角度了解并比较我国肿瘤专科医院和三级医院的缓和医疗服务现况。方法对肿瘤医院(206家)和三级医院(443家)领导进行缓和医疗服务的电子问卷调查并比较两类医院是否存在差异,调查内容包括服务形式和内容、未来计划以及阻碍学科发展的因素等。采用SPSS 16.0软件进行数据分析。根据数据类型,组间比较分别采用t检验、Fisher确切概率检验及χ2检验。结果全国共88家(42.72%)肿瘤医院和174家(39.28%)三级医院领导完成调研,肿瘤医院参加调研的比例明显高于三级医院(P<0.01)。过去10年,78.74%(137/174)的三级医院和85.23%(75/88)肿瘤医院都能提供缓和医疗服务;目前仍能提供这一服务的比例分别为87.59%(120/137)和90.67%(68/75),差异均无统计学意义。48.53%(33/68)的肿瘤医院以及40.83%(49/120)的三级医院有缓和医疗专科医师(P=0.31)。肿瘤医院的专科门诊率为27.94%(19/68),三级医院为17.50%(21/120),差异无统计学意义;肿瘤医院在会诊[29家(42.65%)和33家(27.50%)]、住院床位[28家(41.18%)和32家(26.67%)]、专属安宁疗护机构[20家(29.41%)和20家(16.67%)]情况方面,均较三级医院有显著优势(P<0.05)。在缓和医疗服务和镇痛治疗上,两类医院都比5年前有显著进步(P<0.01);对缓和医疗与肿瘤治疗整合的观点都很支持,肿瘤医院均值为(7.06±3.19)分,三级医院为(6.29±3.64)分,差异无统计学意义;肿瘤医院对增加科研项目的赞同度显著高于三级医院[(7.24±3.12)和(5.91±3.71)分,P<0.01]。阻碍缓和医疗服务的前三位因素依次为缺乏报销支持、专业人员和预算。结论尽管全国85%以上的肿瘤医院和三级医院提供服务,但缓和医疗专科医师的比例不足50%。两类医院的缓和医疗及镇痛治疗都有显著进步,而肿瘤医院在专科会诊、住院床位、安宁疗护机构上比三级医院有优势。缺乏报销支持、专业人才和预算为阻碍缓和医疗服务的三大因素。
