Background:Ocular surface disease in glaucoma patients is a significant ocular co-morbidity that can affect 40%to 59%of these patients *** current study was aimed at evaluating the potential clinical benefit of an intense pulsed light(IPL)-based treatment in glaucomatous patients with ocular surface disease due to prolonged hypotensive eyedrop *** our knowledge,this is the first series analyzing the therapeutic effect of this treatment option in this type of ***:This non-comparative prospective case series study enrolled a total of 30 glaucoma patients ranging in age from 57 to 94 years old and treated with hypotensive eyedrops for years with dry eye *** patients received four sessions of IPL treatment using the Optima IPL system(Lumenis,Yokneam,Israel)adjusted to the official optimized Lumenis *** in symptomatology,corneal staining,conjunctival hyperemia,non-invasive break-up time(NIBUT),tear osmolarity,tear meniscus height(TMH),meiboscore and meibomian gland expressibility was analyzed after ***:Statistically significant reductions were observed after IPL treatment in the symptomatology scores measured with different questionnaires[ocular surface disease index(OSDI),standard patient evaluation of eye dryness(SPEED)and symptom assessment questionnaire in dry eye(SANDE)]as well as with the visual analogue scale(P<0.001).Mean change in OSDI was−15.0±11.3.A significant reduction was found after treatment in the corneal staining score(P<0.001).A significant reduction was found in tear film meniscus height(P=0.012),as well as in tear film osmolarity(P=0.001).A significant reduction was also found in meibomian gland expressibility(P=0.003),changing the percentage of grade 3 eyes from 44.4%before IPL to 17.2%after ***:IPL therapy combined with meibomian gland expression(MGX)seems to be an effective option to improve symptomatology in glaucomatous patients with ocular surface diseas

Rates of non-alcoholic fatty liver disease(NAFLD) are increasing worldwide in tandem with the metabolic syndrome, with the progressive form of disease, nonalcoholic steatohepatitis(NASH) likely to become the most common cause of end stage liver disease in the not too distant future. Lifestyle modification and weight loss remain the main focus of management in NAFLD and NASH, however, there has been growing interest in the benefit of specific foods and dietary components on disease progression, with some foods showing protective properties. This article provides an overview of the foods that show the most promise and their potential benefits in NAFLD/NASH, specifically; oily fish/fish oil, coffee, nuts, tea, red wine, avocado and olive oil. Furthermore, it summarises results from animal and human trials and highlights potential areas for future research.

AIM： To assess if the included vascular endothelial growth factor （VEGF） polymorphisms rs3025035, rs3025021 and rs2010963 are associated to proliferative retinopathy in a Mexican population with type 2 diabetes mellitus （T2DM）. METHODS： A case-control study was conducted in adult individuals with T2DM associated to proliferative retinopathy or non-proliferative retinopathy from Oct. 2014 to Jun. 2015 from the Retina Department of the Asociation to Prevent Blindness in Mexico. The selected patients were adults with a diagnosis of T2DM ≥5y. All subjects had a comprehensive ocular examination and the classification of the retinopathy severity was made considering the Early Treatment Diabetic Retinopathy Study （ETDRS） standardization protocols. Genomic DNA was extracted from whole fresh blood. All samples were genotyped by qPCR for selected VEGF polymorphisms. Hardy-Weinberg equilibrium was calculated by comparing Chi-square values between the expected and the observed values for genotype ***： In total 142 individuals were enrolled, 71 individuals with T2DM and associated proliferative retinopathy and 71 individuals with non-proliferative retinopathy. One-sided Fisher’s exact test was performed for rs3025021 [OR （95% CI）=0.44（0.08-2.2）; P=0.25] and rs2010963 [OR （95% CI）=0.63（0.25-1.6）; P=0.23]. The minor allelic frequencies obtained were 26% for rs3025021, 10% for rs3025035 and 61% for rs2010963. Effect of bone loss in anterior shoulder instability

Anterior shoulder instability with bone loss can be a difficult problem to treat. It usually involves a component of either glenoid deficiency or a Hill-Sachs lesion. Recent data shows that soft tissue procedures alone are typically not adequate to provide stability to the shoulder. As such, numerous surgical procedures have been described to directly address these bony deficits. For glenoid defects, coracoid transfer and iliac crest bone block procedures are popular and effective. For humeral head defects, both remplissage and osteochondral allografts have decreased the rates ofrecurrent instability. Our review provides an overview of current literature addressing these treatment options and others for addressing bone loss complicating anterior glenohumeral instability.

How to improve colon cancer screening rates

Colorectal carcinoma is a common cause of death throughout the world and may be prevented by routine control, which can detect precancerous neoplasms and early cancers before they undergo malignant transformation or metastasis. Three strategies may improve colon cancer screening rates: convince the population about the importance of undergoing a screening test; achieve higher efficacy in standard screening tests and make them more available to the community and develop new more sensitive and efficacious screening methods and make them available as routine tests. In this light, the present study seeks to review these three means through which to increase colon cancer screening rates.

Postural control quantification in minimally and moderately impaired persons with multiple sclerosis:The reliability of a posturographic test and its relationships with functional ability

Background:Postural control has been associated with the functional impairment in persons with multiple sclerosis(pwMS).However,there is a need for reliable methods to assess postural control in early stages of the disease,when subtle changes can be difficult to *** aims of this study were to assess the absolute and relative reliability of a standing and a sitting posturographic protocol in minimally(Expanded Disability Status Scale≤2)and moderately(2.5≤Expanded Disability Status Scale≤4)impaired pwMS,and to analyze relationships among postural control and functional mobility and gait ***:To assess postural control in an upright stance,14 minimally and 16 moderately impaired pwMS performed six 70 s trials in tandem stance,3 with their weaker leg behind(TSWL)and 3 with their stronger leg behind(TSSL).Additionally,participants completed five 70 s trials using an unstable sitting protocol(US)to assess trunk *** mean radial errors of TSWL,TSSL,and US trials were calculated as postural control ***,participants performed the Timed Up and Go test(TUG)and the Timed 25-foot Walk test(T25FW)to measure their functional mobility and gait speed,*** was evaluated using the intraclass correlation coefficient(ICC3,1)and the standard error of measurement(SEM).Analyses of variances were carried out to assess between-group ***’g index(dg)was used to estimate the effect size of *** correlation analyses(r)were performed to examine the relationships among the postural control and the functional ***:Posturographic tests showed a high reliability in both minimally(0.87≤ICC≤0.92;9.32%≤SEM≤11.76%)and moderately(0.80≤ICC≤0.92;10.33%≤SEM≤15.33%)impaired ***,T25FW and TUG displayed a high consistency in minimally(0.89≤ICC≤0.94;3.43%≤SEM≤5.17%)and moderately(0.85≤ICC≤0.93;5.57%≤SEM≤6.56%)impaired *** impaired pwMS showed a better performance on the TUG,T25FW

Isokinetic trunk flexion-extension protocol to assess trunk muscle strength and endurance:Reliability,learning effect,and sex differences

Purpose:The purpose of this study was to examine the reliability and the learning effect of an isokinetic trunk flexion-extension protocol designed to simultaneously assess trunk muscle strength and *** addition,the effect of the participants’sex on the reliability data was ***:Fifty-seven healthy and physically active young men(n=28)and women(n=29)performed the isokinetic protocol 5 times,separated by a week between each of the first 4 sessions and by a month between the last 2 *** protocol consisted of performing 4 trials of 15 maximum flexion-extension concentric exertions at 120°/s(range of trunk motion=50°).The absolute and relative peak torque and total work were calculated to assess trunk flexion and extension *** addition,endurance ratio,modified endurance ratio,fatigue final ratio,recovery ratio,and modified recovery ratio variables were used for the assessment of trunk muscle endurance in both ***:Regarding the absolute reliability,no relevant changes were found between paired-comparison sessions for most strength and endurance variables,except for total work and relative total work variables in the flexion movement in both *** addition,the typical error of the isokinetic variables was lower than 10%in both males and females,and minimum detectable changes ranged from 7%to 20%,with a tendency to be higher in females and in endurance *** strength variables showed high-to-excellent intraclass correlation coefficients(ICCs;>0.74);however,for the endurance variables only the endurance ratio and the modified endurance ratio obtained moderate-to-high ICC values(0.57

In order to assess Stathmin as an immunohistochemical(IHC)indicator of phosphatidylinositol 3-kinase(PI3K)pathway activity in HPV-negative head&neck squamous cell carcinoma(HNSCC),we compared Stathmin IHC to expression of other pathway *** also evaluated the relationship between Stathmin IHC and the mutational status of four key pathway ***,we ascertained whether Stathmin IHC correlates with tumor grade or primary *** exists between high Stathmin expression and high pAKT1 expression,indicating a role for Stathmin IHC as a marker of pathway *** analysis did not show correlation between Stathmin IHC and mutation of the four genes *** also observed an association between high Stathmin expression and oropharyngeal primary *** results suggest utility of Stathmin IHC as an indicator of PI3K pathway activity,and thereby demonstrate potential relevance of Stathmin IHC in the context,of HNSCC.

Colorectal cancer screening and surveillance in patients with inflammatory bowel disease in 2021

The detection of dysplasia in patients with inflammatory bowel disease(IBD)continues to be important given the increased risk of colorectal cancer in this ***,in 2017,we performed a review and update of the recommendations for the management and follow-up of patients with IBD based on the clinical practice guidelines of various scientific *** present manuscript focuses on new aspects of the detection,follow-up,and management of dysplasia according to the latest studies and *** chromoendoscopy with targeted biopsy continues to be the technique of choice for the screening and detection of dysplasia in IBD,the associated difficulties mean that it is now being compared with other techniques(virtual chromoendoscopy),which yield similar results with less technical ***,the emergence of new endoscopy techniques that are still being researched but seem promising(e.g.,confocal laser endomicroscopy and full-spectrum endoscopy),together with the development of devices that improve endoscopic visualization(e.g.,Endocuff Vision),lead us to believe that these approaches can revolutionize the screening and follow-up of dysplasia in patients with ***,further studies are warranted to define the optimal follow-up strategy in this patient population.
