Risk of colon cancer in hereditary non-polyposis colorectal cancer patients as predicted by fuzzy modeling:Influence of smoking

AIM： To investigate whether a fuzzy logic model could predict colorectal cancer （CRC） risk engendered by smoking in hereditary non-polyposis colorectal cancer （HNPCC） patients. METHOdS： Three hundred and forty HNPCC mismatch repair （MMR） mutation carriers from the Creighton University Hereditary Cancer Institute Registry were selected for modeling. Age-dependent curves were generated to elucidate the joint effects between gene mutation （hMLH1 or hMSH2）, gender, and smoking status on the probability of developing CRC. RESULTS： Smoking significantly increased CRC risk in male hMSH2 mutation carriers （P 〈 0.05）. hMLH1 mutations augmented CRC risk relative to hMSH2 mutation carriers for males （P 〈 0.05）. Males had a significantly higher risk of CRC than females for hMLH1 non smokers （P 〈 0.05）, hMLH1 smokers （P 〈 0.1） and hMSH2 smokers （P 〈 0.1）. Smoking promoted CRC in a dose-dependent manner in hMSH2 in males （P 〈 0.05）. Females with hMSH2 mutations and both sexes with the hMLH1 groups only demonstrated a smoking effect after an extensive smoking history （P 〈 0.05）. CONCLUSION： CRC promotion by smoking in HNPCC patients is dependent on gene mutation, gender and age. These data demonstrate that fuzzy modeling may enable formulation of clinical risk scores, thereby allowing individualization of CRC prevention strategies.

Severe acute pancreatitis： pathogenesis, diagnosis and surgical management

BACKGROUNd： Severe acute pancreatitis is a subtype of acute pancreatitis, associated with multiple organ failure and systemic inflammatory response syndrome. In this qualitative review we looked at the principles of pathogenesis, classification and surgical management of severe acute pancreatitis. We also looked at the current shift in paradigm in the management of severe acute pancreatitis since the guideline developed by the British Society of *** SOURCES： Studies published between 1st January 1991 and 31st december 2015 were identified with Pub Med, MEdLINE, EMBASE and Google Scholar online search engines using the following Medical Subject Headings： “acute pancreatitis, necrosis, mortality, pathogenesis, incidence” and the terms “open necrosectomy and minimally invasive necrosectomy”.The National Institute of Clinical Excellence（NICE） Guidelines were also included in our study. Inclusion criteria for our clinical review included established guidelines, randomized controlled trials and non-randomized controlled trials with a follow-up duration of more than 6 ***： The incidence of severe acute pancreatitis within the UK is significantly rising and pathogenetic theories are still controversial. In developed countries, the most common cause is biliary calculi. The British Society of Gastroenterology,acknowledges the Revised Atlanta criteria for prediction of severity. detecting Inhomogeneity in daily Climate Series Using Wavelet Analysis

A wavelet method was applied to detect inhomogeneities in daily meteorological series, data which are being increasingly applied in studies of climate extremes. The wavelet method has been applied to a few well- established long-term daily temperature series back to the 18th century, which have been ＂homogenized＂ with conventional approaches. Various types of problems remaining in the series were revealed with the wavelet method. Their influences on analyses of change in climate extremes are discussed. The results have importance for understanding issues in conventional climate data processing and for development of improved methods of homogenization in order to improve analysis of climate extremes based on daily data.

Mapping informal settlement indicators using object-oriented analysis in the Middle East

Mapping informal settlements is crucial for resource and utility management and *** 2003,the UN-Habitat developed a process for mapping and monitoring urban inequality to support reporting against the sustainable development goals(SdGs).Informal settlement indicators are used as a framework to carry out image analysis,and include vegetation extent,lacunarity of housing structures/vacant land,road segment type and materials,texture measures of built-up areas,roofing extent of built-up areas and dwelling *** image analysis(OBIA)methods are recommended to identify informal *** paper documents the application of OBIA to map informal settlements,drawing on the ontology of Kohli et al.(2012)and the indicators of Owen and Wong(2013)for a Middle Eastern *** informal settlements with different land use histories were selected to represent old and new informal settlements in the city of Jeddah,Saudi *** extent was the most successful indicator detected,with 100% producer accuracy and over 84% user accuracy,followed by the road network,with 84% producer and user accuracies in older informal settlements and 73% producer accuracy and 96% user accuracy across all case *** of housing structures/vacant land was detected well in informal *** texture measure indicator was detected using GLCM_(Ent)(R)with low producer accuracy across all case *** roofing extent of the built-up area is detected with better producer and user accuracies than texture *** dwellings size indicator generally failed to distinguish formal from informal *** and formal were distinguished with an overall accuracy of 83%.This research concludes that OBIA is a useful method to map informal settlement indicators in Middle Eastern ***,a generic ruleset for mapping informal settlements remains elusive,and each indicator requires significant localised‘tuning’.
