右美托咪啶镇静对小儿心血管系统的影响

背景右美托咪啶（dexmedetomidine，DEX）可影响成年人的心率（heartrate，HR）、平均动脉压、心脏指数（cardiacindex，CI）、心搏指数（strokeindex，SI）和体循环血管阻力指数（systemicvascularresistanceindex，SVRI）。本研究旨在探讨DEX镇静对小儿心血管系统是否存在类似作用。方法给予DEX后使用影像学方法监测小儿血流动力学的变化。第一组（DEX．短时）8例患者接受DEX2μg/kg于10分钟内输注完毕且手术在10分钟内完成；第二组（DEX．长时）9例患者在首次给药（DEX2μg／kg）的基础上按需追加DEX1μg／（kg·h），手术时间超过10分钟。使用无创心排血量探头监测CI、SI、SVRI。用重复测量的方差分析比较10分钟、20分钟和出室（Aldrete评分达9）3个时间点血流动力学与基础数值的差异，数据以均数表示（可信区间95％）。结果DEX-短时组共8例，DEX．长时组共9例。在DEX-短时组，T1点的HR和C1分别降低了18．9（2．3～35．5）次／分和0．74（0．15—1．33）L／（min·m2），其余无显著变化，所有指标在R，点都恢复到基础值。在DEX．长时组，HR、CI降低持续至恢复期[分别降低了24．0（8．3—39．6）次／分、1．51（0．95～2．06）L／（min·m2）]；同时，恢复期的SI降低8．0l（1．71-14．31）ml／m2，而SVRI升高[776．0（271．9—1280．4）dyn-s／（cm5·m2）]。两组的平均动脉压均未见显著变化。结论DEX使小儿Cl降低，且有累积效应。对于接受长时间手术的患者，胍、CI持续降低到患者出室，同时伴随SI降低、SVRI升高。

Purpose: Although patient-related factors affect surgical outcomes, preoperative functional status is not measured by any cardiac risk score. Functional status can, however, be objectively measured using validated outcome tools such as the Late-Life Function and Disability Instrument (LLFDI). The purpose of this study was to determine 1) if there was a change over time in functional status, as measured by the LLFDI, in patients who underwent elective cardiac surgery, and if so, 2) what specific aspect(s) of functional status changed. Methods: A prospective longitudinal study of one year was conducted on elective cardiac surgery patients (n = 43) using the self-reported LLFDI, which measures Disability Frequency (frequency of participation in social tasks), Disability Limitation (ability to participate in social tasks) and Function Total (ease in performing routine activities). Higher scores indicate increased function and decreased disability. LLFDI scores were compared at three times (preoperative, six-week and one-year postoperative) using repeated measures ANOVA. Post hoc pairwise comparison was conducted for specific interactions. Results: Both Function Total and Disability Frequency significantly changed over time (p = 0.047 and p = 0.013, respectively). Specifically, patients’ function level was significantly higher one-year postoperative compared to preoperative (M difference = +3.48, SE = 1.48, p = 0.026). Likewise, Disability Frequency scores were significantly higher (i.e. more active) at one-year postoperative versus preoperative (M difference= +5.98, SE = 2.19, p = 0.033). Disability Limitation scores were not significantly different between any time points (p > 0.05). Conclusion: By one-year postoperative, patients demonstrated increased ease in their routine physical activities and were more participatory in social life tasks. Individuals who underwent elective cardiac surgery took more than six weeks to detect notable improvement in functional status,

The efficacy of different combinations of undervine and inter-row treatments for managing a soil-dwelling orthopteran pest, weta （Hemiandrus sp.）, in vineyards was investigated over 2 seasons. This insect damages vine buds, thus reducing subsequent grape yield. The undervine treatments comprised pea straw mulch, mussel shells, tick beans [Viciafaba Linn. var minor （Fab）], plastic sleeves on vine trunks （treated control） and control （no intervention）, while inter-rows contained either the existing vegetation or tick beans. Treatments were arranged in a randomized complete block design with 10 replicates. Data were collected on weta densities, damage to beans and components of yield. The latter were numbers of bud laid down per vine, shoots per bud, clusters per shoot, grape bunches per vine, bunch weight and yield. The undervine treatments significantly affected all variables except the number of shoots per bud. In contrast, none of the variables was significantly affected by the inter-row treatments or their interaction with undervine treatments, apart from weta density. At the end of the experiment, weta density in the shell treatment was about 58% lower than in the control. As a result, there was about 39% significant yield increase in that treatment compared to the control. Although the undervine beans and sleeves treatments increased yield, there were no reductions in weta density. With undervine beans, the insect fed on the bean plants instead of vine buds. Thus, yield in that treatment was approximately 28% higher than in the control. These results demonstrate that simple agroecological management approaches can reduce above-ground damage by soil-dwelling insects.
