Accessory spleen-like masses in oncology patients:Are they always benign?

AIM:To assess retrospectively the significance of accessory spleen-like mass(ASLM) in oncology patients undergoing positron emission tomography/computed tomography(PET/CT).METHODS:The results of PET/CT of 913 patients(278 lymphoma;635 solid tumors) were *** number,size,location and attenuation of all ASLMs,and spleen attenuation,were *** fluorodeoxyglucose uptake was graded as normal(less than or equal to that in the liver) or representative of malignancy(more than in the liver).Follow-up PET/CT in patients with ASLM was reviewed when *** size and attenuation for spleen and ASLM were compared by unpaired Student's t *** χ2 and fisher's exact tests were used to compare ASLM frequency and uptake for lymphomatous and solid tumors,***:ASLM frequency was 14.8,with 152 ASLMs found in 135 *** attenuation was lower in ASLM compared with spleen by enhanced and nonenhanced CT(80.7 ± 20.4 HU vs 92.0 ± 14.4 HU,P < 0.0011 and 42.3 ± 9.0 HU vs 51.5 ± 6.3 HU,P < 0.0001,respectively).ASLM incidence was higher in lymphoma patients(56/278,20.1) than in those with solid tumors(56/278,20.1 vs 79/635,12.4,P = 0.0036).Pathological uptake was found in four(7.1) lymphoma patients but not in any patients with a solid tumor(P = 0.028) and it upstaged one patient with ***-up PET/CT within 3-16 mo was available in 54 of patients with *** regression was noted in all four pathological ASLMs on follow-up PET/CT after ***:In patients with lymphoma,ASLM can represent malignancy,and thus further characterization with PET/CT might be *** with neoplasia other than ASLM can be confidently diagnosed with accessory spleen.

Outcome of Assisted Reproduction Treatment in Women with Extremely Low Antimullerian Hormone (AMH) Levels

Purpose: To evaluate age related ovarian response to controlled ovarian hyperstimulation, clinical pregnancy and ongoing pregnancy rates beyond 20 weeks in women undergoing assisted reproduction treatment (ART) with antimullerian hormone (AMH) levels of 38 years of age. Non parametric variables were expressed as median (Interquartile range) and compared by Kruskal-Wallis test. Categorical variables were expressed as numbers with proportions (%) and compared by fisher’s exact test. Results: There was no statistical difference in body max index, level of antimullerian hormone (AMH), follicle stimulating hormone (FSH), dose of gonadotrophins used and cycles cancellation rate in two groups. Although number of oocytes retrieved (median 5), clinical pregnancy (18.4%) and ongoing pregnancy rate beyond 20 weeks (18.4%) was higher in group 1, there was no statistical difference between the two groups. There was one miscarriage in group 2. Conclusion: Women with extremely low-serum AMH levels can still have clinical pregnancy and ongoing pregnancies beyond 20 weeks after ART, though chances will be lower than women with normal ovarian reserve.

Comparison of Pentax HiLine and Olympus Lucera systems at screening colonoscopy

AIM:To compare the performance characteristics of Pentax HiLine(PHL)(with i-scan) and Olympus Lucera(OL) systems in a screening ***:Screening colonoscopies in asymptomatic guaiac faecal occult blood test-positive patients with PHL(n = 58) and OL(n = 425) colonoscopes were *** procedures were performed by a single *** used white-light endoscopy(WLE) on scope insertion and contrast/surface enhancement(i-scan 1) on withdrawal,and OL utilised WLE both on insertion and *** age,sex,instrument insertion and withdrawal times,nurse assessed patient comfort scores,midazolam and fentanyl doses,procedure completion and rates of lesion detection were recorded separately for each *** between the groups were made using either fisher's exact test(for dichotomous variables) or Mann-Whitney U test(for ordinal and continuous variables).RESULTS:Colonoscopy completion rates were similar in both groups:413/425(97.2%) for OL and 55/58(94.9%) for PHL(P = 0.24).For complete colonoscopies,the two groups were well matched for age,sex,colonoscope insertion times(mean 11.1 min in OL vs 11.6 min in PHL,P = 0.93) and normal colonoscopy withdrawal times(mean 15.6 min in OL vs 14.7 min in PHL,P = 0.2).Patients in the PHL group experienced a small increase in discomfort(mean patient comfort scores were 0.49 in the OL and 0.95 in the PHL group,P < 0.0001).While Fentanyl doses required were similar between groups(mean 57.5 μg in OL vs 61.4 μg in PHL,P = 0.13),slightly more Midazolam was required in the PHL group(mean 2.1 mg in OL vs 2.4 mg in PHL,P = 0.035).There was no difference in polyp(58% in OL vs 67% in PHL) or adenoma(49% in OL vs 56% in PHL) detection rates between the *** the total number of polyps and adenomas,nor the characteristics of these(including size,location or presence of advanced features) were different between the two ***:This study suggests that there is no advantage of either colonoscop

Obstetrical Emergencies in Ouahigouya Regional Teaching Hospital (Burkina Faso) in the Context of Insecurity Linked to Armed Groups: A Comparative Study between Patients from Precarious Security Zones and Those from Safe Areas

Introduction: Insecurity can be an obstacle to access to emergency obstetric and newborn care, that is why we proposed to study obstetrical emergencies in Ouahigouya Regional Teaching Hospital, a referral hospital in a region plagued by insecurity linked to armed groups. Method: This was an analytical cross-sectional study with prospective data collection over a 4-month period, from June 10 to October 10, 2020. Patients from precarious security areas were compared to those from safer areas. The Chi squared and fisher tests were used for comparison of variables. Results: Obstetric emergencies accounted for 38.62% of admissions, from which 25.59% came from precarious security zones. Age was similar in both comparison groups. However, patients from unsafe areas were more likely to reside in rural areas (p 0.001) and more likely to be in unpaid occupations (p 0.001). Prenatal visits were less frequent (p 0.01) and women were more often multigravidae (p 0.01) in the precarious security group of patients. Apart from the more frequent uterine rupture (p = 0.02) in the group from precarious security zones, diagnosed complications and maternal mortality were similar in the 2 groups, while perinatal mortality was higher in the group of patients from precarious security zones (p 0.01). Conclusion: The precarious security situation has negative consequences on maternal and perinatal morbidity and mortality. Further studies are needed for better understanding of these consequences, and improvement of health system resilience strategies, to reduce related maternal and fetal morbidity and mortality.

A Rapid Method for Measuring Seed Protein Content in Cowpea (<i>Vigna unguiculata</i>(L.) Walp)

Cowpea (Vigna unguiculata (L.) Walp), a legume crop that is grown in the worldwide, provides beneficial proteins for human consumption and animal feeding. In comparison, Rapid N analyzer as traditional method, has been widely used to measure protein content through the percentage of total nitrogen in the seed’s grounded powder. Near-Infrared Reflectance (NIR) has commonly been used to measure protein content in soybean seeds using whole grain without the need of seed grinding, which makes it possible to obtain fast results at a lower cost-per-analysis than the traditional combustion method. The specific objective of this study is to test a rapid method for measuring cowpea seed protein content by the NIR analyzer comparing to the traditional rapid N analyzer. A total of 240 cowpea genotypes were used in this study, including six seed coat colors, black, blackeye, browneye, cream, pinkeye, and red with 40 cowpea genotypes. The results showed that a linear relationship exists between the NIR analyzer and the Rapid N analyzer in the six different color groups. The correlation efficiency (r) between the seed protein contents from NIR and Rapid N was higher for pinkeye seed (r = 0.867), blackeye (0.771), cream (0.729), browneye (0.700), and red (0.623), respectively, but lower for black seeds, indicating that the NIR analyzer can be used to measure protein content for cowpea seeds with the five types of seed coat except black. Overview, the cowpea seed protein content measured from the NIR analyzer showed a little higher seed protein content. A series of regression models with different seed coat color have been built to adjust to protein content of colorful cowpea seeds from the NIR analyzer. But, it is not recommended to use for black color seeds due to a very low correlation efficiency (r) value with 0.184.

METHODS： A case-control study was conducted in adult individuals with T2DM associated to proliferative retinopathy or non-proliferative retinopathy from Oct. 2014 to Jun. 2015 from the Retina Department of the Asociation to Prevent Blindness in Mexico. The selected patients were adults with a diagnosis of T2DM ≥5y. All subjects had a comprehensive ocular examination and the classification of the retinopathy severity was made considering the Early Treatment Diabetic Retinopathy Study （ETDRS） standardization protocols. Genomic DNA was extracted from whole fresh blood. All samples were genotyped by qPCR for selected VEGF polymorphisms. Hardy-Weinberg equilibrium was calculated by comparing Chi-square values between the expected and the observed values for genotype ***： In total 142 individuals were enrolled, 71 individuals with T2DM and associated proliferative retinopathy and 71 individuals with non-proliferative retinopathy. One-sided fisher’s exact test was performed for rs3025021 [OR （95% CI）=0.44（0.08-2.2）; P=0.25] and rs2010963 [OR （95% CI）=0.63（0.25-1.6）; P=0.23]. The minor allelic frequencies obtained were 26% for rs3025021, 10% for rs3025035 and 61% for rs2010963.
