Effects of percutaneous varicocele repair on testicular volume:results from a 12-month follow-up

Varicocele is a comm on fin ding in men. Varicocele correcti on has bee n advocated for young patients with testicular hypotrophy, but there is a lack of morphofunctional follow-up data. We assessed whether percutaneous treatment of left varicocele is associated with testicular "catch-up growth" in the following 12 months by retrospectively reviewing data from an electronic database of 10 656 patients followed up in our clinic between 2006 and 2016. We selected all young adults (n derwe nt percuta neous treatment, had a minimum of 12 mon ths' ultraso und imagi ng follow-up, and had no other con ditions affecting testicular volume. One hundred and fourteen men (mean±standard deviation [s.d.] of age: 22.8 ± 5.4 years) met the inclusion and exclusion criteria. Left testicular hypotrophy (LTH), defined as a >20% difference between left and right testicular volume at baseline, was observed in 26 (22.8%) men. Participants with LTH (mean±s.d.: 14.5 ± 2.7 ml) had lower baseline testicular volume compared to those without LTH (mean±s.d.: 15.7 ± 3.8 ml;P= 0.032). Repeated measures mixed models showed a sign ifica nt in teraction betwee n LTH and time posttreatme nt when correcting for baseli ne left testicular volume (β= 0.114, 95% confidence interval [Cl]: 0.018-0.210, P=0.020), resulting in a catch-up growth of up to 1.37 ml per year (95% Cl: 0.221- 2.516). Age at intervention was also associated with reduced testicular volume (-0.072 ml per year, 95% Cl:-0.135--0.009;P = 0.024). Percutaneous treatment of left varicocele in young adults with LTH can result in catch-up growth over 1 year of follow-up. The reproductive and psychological implicati ons of these findings n eed to be confirmed in Ion ger and larger prospective studies.

Recurrence in node-negative advanced gastric cancer:novel findings from an in-depth pathological analysis of prognostic factors from a multicentric series

AIm To analyze the clinicopathological characteristics of patients with both node-negative gastric carcinoma and diagnosis of recurrence during follow-up. mETHODS We enrolled 41 patients treated with curative gastrectomy for p T2-4 a n0 gastric carcinoma between 1992 and 2010,who developed recurrence(Group 1). We retrospectively selected this group from the prospectively collected database of 4 centers belonging to the Italian Research Group for Gastric Cancer,and compared them with 437 p T2-4 a n0 patients without recurrence(Group 2). We analyzed lymphatic embolization,microvascular infiltration,perineural infiltration,and immunohistochemical determination of p53,Ki67,and HER2 in Group 1 and in a subgroup of Group 2(Group 2 bis) of 41 cases matched with Group 1 according to demographic and pathological characteristics. RESULTS T4 a stage and diffuse histotype were associated with recurrence in the group of p n0 patients. In-depth pathological analysis of two homogenous groups of p n0 patients,with and without recurrence during longterm follow-up(groups 1 and 2 bis),revealed two striking patterns: lymphatic embolization and perineural infiltration(two parameters that pathologists can easily report),and p53 and Ki67,represent significant factors for *** The reported pathological features should be considered predictive factors for recurrence and could be useful to stratify node-negative gastric cancer patients for adjuvant treatment and tailored follow-up.

Possible multiple Sources of the Strong 1117 Po Plain Earthquake, Inferred from the Plio-Quaternary Evolution of the northern Adriatic Area

The strongest documented seismic disaster ever occurred in the Po Plain area (January 3, 1117, m = 6.5) involved significant damage over a large zone. The genetic mechanism of such n>an event, most probably caused by more than one earthquake, is still an object of debate. Above all, the sources so far proposed cannot account for significant features of the observed macroseismic field. In this work, we suggest that the damage in the Verona zone was caused by the activation of a fault in the Lessini tectonic district, while damage in the central Po Plain may be related to a thrust fault located beneath the Giudicarie belt. The effects felt in northern Tuscany might derive from the seismic activation of the presumed SW-ward buried prolongation of the Giudicarie fault. The presence of such transpressional lithospheric discontinuity in the Adriatic domain since the upper Miocene and its reactivation (Pliocene-Pleistocenen style="font-family:Verdana;">) as a thrust zone is mainly suggested by an accurate analysis of the observed deformation pattern in the central mediterranean region. The proposed Giudicarie source may also help to explain the damage observed in the central Po Plain on December 25, 1222, which is not compatible with the seismic sources so far proposed.

Transient measurement of phononic states with covariance-based stochastic spectroscopy

We present a novel approach to transient Raman spectroscopy,which combines stochastic probe pulses and a covariance-based detection to measure stimulated Raman signals in alpha-quartz.A coherent broadband pump is used to simultaneously impulsively excite a range of different phonon modes,and the phase,amplitude,and energy of each mode are independently recovered as a function of the pump-probe delay by a noisy-probe and covariance-based *** experimental results and the associated theoretical description demonstrate the feasibility of 2D-Raman experiments based on the stochastic-probe schemes,with new capabilities not available in equivalent mean-value-based 2D-Raman *** work unlocks the gate for nonlinear spectroscopies to capitalize on the information hidden within the noise and overlooked by a mean-value analysis.

Prevalence and Incidence of Hand Osteoarthritis and Upper Limb Complaints in Patients with Knee Osteoarthritis. Correlations among Functionality, Grip Strength, Changes in Body mass Index and Symptoms among Patients in an Educational Osteoarthritis Program

Objective: To evaluate the prevalence of hand osteoarthritis (hOA) in a population with knee osteoarthritis (KOA) at baseline and one year following the administration of a multi-professional OA educational program correlating symptoms and changes in BmI with function questionnaires of the upper limbs and direct grip strength measurements. Design: Epidemiological study of the prevalence of hand OA in patients with knee osteoarthritis. The Stanford Health Assessment Questionnaire instruments (HAQ);Disabilities of the Arm, Shoulder and Hand* (DASH);grip strength;and finger pinch were utilized, and the upper limbs symptoms were verified at baseline and one year following the educational program. Results: The prevalence of hOA was 23.7% at baseline and 47.4% at one year (incidence of 31.8% per year). The HAQ indicated that patients who did not alter or increased their BmI experienced worsened global strength, whereas patients who reduced BmI improved global strength (p = 0.041). Patients with higher initial BmIs experienced less improvement in the HAQ (r = -0.148, p = 0.041). The DASH results improved, but the right and left tripod grip worsened in all patients, irrespective of BmI change (p mptoms at baseline and reassessment (p nch strength (right and left tripod and left pulp-pulp) was higher in patients without symptoms at baseline (p = 0.048, p = 0.045 and 0.033, respectively). Conclusions: The prevalence of hand OA increased for patients with OA undergoing an educational program irrespective of improved upper function and regardless of BmI change. Patients who decreased their BmI improved their global strength.

Glial cells are crucial for maintaining central nervous system(CnS)*** actively participate in immune responses,as well as form functional barriers,such as blood-brain barrier(BBB),which restrict the entry of pathogens and inflammatory mediators into the *** general,viral infections during the gestational period can alter the embryonic and fetal environment,and the related inflammatory response may affect neurodevelopment and lead to behavioral dysfunction during later stage of life,as highlighted by our group for Zika virus *** acute respiratory syndrome coronavirus-2(SARS-CoV-2)induces a cytokine storm and,during pregnancy,may be related to a more severe form of the coronavirus disease-19(COVID-19)and also to higher preterm birth ***-CoV-2 can also affect the CnS by inducing neurochemical remodeling in neural cells,which can compromise neuronal plasticity and synaptic ***,the impact of SARS-CoV-2 infection during pregnancy on postnatal CnS,including brain development during childhood and adulthood,remains *** group has recently highlighted the impact of COVID-19 on the expression of molecular markers associated with neuropsychiatric disorders,which are strongly related to the inflammatory ***,based on these relationships,we discussed the impact of SARS-CoV-2 infection either during pregnancy or in critical periods of neurodevelopment as a risk factor for neurological consequences in the offspring later in life,focusing on the potential role of glial ***,it is important to consider future and long-term public health concerns associated with SARS-CoV-2 infection during pregnancy.

Three-dimensional imaging reconstruction of the kidney’s anatomy for a tailored minimally invasive partial nephrectomy:A pilot study

Objective:The aim of the study was to evaluate three-dimensional virtual models(3DVms)usefulness in the intraoperative assistance of minimally-invasive partial nephrectomy in highly complex renal ***:At our institution cT1-2n0m0 all renal masses with Preoperative Aspects and Dimensions Used for an Anatomical classification score≥10 treated with minimally-invasive partial nephrectomy were considered for the present *** inclusion a baseline contrast-enhanced computed tomography in order to obtain 3DVms,the baseline and postoperative serum creatinine as well as estimated glomerular filtration rate values were *** patients,in which 3DVms were used to assist the surgeon in the planning and intraoperative guidance,were then compared with a control group of patients who underwent minimally-invasive partial nephrectomy with the same renal function assessments,but without *** logistic regression models were used to predict the margin,ischemia,and complication score ***:Overall,79 patients met the inclusion criteria and were compared with 143 complex renal masses without 3DVm *** 3DVm group showed better postoperative outcomes in terms of baseline-weighted differential estimated glomerular filtration rate(-17.7%vs.-22.2%,p=0.03),postoperative complications(16.5%vs.23.1%,p=0.03),and major complications(Clavien Dindo>III,2.5%vs.5.6%,p=0.03).At multivariable logistic regression 3DVm assistance independently predicted higher rates of successful partial nephrectomy(odds ratio:1.42,p=0.03).Conclusion:3DVms represent a useful tool to plan a tailored surgical approach in case of surgically complex *** can be used in different ways,matching the surgeon's needs from the planning phase to the demolitive and reconstructive phase,leading towards maximum safety and efficacy outcomes.

Long-Term Land Deformation monitoring Using Quasi-Persistent Scatterer (Q-PS) Technique Observed by Sentinel-1A: Case Study Kelok Sembilan

Located on the mountainous area, Kelok Sembilan flyover area in West Sumatra, Indonesia has a long history of land deformation, therefore monitoring and analyzing as continuously is a necessity to minimize the impact. notably, in the rainy season, the land deformation occurs along this area. The zone is crucial as the center of transportation connection in the middle of Sumatra. Quasi-Persistent Scatterer (Q-PS) Interferometry technique was applied for extracting information of land deformation on the field from time to time. not only does the method have high performance for detecting land deformation but also improve the number of PS point, especially in a non-urban area. This research supported by 90 scenes of Sentinel-1A (C-band) taken from October 2014 to november 2017 for ascending and descending orbit with VV and VH polarization in 5 × 20 m (range × azimuth) resolution. Both satellite orbits detected two critical locations of land deformation namely as zone A and Zone B, which located in positive steep slope where there is more than 500 mm movement in the Line of Sight (LOS) during acquisition time. Deformations in the vertical and horizontal direction for both zone, are 778.9 mm, 795.7 mm and 730.5 mm, 751.7 mm, respectively. Finally, the results were confirmed by ground truth data using Unmanned Aerial Vehicle (UAV) observation.

Comparison of YKL-39 and CHIT-1 expression during macrophages differentiation and polarization

The chitinase-like proteins YKL-39 (chitinase 3-like-2) and Chitortriosidase (CHIT-1) are members of the chitinases family. YKL-39 expression has been associated with osteoarthritis, whereas CHIT-1 activity is regarded as a biochemical marker of macrophage activation. So far, the physiological or pathological role of YKL-39 in the inflammation is still poorly understood. We compared YKL-39 and CHIT-1 modulation during monocyte to macrophage transition and polarization. Gene expression analysis was investigated by real-time PCR from mRnA of human monocytes obtained from buffy coat of healthy volunteers, from mRnA of polarized macrophages to classically activated macrophages (or m1), obtained by interferon-γ and lipopolysaccharide exposure, and from mRnA of alternatively activated macrophages (or m2) obtained by interleukin-4 exposure. We demonstrated different variations of YKL-39 and CHIT-1 production during macrophages polarization. CHIT-1 levels gradually increase in the course of the time with a peak of expression between the fifth and the seventh day of culture. In contrast, YKL-39 expression was unaltered in the diverse stage of Hmms differentiation, but increased significantly in m1 polarized macrophages and reverted to base levels in m2 polarized macrophages. These findings indicated that the function of YKL-39 is much more restricted and selective than that exerted by CHIT-1.
