AIM To describe our institutional experience with conversion from intravenous(IV) fentanyl infusion directly to enteral methadone and occurrence of withdrawal in critically ill mechanically ventilated children exposed to prolonged sedation and *** With Institutional Review Board approval,we retrospectively studied consecutively admitted invasively mechanically ventilated children(0-18 years) sedated with IV fentanyl infusion > 5 d and subsequently converted directly to enteral *** were obtained onsubject demographics,illness severity,daily IV fentanyl and enteral methadone dosing,time to complete conversion,withdrawal scores(WAT-1),pain scores,and need for rescue *** were classified as rapid conversion group(RCG) if completely converted ≤ 48 h and slow conversion group(SCG) if completely converted in > 48 *** outcome was difference in WAT-1 scores at 7 *** outcomes included differences in overall pain scores,and differences in daily rescue *** Compared to SCG(n = 21),RCG(n = 21) had lower median WAT-1 scores at 7 d(2.5 vs 5,p = 0.027).Additionally,RCG had lower overall median pain scores(3 vs 6,p = 0.007),and required less median daily rescue opioids(3 vs 12,p = 0.003) than *** starting daily median methadone dose was 2.3 times the daily median fentanyl dose in the RCG,compared to 1.1 times in the SCG(p = 0.049).CONCLUSION We observed wide variation in conversion from IV fentanyl infusion directly to enteral methadone and variability in withdrawal in critically ill mechanically ventilated children exposed to prolonged *** those children who converted successfully from IV fentanyl infusion to enteral methadone within a period of 48 h,a methadone:fentanyl dose conversion ratio of approximately 2.5:1 was associated with less withdrawal and reduced need for rescue opioids.

Multiple myeloma(MM)is a hematological malignancy characterized by the accumulation of immunoglobulin-secreting clonal plasma cells at the bone marrow(BM).The interaction between MM cells and the BM microenvironment,and specifically BM mesenchymal stem cells(BM-MSCs),has a key role in the pathophysiology of this *** data support the idea that BM-MSCs not only enhance the proliferation and survival of MM cells but are also involved in the resistance of MM cells to certain drugs,aiding the progression of this hematological *** relation of MM cells with the resident BM-MSCs is a two-way *** modulate the behavior of BM-MSCs altering their expression profile,proliferation rate,osteogenic potential,and expression of senescence *** turn,modified BM-MSCs can produce a set of cytokines that would modulate the BM microenvironment to favor disease *** interaction between MM cells and BM-MSCs can be mediated by the secretion of a variety of soluble factors and extracellular vesicles carrying microRNAs,long non-coding RNAs or other ***,the communication between these two types of cells could also involve a direct physical interaction through adhesion molecules or tunneling ***,understanding the way this communication works and developing strategies to interfere in the process,would preclude the expansion of the MM cells and might offer alternative treatments for this incurable disease.

BACKGROUND Non-responsive celiac disease(NRCD) is defined as the persistence of symptoms in individuals with celiac disease(CeD) despite being on a gluten-free diet(GFD). There is scant literature about NRCD in the pediatric *** To determine the incidence, clinical characteristics and underlying causes of NRCD in *** Retrospective cohort study performed at Boston Children’s Hospital(BCH). Children positive serology and duodenal biopsies compatible with Marsh Ⅲ histology between 2008 and 2012 were identified in the BCH’s Celiac Disease program database. Medical records were longitudinally reviewed from the time of diagnosis through September 2015. NRCD was defined as persistent symptoms at 6 mo after the initiation of a GFD and causes of NRCD as well as symptom evolution were detailed. The children without symptoms at 6 mo(responders) were compared with the NRCD group. Additionally, presenting signs and symptoms at the time of diagnosis of CeD among the responders and NRCD patients were collected and compared to identify any potential predictors for NRCD at 6 mo of GFD *** Six hundred and sixteen children were included. Ninety-one(15%) met criteria for NRCD. Most were female(77%). Abdominal pain [odds ratio(OR) 1.8 95% confidence interval(CI) 1.1-2.9], constipation(OR 3.1 95%CI 1.9-4.9) and absence of abdominal distension(OR for abdominal distension 0.4 95%CI 0.1-0.98) at diagnosis were associated with NRCD. NRCD was attributed to a wide variety of diagnoses with gluten exposure(30%) and constipation(20%) being the most common causes. Other causes for NRCD included lactose intolerance(9%), gastroesophageal reflux(8%), functional abdominal pain(7%), irritable bowel syndrome(3%), depression/anxiety(3%), eosinophilic esophagitis(2%), food allergy(1%), eating disorder(1%), gastric ulcer with Helicobacter pylori(1%), lymphocytic colitis(1%), aerophagia(1%) and undetermined(13%). 64% of children with NRCD

Objectives:To determine immediate and long-term follow-up of transcatheter closure of patent ductus arteriosus(pDA)in ***:National antibiotic prophylaxis(Ap)guideline for infective endocarditis changed after 2009,the effect on practice of pDA closure is ***:Observational single center study analyzing follow-up of pDA closure comparing two time periods before(2002–2009)and after(2010–2019)changes in Ap ***:332 patients(68.1%female),median(interquartile range)age 3.0 years(1.5–5.7)and body weight 14.0 kg(10.0–19.3),were *** morphology was conical type A(50.3%),window type B(1.2%),tubular type C(40.1%),complex type D(2.1%),elongated type E(0.9%)and other(5.4%).Minimal pDA diameter and length were 1.9 mm(1.3–2.5)and 8.0 mm(6.2–10.2).pDA was closed using coils(56.3%),Amplatzer Duct Occluders(41.9%)and others(1.8%).Complete closure rate was 61.1%at catheter intervention,72.3%on day 1,87.7%after 6 months and 98.4%at last follow-up on *** complication rate(severity level 3)was 4.2%and major complication rate(severity level 4)0.3%,with no catastrophic complications(severity level 5).Annual pDA closure rate declined in the second time period(22.6/year vs.15.5/year,p=0.018),pDA size increased(1.6 mm vs.2.0 mm,p=0.002)and proportion of coils decreased(72.4%vs.37.1%,ppDA is associated with excellent closure rates during follow-up(>98%)and only a small number of complications leading to reintervention or *** in Ap guidelines changed indication for and practice of pDA closure.

Objectives:Factors influencing results of balloon valvuloplasty(BVp)of pulmonary valve stenosis(pS)in children are ***:BVp has become the standard of care for pS,medium-term results are not uniform and depend on various ***:We analysed the medium-term results of BVp of pS in children in an observational,single centre *** for additional procedure was defined as outcome after initial ***:We included 143 children(83 female)at a median(IQR)age of 2.6(0.26–9.24)months and body weight of 5(3.4–8)kg at BVp with a follow–up of 5.04(1.6–10.2)*** used balloon size of 10(9–14)mm and maximal balloon pressure of 4(3.5–10)atm,resulting in balloon–to–pulmonary annulus ratio of 1.28(1.2–1.4).Systolic pressure gradient of pS was reduced with BVp(43.5 mmHg vs.14.0 mmHg,pphy(68.0 mmHg vs.25.0 mmHg,ppost *** BVp with associated supravalvular pS resulted in a relevant reduction of systolic pressure gradient in 23 of 31 patients(74.2%).Early additional procedure was necessary in 14 patients(9.8%)after 0.2(0.1–0.7)years due to residual pS(n=13)and infective endocarditis(n=1).Factors for additional procedures were associated supravalvular pS with a higher residual pressure gradient,but not genetic *** further follow–up of 5.04(1.6–10.2)years no further additional procedures were ***:pulmonary BVp of native pulmonary valve stenosis leads to excellent medium-term results,even in 3 of 4 infants with associated supravalvular obstruction sufficient pressure relief can be obtained.

The salinity stress is one of the most relevant abiotic stresses that affects the agricultural *** present study was performed to study the improvement of the salt tolerance of tomato plants which is known for their susceptibility to salt *** present study aimed to assess to what extent strain Azospirillum brasilense(N040)and Saccharomyces cerevisiae improve the salt tolerance to tomato plants treated with different salt *** inoculant strain ***(N040)was previously adapted to survive up to 7%NaCl in the basal media.A greenhouse experiment was conducted to evaluate the effect of this inoculation on growth parameter such as:plant height,root length,fresh and dry weight,fruits fresh weight,chlorophyll content,proline and total soluble sugar in tomato plants under salt stress *** results revealed that co-inoculation of Azospirillum brasilense(N040)and Saccharomyces cerevisiae significantly increased the level of proline(8.63 mg/g FW)and total soluble sugar(120 mg/g FW)of leaves under salinity condition comparing to non-inoculated plants(2.3 mg/g FW and 70 mg/g FW,respectively).plants co-inoculated with adapted strain of *** and *** showed the highest significant(pplant),plant high(115 cm)and roots length(52.6)compared whit un-inoculated control plants(42 g/pant,43.3 cm and 29.6 cm,respectively).In contrast,Na^(+)ion content was significantly decreased in the leaves of salt stressed plants treated with the ***(N040)and ***,the results showed that dual benefits provided by both ***(N040)and *** can provide a major way to improve tomato yields in saline soils.

Background:Data regarding tuberculosis(TB)treatment outcomes,proportion of TB/HIV co-infection and associated factors have been released at different TB treatment facilities in Ethiopia and elsewhere in the world as part of the auditing and surveillance ***,these data are missing for the TB clinic offering directly observed treatment short-course(DOTs)at Debre Tabor General Hospital(DTGH).Methods:The authors analysed the records of 985 TB patients registered at the DTGH from September 2008 to December *** on patients’sex,age,type of TB,and treatment outcomes were extracted from the TB treatment registration *** treatment outcome of patients was categorized according to the National TB and Leprosy Control program guidelines:cured,treatment completed,treatment failed,died,and not evaluated(transferred out and unknown cases).Results:Around half of the registered patients were males(516,52.4%).In terms of TB types,381(38.7%),241(24.5%),and 363(36.9%)patients had smear-negative pulmonary TB,smear-positive pulmonary TB,and extra pulmonary TB,*** hundred and seventy-two patients(90.1%)had successful treatment outcomes(cured and treatment completed),while 74 patients(9.9%)had unsuccessful treatment outcomes(death and treatment failure).TB treatment outcome was not associated with age,sex,type and history of TB,or co-infection with HIV(p>0.05).The proportion of TB/HIV co-infection was at 24.2%,and these were found to be significantly associated with the age groups of 25-34,35-44 and≥65 years:(aOR:0.44;95%CI:0.25-0.8),(aOR:0.39;95%CI:0.20-0.70),(aOR:4.2;95%CI:1.30-12.9),***:The proportion of patients with successful treatment outcomes was above the World Health Organization target set for Millennium Development Goal of 85%and in line with that of the global milestone target set at>90%for *** higher proportions of transfer-out cases were recorded in the present ***,the proportion of TB/HIV co-i
